From the operating room to the cloud: the evolving continuum from live surgical broadcasting to remote tele-surgery.

In the last four decades, the transmission of a live surgical operation to a remote audience has evolved from closed-circuit educational broadcast into a fully remote, robot-assisted surgery performed across different continents. This review maps that journey systematically, defining a six-rung Digital Surgical Spectrum - from passive live broadcast to artificial-intelligence-augmented autonomous surgery - grounding each stage in published clinical evidence across multiple surgical domains. We demonstrate that each rung of this spectrum is no longer theoretical. Live broadcasts have reached au

Specification and Causal Pathway Diagramming of Implementation Strategies to Support Development of Access-Enhancing Injectable HIV Treatment Programmes at Eight Diverse Health Clinics Across the United States.

INTRODUCTION: Long-acting injectable antiretroviral therapy (LAI ART) represents a transformative development in HIV care because it may mitigate longstanding barriers that contribute to gaps in health outcomes. Providing LAI ART to people not consistently on HIV treatment or lacking treatment options requires intentional programme planning to overcome implementation barriers, without which only well-resourced patients benefit, worsening health outcome gaps. The ALAI UP Project delivered clinic-level implementation strategies to support eight clinics across Ending the HIV Epidemic jurisdiction

Unveiling the antiviral mechanism of A4S7: a synthetic antimicrobial peptide targeting dengue and other mosquito-borne enveloped virus infections.

Dengue virus (DENV), an enveloped mosquito-borne flavivirus, is the causative agent of dengue and prevalent in tropical and subtropical regions. Severe cases can progress to dengue hemorrhagic fever or shock syndrome, posing significant public health challenges. In the absence of specific antiviral drugs, antimicrobial peptides (AMPs) have emerged as promising therapeutic agents against DENV due to their broad-spectrum antibacterial and antiviral properties. This study investigated the synthetic AMP A4S7, which effectively inhibited DENV-2 RNA and protein expression and reduced viral progeny p

Impact of ultra-low tidal volume ventilation on 1-year functional outcome in COVID-19 ARDS patients. A long-term follow-up analysis of a randomized controlled trial.

BACKGROUND: Ultra-low tidal volume ventilation (ULTV) aims to minimize ventilator-induced lung injury in acute respiratory distress syndrome (ARDS), but is associated with an increase in arterial carbon dioxide (PaCO2). This follow-up study aimed to evaluate whether ULTV impacted long-term pre-specified secondary outcomes in the VT4COVID trial. METHODS: VT4COVID was an open-label multicenter randomized controlled superiority trial performed in ten French intensive care units. Eligible patients were COVID-19 ARDS patients with arterial oxygen pressure (PaO2) over inspired oxygen fraction (FiO2

Simulation-Based Bayesian Predictive Probability of Success for Interim Monitoring of Clinical Trials With Competing Event Data: Two Case Studies.

Bayesian predictive probabilities of success (PPoS) use interim trial data to calculate the probability of trial success. These quantities can be used to optimise trial size or to stop for futility. In this paper, we describe a simulation-based approach to compute the PPoS for clinical trials with competing event data, for which no specific methodology is currently available. The proposed procedure hinges on modelling the joint distribution of time to event and event type by specifying Bayesian models for the cause-specific hazards of all event types. This allows the prediction of outcome data

Changes in Medication Use During Medicaid Continuous Enrollment and Unwinding.

IMPORTANCE: During the COVID-19 pandemic, Medicaid enrollment increased because states suspended routine eligibility determinations. After this continuous enrollment provision ended in April 2023, millions of US individuals lost Medicaid coverage. OBJECTIVE: To measure how the unwinding of Medicaid enrollment was associated with changes in patients' use of health services, such as prescription medications. DESIGN, SETTING, AND PARTICIPANTS: A cross-sectional study was carried out using interrupted time series analysis to compare changes in quarterly Medicaid enrollment and prescription medic

A Prospective, Cross-Sectional, Multicenter Study Evaluating a Multi-Target Blood Protein in Vitro Diagnostic Test for Colorectal Cancer.

PURPOSE: Earlier detection of colorectal cancer (CRC) can improve survival rates. A simple, effective blood test may help improve screening participation. The multi-target blood protein (MTBP; ColoSTAT®) test and algorithm uses concentrations of five protein biomarkers of CRC and patient's sex and age to generate a CRC likelihood score. We compared the performance of the MTBP test in detecting CRC to colonoscopy, the 'gold standard'. METHODS: This cross-sectional, multicenter study enrolled participants into two cohorts: participants recently colonoscopically diagnosed with CRC and progressin

Primary gastric EBV-positive cytotoxic-molecule-negative T-cell lymphoma with TET2 multihit mutations: a case report.

Extranodal NK/T-cell lymphoma (ENKTL) is an Epstein-Barr virus (EBV)-positive lymphoma of the NK- or cytotoxic T-cell lineage. According to the World Health Organization Classification of Hematolymphoid Tumors, Fifth Edition (WHO-HAEM5), EBV-positive cases lacking cytotoxic molecules cannot be classified as ENKTL. To the best of our knowledge, this is the first documented case of primary gastric EBV-positive T-cell lymphoma lacking the expression of cytotoxic molecules. A 76-year-old woman initially presented with a disease confined to the stomach, with subsequent contiguous extension into the

The global prevalence of eating disorders in children and young people: a systematic review and meta-analysis.

The increase in eating disorder (ED) presentations among children and young people (CYP) during the Covid-19 pandemic represents a global health concern, given the associated morbidity and mortality associated with these conditions. We conducted a meta-analysis to estimate the worldwide pooled prevalence of EDs in CYP at a population level. We searched MEDLINE, EMBASE, PsycINFO and LILACS for all English-language studies reporting population level ED prevalence data between January 2013 to February 27th, 2024. The primary outcome was overall prevalence of EDs. We used a random-effects model fo

Synergistic adverse prognostic impact of CD30 and Epstein-Barr virus in peripheral T-cell lymphoma: CD30 identifies a high-risk subgroup in Epstein-Barr virus-encoded RNA -positive patients.

Patients with Epstein–Barr virus–encoded RNA (EBER)-positive Peripheral T-cell lymphoma (PTCL) exhibit poor survival. Current prognostic scores lack efficacy in this group, necessitating more specific biological markers. This multicenter retrospective cohort study evaluated the prognostic significance of CD30 status in EBER-positive angioimmunoblastic T-cell lymphoma (AITL) and PTCL-not otherwise specified (PTCL-NOS) patients. Receiver operating characteristic (ROC) analysis, Kaplan–Meier survival curves, and Cox regression analysis were employed to assess the predictive value of CD30 expressi

Identifying the optimal rapid antigen test for screening and determining the end of isolation: A modeling study.

During the COVID-19 pandemic, rapid antigen tests (RATs) were used to detect infections, improving the effectiveness of targeted non-pharmaceutical interventions (NPIs). However, RATs based on either nasal swab or saliva samples were used, raising the question as to which type of RAT is most effective at detecting viral infections. Here, we develop a model-driven computational framework to assess different RATs and identify the most suitable test for a specified purpose, such as infection screening or determining the end of isolation, for various viral infections. Using symptomatic COVID-19 ca

International guidelines for the management of healthcare workers living with HIV: a systematic comparative review.

BACKGROUND: Despite strong evidence that sustained HIV viral suppression eliminates transmission risk ("undetectable equals untransmittable", U=U), national approaches to managing healthcare workers (HCWs) living with HIV remain heterogeneous. Because such cases are rare but ethically and legally sensitive, clear guidance is needed to ensure consistent patient safety standards while avoiding unnecessary restrictions and stigma. METHODS: A systematic comparative review of international guidance on the management of HCWs living with HIV was performed. A broad PubMed search identified peer-revie

Impact of COVID-19 Lockdown on Psychopathology at the Onset of Eating Disorders.

OBJECTIVE: Previous research has shown that the COVID-19 pandemic worsened eating-disorder (ED) symptoms, but most studies have focused on inpatients and compared only two periods: pre-COVID and the period following the pandemic's onset. Outpatient populations, particularly those with binge eating disorder (BED) and other specified feeding or eating disorders (OSFED), have been underrepresented. It remains unclear whether the pandemic influenced the clinical presentation at ED onset. This study aimed to examine differences in general and eating-disorder-related psychopathology at first present
