Bloody nipple discharge in children is rare and often alarms families because of its malignant associations in adults. We describe two prepubertal boys (3 and 5 years) with unilateral, spontaneous, painless bloody nipple discharge without trauma, infection, systemic symptoms, or exogenous hormone exposure. Examinations showed only mild unilateral breast change, with no masses, skin or nipple abnormalities, or lymphadenopathy. Breast ultrasonography was the primary investigation: one child had retroareolar ductal dilatation with cystic change consistent with mammary duct ectasia; the other had

Mammary duct ectasia is a benign breast condition that can mimic malignancy on MRI. We describe a woman with a pathogenic RAD51C variant who entered high-risk MRI surveillance at age 53 and was found to have segmental non-mass enhancement in the central left breast. MRI-guided core biopsy demonstrated duct ectasia with inspissated eosinophilic secretions, dense periductal lymphoplasmacytic inflammation with concentric fibrosis, and coarse microcalcifications without epithelial atypia or carcinoma. Her history included hyperprolactinemia from a growth hormone- and prolactin-positive pituitary a

BACKGROUND: Plasma cell mastitis (PCM), also termed mammary duct ectasia, is a chronic nonbacterial inflammatory disease characterized by mammary duct dilation and plasma cell infiltration. Due to its varied and nonspecific clinical presentation, PCM is frequently misdiagnosed as breast cancer, complicating clinical management.
OBJECTIVES: This review aims to summarize recent advances in the understanding of PCM, focusing on its etiology, clinical manifestations, diagnosis, and treatment strategies, as well as clarifying differential diagnostic points with granulomatous mastitis (GLM).

INTRODUCTION: 17α-hydroxylase/17,20-lyase deficiency (17-OHD) typically presents with sexual infantilism, hypertension, and hypokalemia. However, phenotypic variability, particularly breast development, may obscure diagnosis. This study aims to characterize an atypical presentation of 17-OHD with preserved breast development and breast nodules, and to evaluate clinical and hormonal features associated with breast development through a systematic literature review.
METHODS: A 38-year-old woman with bilateral breast nodules and ductal ectasia was diagnosed with 17-OHD, confirmed by CYP17A1 vari

Breast manifestations of autoimmune diseases are rare but may present as localized disease or as part of a systemic disease. The most common clinical presentations of autoimmune mastitis are palpable masses and mastalgia, but patients can also be asymptomatic. Its radiological features are nonspecific and varied; it usually appears as an irregular hypoechoic mass or ill-defined hypoechoic nonmass lesion that mimics malignancy, with or without duct ectasia on breast ultrasonography. On breast MRI, segmental or regional nonmass enhancement is observed. However, due to its nonspecific and rapid c

BACKGROUND AND OBJECTIVE: Non-puerperal mastitis (NPM) is a breast disease with poor clinical manifestations, which seriously affects women's health and quality of life. Due to the low incidence rate of the disease and the paucity of related research, there is much misdiagnosis and mis-management of periductal mastitis (PDM) and granulomatous lobular mastitis (GLM). Therefore, understanding the differences between PDM and GLM, in terms of etiology and clinical manifestations, is crucial for patient treatment and prognosis. At the same time, choosing different treatment methods may not achieve

BACKGROUND: Non-puerperal mastitis (NPM), a recurrent chronic inflammation of non-lactating breast, often proves tremendous difficulty in treatment, and it may give rise to its complicated symptoms and unclear etiology. Furthermore, the clinical morbidity rate of NPM has been increasing in recent years.
METHODS: Overall, 284 patients diagnosed with NPM were consecutively recruited as cases in this study, and patients with benign breast disease (n = 1128) were enrolled as control. The clinical, biomedical, and pathological indicators were analyzed. Univariate and multivariate logistic analysis

Mammary duct ectasia is a benign breast condition of unknown etiology. Patients with ductal ectasia might have no symptoms or might present with symptoms so severe that surgery is indicated. Symptomatic patients require a full and careful workup because ductal ectasia in advanced stages can be mistaken for carcinoma of the breast. These patients are likely to undergo mammography, sonography, magnetic resonance imaging, and galactography. Therefore, radiologic technologists are involved directly with their imaging and care.

The aim of the present study was to compare the efficacy of vacuum sealing drainage (VSD) and precise ultrasound-guided debridement in the treatment of non-lactational mastitis and to determine the optimal surgical treatment. A set of 60 cases diagnosed with non-lactational mastitis who had received surgical treatment at the Department of Thoracic and Breast Surgery of Xiamen Hospital of Traditional Chinese Medicine (Xiamen, China) between July 2017 and June 2019 were included. According to the surgical method, 30 patients were assigned to the VSD group and 30 patients were assigned to the pre

BACKGROUND: Plasma cell mastitis (PCM), also known as mammary duct ectasia, is a chronic nonbacterial breast inflammation characterized by duct expansion and plasma cell infiltration. The severe and intense clinical manifestations profoundly affect the quality of life of female patients. Although the pathological process of PCM is known to include four stages (duct dilatation, inflammation, abscess and fistula), there is still lack of imaging techniques and serum markers with high specificity in clinical practice. Due to recurrent acute attacks and the prolonged healing process of the disease,

AIM: To explore the effect of dexamethasone combined with metronidazole in the treatment of mammary duct ectasia (MDE) and its relationship with changes in serum interleukin-10 (IL-10) and IL-17 expression.
METHODS: One hundred and twenty patients with MDE were divided into two groups randomly, control and observation groups (each n = 60). Another 50 patients with normal physical examination were recruited in the normal group. The expressions of serum IL-10 and IL-17 in three groups before and after treatment were observed. The prediction value of IL-10 and IL-17 in clinical efficacy was eval

Although pubertal gynecomastia is a common clinical presentation of adolescent males, prepubertal gynecomastia is uncommon and mostly idiopathic. However, pathological causes of prepubescent gynecomastia are encountered in clinical practice. This manuscript carries an important message to general pediatricians, to care about exclusion of pathological causes for every patient of prepubertal gynecomastia. We present four different patients with pathological gynecomastia. One of them revealed to be secondary to Sertoli cell tumor, while the second patient describes trauma as a rare cause of prepu

Mastitis frequently affects women of childbearing age. Of all the pathological breast conditions requiring specific management, autoimmune mastitis is in the third position after infection and breast cancer. The aim of this literature review was to make a comprehensive description of autoimmune diseases targeting the mammary gland. Four main histological patterns of autoimmune mastitis are described: (i) lymphocytic infiltrates; (ii) ductal ectasia; (iii) granulomatous mastitis; and (iv) vasculitis. Our literature search found that all types of autoimmune disease may target the mammary gland:

Bloody nipple discharge in infancy and childhood is extremely rare, and mammary duct ectasia is the most common etiology. Ultrasound (US) findings of mammary duct ectasia include dilated ducts and tubular anechoic lesions that may contain echogenic debris in the subareolar region. However, mammary duct ectasia may show variable US findings, which are not well described in the literature. We report 3 cases of mammary duct ectasia in infancy and childhood with variable imaging findings, including complex cystic and solid lesions. Detailed initial clinical and US findings and serial follow-up US

Granulomatous lobular mastitis (GLM) and mammary duct ectasia (MDE) are inflammatory diseases. However, only a limited number of studies have focused on characterizing their clinicopathological features. The aim of the present study was to investigate the etiology, clinicopathological characteristics and diagnosis of GLM and MDE. The clinical information and treatment of 118 female patients with pathologically-proven GLM or MDE were retrospectively analyzed in the present study. A total of 29 cases had GLM, 77 had MDE and 12 had GLM accompanied by MDE. GLM tends to occur in patients who have h

Idiopathic granulomatous mastitis (IGM) is a rare benign inflammatory breast entity characterized by lobulocentric granulomas. IGM has a persistent or recurrent disease course and affects parous premenopausal women with a history of lactation. It has also been associated with hyperprolactinemia. The most common clinical sign is a palpable tender mass. However, the nonspecific manifestations and varied demographic features of this condition, as well as the other similar-appearing and superimposed breast entities, pose substantial diagnostic challenges. Entities with similar manifestations inclu

Bloody discharge from the nipple (thelorrhagia) is a rare symptom in childhood and is a cause of great concern due to the association with carcinoma in adults. The most common cause in children is mammary duct ectasia, which is a benign and self-limiting condition. It is characterized by dilatation of the mammary ducts, fibrosis and periductal inflammation. The etiology has not been identified and is multifactorial. Up to date, only isolated case reports have been published. Therefore, we present a review of the literature and we report a case of a fivemonth- old male infant that resolves spon
