SSTR2 expression in EBV-positive and EBV-negative lymphomas.

BACKGROUND: Somatostatin receptor 2 (SSTR2) protein expression is aberrantly upregulated in various tumors and can be visualized using scintigraphy or radiological techniques and therapeutically targeted using octreotide-based molecules. PURPOSE: In this study, we assessed SSTR2 expression in the largest cohort of hematopoietic proliferations examined to date and interrogated the relationship between EBV and SSTR2 in EBV-associated lymphomas as a possible mechanism for SSTR2 upregulation. METHODS: We retrospectively identified 407 cases of lymphoma from Guatemala, a country with a high preva

International guidelines for the management of healthcare workers living with HIV: a systematic comparative review.

BACKGROUND: Despite strong evidence that sustained HIV viral suppression eliminates transmission risk ("undetectable equals untransmittable", U=U), national approaches to managing healthcare workers (HCWs) living with HIV remain heterogeneous. Because such cases are rare but ethically and legally sensitive, clear guidance is needed to ensure consistent patient safety standards while avoiding unnecessary restrictions and stigma. METHODS: A systematic comparative review of international guidance on the management of HCWs living with HIV was performed. A broad PubMed search identified peer-revie

Impact of COVID-19 Lockdown on Psychopathology at the Onset of Eating Disorders.

OBJECTIVE: Previous research has shown that the COVID-19 pandemic worsened eating-disorder (ED) symptoms, but most studies have focused on inpatients and compared only two periods: pre-COVID and the period following the pandemic's onset. Outpatient populations, particularly those with binge eating disorder (BED) and other specified feeding or eating disorders (OSFED), have been underrepresented. It remains unclear whether the pandemic influenced the clinical presentation at ED onset. This study aimed to examine differences in general and eating-disorder-related psychopathology at first present

Adult T-Cell Leukemia/Lymphoma and Targeted Maternal Screening.

IMPORTANCE: Adult T-cell leukemia/lymphoma (ATLL) is a rare but highly aggressive malignant disease caused by lifelong infection with human T-cell leukemia virus type 1 (HTLV-1), typically acquired through mother-to-child transmission. In southwestern Japan, the world's highly documented HTLV-1 endemic region, maternal HTLV-1 screening and breastfeeding counseling have reduced vertical transmission. Because of the long latency between infection and disease, reductions in ATLL burden are expected to occur over time. In the US, HTLV-1 screening is limited to blood donors, while mother-to-child t

An online singing-based breathing and wellbeing programme (ENO Breathe) in people with long COVID breathlessness in the UK: a cohort study.

BACKGROUND: Post-COVID-19 condition (also known as long COVID) breathlessness is a common, complex, and frequently debilitating problem for which few evidence-based interventions exist. A previous randomised trial found that participation in an online 6-week breathing and wellbeing programme (ENO Breathe), using singing techniques, was associated with improvements in health-related quality of life (HRQOL) and breathlessness. We aimed to assess the impact of this intervention outside a trial setting. METHODS: In this cohort study, participants were referred from 51 UK-based National Health Ser

Viral infections alter the human salivary volatilome.

The volatile metabolome, or volatilome, contains a wealth of information regarding individual health. Two parallel approaches can detect patterns of these volatile metabolites: chemometric analyses employing gas chromatography-mass spectrometry (GC-MS) or "noses" (trained animals or so-called electronic noses). While "noses" are exquisite odor discrimination tools, chemometric analyses are superior for determining the chemical identity of odorants related to disease conditions. Without identifying compounds altered by disease, it is difficult to uncover the responsible mechanism(s) and develop

Unveiling post-vaccination proteomic signatures in SARS-CoV-2 infection-naïve individuals associated with Omicron breakthrough infections.

BACKGROUND: Given the persistence of the SARS-CoV-2 virus, it is important to understand the proteome associated with breakthrough infections among COVID-19 vaccinated individuals. METHODS: We conducted a nested case-control study within the frontline worker HEROES-RECOVER cohorts to specify a study population of SARS-CoV-2 infection-naïve participants who had a third dose of COVID-19 origin strain WA-1 monovalent mRNA vaccine from August 2021 to January 2022. We compared serum proteomic profiles for those who subsequently experienced Omicron breakthrough infections with those of matched cont

Catch-up and life-course vaccination policies for adult and adolescent migrants in low-and-middle income countries: A policy analysis.

BACKGROUND: Adolescent and adult migrants in low- and middle-income countries (LMICs) are often under-immunised because of disrupted vaccination in childhood and barriers to care, increasing the risk of vaccine-preventable diseases. WHO's Immunization Agenda 2030 strengthens calls for life-course immunisation and inclusion of marginalised groups in catch-up initiatives (including Td/Tdap, IPV, MR/MMR, HepB, HPV). Despite evidence from Europe that migrants are frequently missed, it is unclear how far LMIC policies include adolescents and adults beyond child-focused programmes. We analysed catch

Projected public health benefits of a hypothetical HSV-1 vaccine in the United States: A mathematical modeling analysis.

Herpes simplex virus type 1 (HSV-1) infection is highly prevalent worldwide and causes oral, genital, and neonatal herpes. Several HSV vaccine candidates are currently in development. This study used mathematical modeling to estimate the potential population-level impact of HSV-1 vaccination strategies in the United States. Infant vaccination with 70% efficacy and lifelong protection introduced in 2030 reduced oral and genital incidence rate by 47.3 and 58.3% by 2075, respectively, with annual new infections declining by 41.2 and 53.5%, corresponding to 28.5 million cumulative infections avert

Trends in Diarrhea-associated Mortality in Children Less Than 5 Years of Age in the United States, 2005-2024.

BACKGROUND: The introduction of rotavirus vaccines in the United States in 2006 and 2008 has had large impacts on reducing rotavirus- and diarrhea-associated healthcare utilization. We evaluated changes in diarrhea-associated mortality from 2005 to 2024 among children 1-59 months of age. METHODS: We examined diarrhea-associated deaths, identified by International Classification and Diagnostic codes related to diarrhea listed anywhere on the death certificate, reported to the National Center for Health Statistics from 2005 to 2024. Diarrhea-associated death rates were calculated using national

Generalized Local Kendall's τ: a novel framework for uncovering nonlinear local dependence.

Identifying patient subgroups is increasingly vital for advancing personalized and precision medicine, where patient heterogeneity plays a central role. Subgroups can be revealed by examining how predictors depend on each other. However, traditional global dependence measures, such as Kendall's τ, often overlook localized and nonlinear relationships, potentially missing critical patterns that define subpopulations. Existing local dependence measures are typically constrained to linear structures or lack flexibility in capturing complex local behaviors. We propose a novel, intuitive, and highly

Heparin formulation and thromboprophylaxis intensity in critically ill patients: Results from the ESICM UNITE registry.

PURPOSE: Decisions regarding heparin formulation and thromboprophylaxis intensity are central to intensive care practice, yet the optimal strategy remains uncertain. We evaluated associations between anticoagulation strategies and three primary outcomes - life-threatening haemorrhage, thromboembolic events, and transfusion requirement - in a large international critically ill COVID-19 cohort. METHODS: Two analyses used variables pre-specified in the ESICM UNITE-COVID registry. First, outcomes were compared between unfractionated heparin (UFH) and low-molecular-weight heparin (LMWH) among pati

Shifting neuropsychiatric diagnoses across the antiretroviral therapy era at an urban Ugandan HIV centre: a descriptive analysis of routinely collected electronic health records, 2017 to April 2026.

BACKGROUND: As antiretroviral therapy (ART) coverage has matured in sub-Saharan Africa, the clinical profile of people living with HIV has shifted from acute opportunistic disease toward chronic neuropsychiatric morbidity. Few high-volume African HIV programmes have systematically described the full neuropsychiatric diagnostic architecture captured in their routine electronic records. We characterised the neuropsychiatric diagnoses recorded at Mildmay Uganda, an urban HIV centre serving approximately 14,500 active patients. METHODS: We conducted a descriptive analysis of routinely collected,
