The complement fixation test, microprecipitation, leukocyte migration inhibition, leukocyte agglomeration phenomenon, direct and indirect immunofluorescence were used to demonstrate that chronic prostatovesiculitis associated with Reiter's disease may be of immune-induced nature. This requires a differentiated approach to the choice of the treatment policy. The methods of the treatment for the urogenital inflammatory focus in men suffering from Reiter's disease are provided. Forty-six patients were investigated by ultrasonography in order to assess the state of the tissues directly adjacent to the prostate in patients with chronic prostatitis. Inflammatory infiltrations in paraprostatic fat, seen as irregularly shaped rarefication foci with a vague outline were detected in 16 (33.3%) and termed as paraprostatitis. The inflammatory infiltration involved seminal vesicles in 56.3% of the patients. We report on a case of lymphogranuloma venereum in a 40-year-old man who acquired the disease while on vacation in Tunisia. After 4 years, a fibrosing, fistulating inflammation developed in an atypical distribution that involved the scrotum, perineum, and left gluteal and upper femoral regions; the disease progressed because of insufficient therapy. The diagnosis was established on serological examination, and treatment with doxycycline (2 x 100 mg for 4 weeks) improved the chronic inflammatory process.

Fifty-four patients complaining of acute painful scrotal swelling were submitted to urgent radionuclide scrotal imaging. Clinical and surgical findings were compared with the results of radionuclide studies. Radionuclear scrotal imaging is an easy rapid none invasive investigation with high accuracy in inflammatory disease of the scrotal content. Two falsely negative examinations confirmed by surgical exploration, however, warn against relying on this method as a single test in the diagnosis of acute testicular torsion.

The acute scrotum remains a difficult clinical challenge because of numerous etiologies including inflammatory processes, testicular torsions, traumas, and tumors that can justify early surgery. A case of acute scrotal swelling with pain in a 4-year-old boy suffering from Schoenlein-Henoch purpura is reported. Scrotal abnormalities are difficult to assess using clinical criteria alone. Ultrasound provides an accurate means of demonstrating the scrotal contents so that appropriate therapy may be instituted. Thirty-six examples of inflammatory and neoplastic disease of the scrotum were evaluated with ultrasound. Analysis of the images suggests that sonographic differentiation of these conditions is possible in certain instances. The ability to separate testicular from extratesticular pathology is quite accurate. In some cases, neoplastic disease of the testis can be differentiated from malignancy, although surgical intervention is often required for diagnosis.

The incidences of various diseases responsible for the occurrence of a scrotal mass have been reported. The testicular scan was used infrequently but was 100 percent accurate in differentiating inflammatory masses from torsion. Fifty men with cytologically confirmed chronic prostatitis were treated for 3 months by consecutive p.o. administration of doxycycline, sulfamethoxazole/trimethoprim, and cephalexin. The subjective symptoms, palpatory findings, secretory capacity of the accessory genital glands (values of acid phosphatase and fructose in the seminal plasma) were evaluated. Bovid herpesvirus 2 infection was studied in calves exposed to the virus by intradermal inoculation of the skin of the left cheek or by nasal spray. A Doppler flowmeter was used to evaluate an acute scrotum in 23 children. A diminished or absent flow was noted in cases of torsion of the testis. Orchitis, epididymitis and torsion of the testicular appendix were accompanied with an increased blood flow. Inflammatory and ischemic lesions could be detected and in all operated cases the diagnosis proved to be correct. Unnecessary operations in cases of torsion of the testicular appendix can be avoided with this accurate non-invasive method.
