Validation of a novel prognostic staging system for breast cancer incorporating age at diagnosis.

PURPOSE: Age at diagnosis is an independent prognostic variable for breast cancer-specific and overall survival (OS). We previously developed a novel prognostic staging system that incorporates age and demonstrated refined risk stratification compared with the current American Joint Committee on Cancer (AJCC) staging schema. We now aim to externally validate this staging system. METHODS: The National Cancer Database was used to identify adult females diagnosed with invasive breast cancer from 2010-2015. Women with prior malignancy, unknown vital status, or unknown AJCC clinical prognostic sta

Discovery of ZJC-11 as a Novel Selective CDK7 Inhibitor for Treating Triple-Negative Breast Cancer by Inducing Cell Senescence.

The development of cyclin-dependent kinase 7 (CDK7) inhibitors represents a promising therapeutic strategy for triple-negative breast cancer (TNBC). Herein, we designed and synthesized 21 novel CDK7-targeted small molecules and identified ZJC-11 as a potent lead candidate. ZJC-11 demonstrated significant antiproliferative activity against TNBC both in vitro and in vivo, with reduced toxicity compared to the reported CDK7 inhibitor THZ1. Molecular docking, Kinact/KI tests, kinase selectivity profiling, and pharmacokinetic studies confirmed that ZJC-11 selectively and covalently targets CDK7 wit

Discovery of Deuterated 3,4-Dihydro-benzo[e][1,4]diazepine-2,5-dione Derivatives as Potent, Selective and Orally Bioavailable ROCK2 Inhibitors.

Rho-associated coiled-coil-containing kinase 2 (ROCK2) is a key regulator of cellular motility and invasion and represents a promising therapeutic target for metastatic breast cancer. Through systematic structure-activity relationship (SAR) studies, we designed and synthesized a novel series of 1-methyl-3,4-dihydro-1H-benzo[e][1,4]diazepine-2,5-dione derivatives as selective ROCK2 inhibitors. Strategic deuterium incorporation during structural optimization was employed to enhance pharmacokinetic profiles. This effort led to the identification of compound 22d, a highly potent deuterated analog

A Novel Dual-Targeting PROTAC Overcomes Endocrine Resistance by Engaging Orthosteric and Allosteric Sites of Estrogen Receptor α.

Estrogen receptor α (ERα) remains a pivotal therapeutic target for ER-positive (ER+) breast cancer, yet resistance to endocrine therapies driven by ligand-binding pocket (LBP) mutations demands novel strategies. To integrate the complementary strengths of binding modalities of the orthosteric LBP and the allosteric coactivator binding site (CBS) against endocrine resistance, we designed a series of ERα LBP/CBS dual-targeting proteolysis-targeting chimeras (PROTACs). The lead compound P5 demonstrated high binding affinities for LBP and CBS sites, potent antiproliferative activity, and efficient

Selection of Breast Biopsy Markers: Effect on Breast Imaging Procedures, Follow-up, and Costs.

Breast biopsy markers have evolved from simple metallic markers to sophisticated multimaterial devices that serve critical purposes throughout the breast cancer care continuum. This evolution began at The University of Texas MD Anderson Cancer Center in the 1960s for gynecologic and head and neck tumors. After the MicroMark (Biopsys) clip was approved by the U.S. Food and Drug Administration in 1995, markers were subsequently adapted for patients with breast cancer in 1999. By 2003, markers had become the standard of care. Modern markers serve multiple clinical functions: marking biopsy sites,

Scalp cooling versus standard cold-cap in preventing chemotherapy-induced alopecia in patients with localized breast cancer: ICELAND protocol, a randomized controlled trial and economic evaluation.

BACKGROUND: Chemotherapy-induced alopecia (CIA) remains one of the most distressing adverse effects for patients with localized breast cancer receiving anthracycline and taxane-based chemotherapy regimens. Although automated scalp cooling systems and standard non-automated cold-cap devices have demonstrated efficacy in reducing CIA, controlled trials with direct randomization comparing these two modalities of scalp cooling are lacking. This study aims to compare clinical efficacy, patient-reported outcomes, and medico-economic impact of a scalp cooling system (PAXMAN® PSCS 2) versus standard c

Integrating dynamic modeling of signaling pathways with subject-specific transcriptomic data to assess breast cancer risk.

Breast cancer is the most common cancer in women and a leading cause of death. Traditional risk assessment models, such as the Gail model, lack molecular insight, limiting their usefulness for personalized prevention strategies. We developed a computational framework that integrates individual transcriptomic data with dynamic modeling of cell signaling to create personalized models for 30 subjects (including 15 who later developed breast cancer). Using features extracted from the dynamic simulation, we stratified individuals into four risk clusters with significantly different disease-free per

Shedding light on neural learning to rank models for anticancer drug prioritization.

Learning to Rank (LeToR) methods have gained increasing attention in drug response prediction, offering a direct way to prioritize effective treatments for cancer cell lines. In this study, we systematically benchmark six ranking loss functions, including state-of-the-art listwise methods, and five types of molecular representations across two large-scale drug screening datasets, CTRP and PRISM. Using high-dimensional gene expression profiles and various drug fingerprints and descriptors, we evaluated models under multiple validation setups and ranking metrics. Our results demonstrate that lis

Breast Neuroendocrine Carcinoma: Molecular Insights Beyond Histology.

PURPOSE: Primary breast neuroendocrine carcinomas (NECs) are rare, high-grade malignancies that are frequently grouped with invasive breast carcinomas with neuroendocrine differentiation (IBC-NED), despite uncertain biological equivalence. We sought to define the clinicopathologic, immunophenotypic, and genomic features of breast NEC and to determine whether they represent a biologically distinct entity. METHODS: We performed a retrospective analysis of 24 primary breast NECs and compared them with 28 grade-matched IBC-NEDs. Clinicopathologic features, treatment response, and outcomes were ev

A lipid droplet-targeted AIE photosensitizer for efficient multi-type ROS generation and breast cancer photodynamic therapy.

Photodynamic therapy (PDT) is a promising non-invasive strategy for breast cancer, yet its clinical translation is hindered by fluorescence quenching, insufficient reactive oxygen species (ROS) generation and poor tumor targeting. Herein, a novel lipid droplet-targeted aggregation-induced emission (AIE) photosensitizer CTC was designed and synthesized. CTC has a large Stokes shift of 150 nm to avoid self-absorption and exhibits typical AIE properties in aqueous environments. Under white light irradiation, it efficiently generates three cytotoxic ROS (1O2, ·OH, O2•-), with a high colocalization

Revisiting OPTIMIST trial imaging criteria for pathologic complete response to neoadjuvant therapy.

INTRODUCTION: Omission of breast surgery after neoadjuvant systemic therapy (NST) may reduce treatment burden in exceptional responders, but safe implementation requires reliable identification of pathologic complete response (pCR). This study assessed the performance of OPTIMIST imaging criteria for predicting pCR and explored whether refined thresholds could improve applicability without compromising performance. MATERIALS AND METHODS: We retrospectively reviewed patients who received NST followed by surgery at a single tertiary center between 2016 and 2020. Among 1659 patients, 499 met OPT

Finding the Sweet Spot? Differential Diabetes Risks in Autologous Breast Reconstruction.

BACKGROUND: Autologous breast reconstruction (ABR) offers superior quality of life compared with implant-based reconstruction or mastectomy alone but carries substantial perioperative risk. Diabetes mellitus is a known predictor of postoperative morbidity but is often treated as a binary variable, obscuring differences between patients on oral antihyperglycemic agents (diabetes managed with oral antihyperglycemic agents [DM-ORAL]) and insulin (diabetes requiring insulin therapy [DM-INS]). MATERIALS AND METHODS: Using the National Surgical Quality Improvement Program, we identified female brea
