The management of Peyronies's disease should include an exact pretherapeutic staging. Diagnostic methods like ultrasound imaging, X-ray using "mammography technique" and magnetic resonance imaging are modalities of choice. The presence of calcified plaques is one of the few unequivocal indications for surgery of Peyronie's disease. The mean size of plaques described in the literature is 1.5-2 cm whereby it may vary from 1 mm all along the penis. The authors treated successfully an unusually large calcified plaque 7 x 0.8 cm with excellent postoperative results--restoration of the patient's full sexual activity which was not possible before operation.

The radical surgical option we propose for Peyronie's disease consists in removing the sclero-hyalinotic focus of disease and replacing it by an autologous dermal graft taken from the upper outer thigh area. OBJECTIVE: To evaluate the effectiveness of plication surgery for patients with congenital and acquired curvature of the penis.
PATIENTS AND METHODS: Forty-eight patients with congenital curvature of the penis and 57 with induratio penis plastica were treated by plication of the tunica albuginea. Their average age was 23 and 56 years, respectively. The operation was performed under local anaesthesia, using 2/0 Prolene and a specially designed suture. The average observation time in the two groups was 24 and 20 months, respectively.

Clinical investigation is still fundamental in the diagnosis of La Peyronie's disease. In this study 36 patients underwent clinical investigation and ultrasound in basal conditions and during drug-induced erection, and the results were compared. In basal condition US does not provide much useful supplementary informations. During drug-induced erection US accurately visualizes plaque site and dimensions and anatomical abnormalities about corpora cavernosa, albuginea, septum and dorsal vessels.
