A clinical and galactographic investigation was carried out on 103 patients with hematic, serous-hematic, and serous nipple discharge. The age of the patients ranged from 18 to 72 years. A single papilloma was found in 20 cases, diffuse papillomatosis in 2 cases, atypical ductal hyperplasia in 8 cases, and ductal carcinoma in 4 cases (3 of these were infiltrating and 1 was noninfiltrating associated with a diffuse papillomatosis). An analysis of the own material and of literature data permits to note alterations in the clinical course of mammary gland tuberculosis, which is the manifestation of tuberculosis pathomorphism under modern conditions. The lesion of the mammary gland is most commonly an isolated disease. This interferes with the differential diagnosis between tuberculous mastitis and tumors of the mammary gland and supports the opinion that the spreading of infection from other old tuberculous foci via hematogenic path plays the main part in the pathogenesis of tuberculosis of the mammary gland.

In 9 cases of human adjuvant disease, 4 cases of scleroderma (3 progressive systemic sclerosis and 1 localized morphea) were observed. A case of mediastinal sclerosis after radiotherapy is presented as a curiosity in iatrogenic etiologies of chylothorax. After 7 weeks of conservative treatment, an operative pleurodesis was successful. In a second clinical case, the hazardous intra-thoracic coagulation of the chyle is illustrate. Etiology and therapeutic mangements of iatrogenic chylothorax in the literature are briefly reviewed.

Elastosis, an abundance of elastic tissue, is commonly present in breast carcinoma. However, its diagnostic significance remaines an unsettled issue. This study documents 17 cases of elastosis occurring in a distinctive benign sclerosing ductal lesion of the female breast (Fenoglio and Lattes: Cancer 33: 691-700, 1974). Elastosis was characterized by staining reactions and, in several instances, by elastase digestion and electron microscopy. Ischemic enteritis or infarction occurred in four of 15 patients who had carcinoid tumors of the small intestine. All four, in addition, had elastic vascular sclerosis of the mesenteric vessels. Both arteries and veins were involved, and the change occurred in the region of tumor deposit. The frequency of malignant diseases among 1866 living and 340 deceased multiple sclerosis (MS) patients was investigated in Finland. There is a definite place for estrogen replacement in symptomatic menopausal women. Requisites of such therapy are a through history to establish a real need, a complete gynecologic examination with a Pap smear before therapy begins and annually afterward, use of the smallest daily dose of estrogen that gives the desired effect, and periodic attempts to reduce the dosage or stop the hormone. Endometrial biopsy should be done regularly if this is feasible, and any uterine bleeding should be investigated. Infiltration of the walls of blood vessels by mammary epithelium was found in two cases of sclerosing adenosis. Survey of a series of cases of sclerosing adenosis and severe epitheliosis revealed a 10 per cent incidence of vascular infiltration. Usually only a single vein was involved but in the propositus an artery and two veins were infiltrated. The findings highlight the infiltrative propensities of sclerosing adenosis, already recognised as infiltrating nerve sheaths. The vascular lesions appear to be benign structurally and this is borne out by the clinical behaviour.
