Differentiating Avian and Seasonal Influenza Viruses in Wastewater by Digital PCR.

Highly pathogenic avian influenza A(H5Nx) viruses, particularly clade 2.3.4.4b, pose substantial global health risks. We combined and piloted 2 duplex reverse transcription digital PCRs to discriminate between avian and seasonal influenza viruses in wastewater. This approach enabled rapid, strain-specific detection to reduce ambiguity in environmental surveillance for influenza viruses.

Nurse-led implementation of the Closer to Home project to increase access to specialist dementia care.

People living with dementia may face barriers to accessing local specialist dementia care in the UK. The Closer to Home (C2H) project, a national initiative led by nurses at Dementia UK in partnership with Leeds Building Society, sought to address this inequality by bringing specialist dementia services nearer to service users. Although C2H had been conceived as a mobile clinic, COVID-19 restrictions required a rapid transition to virtual delivery. Over its lifespan, C2H provided 3521 appointments, nearly 50% more than its original target, and received positive feedback from service users. Its

Transmission Dynamics of Dengue Virus, Ceará State, Brazil, 2010-2025.

We evaluated the transmission dynamics of dengue virus (DENV) during 2010-2025 in Ceará State, Brazil. Ceará has been a dengue hyperendemic region, where all 4 DENV serotypes co-circulate, predominantly DENV-1 and DENV-2, and where DENV-3 was recently reintroduced. Our findings provide an epidemiologic baseline for postlicensure dengue vaccine evaluation.

Pharmacokinetic diversity of baloxavir marboxil across rare avian species.

High pathogenicity avian influenza virus (HPAIV) poses a serious threat to captive rare birds. Because effective countermeasures are limited, the use of human antiviral drugs to treat HPAIV infections in captive rare birds has been considered. Although the human antiviral drug, baloxavir marboxil (BXM), is not approved for the treatment of influenza virus infection in birds, previous studies demonstrated the antiviral efficacy of BXM in experimental avian models. Pharmacokinetic studies have also revealed substantial interspecies differences following BXM administration, suggesting the need to

Immunoinformatics and reverse vaccinology design of self-amplifying RNA vaccine against marburg virus with structural dynamics and immune simulation analysis.

Marburg virus disease (MVD), a deadly zoonotic illness in humans, is caused by the Marburg virus (MARV), a single-stranded RNA virus belonging to the Orthomarburgvirus genus of the Filoviridae family. There are currently no approved vaccines or efficient antiviral therapies for MARV, despite high morbidity and fatality rates of the virus, particularly in Africa. Recognizing its epidemic potential and its inclusion on the WHO list of blueprint priority pathogens, an immunoinformatics approach was used to design novel multi-epitope saRNA vaccine candidates (MB-saVaxI and MB-saVaxII) targeting th

Data-driven forecasting of Flu, RSV, and COVID-19 related outcomes in the United States and Canada via Hankel dynamic mode decomposition.

The (large) season-to-season variability and limited dynamical history make the forecasting of infectious diseases a challenging problem. Here, we examine the extent to which advances in data-driven dynamical modeling can provide accurate predictions by benchmarking the performance of one such method, Hankel dynamic mode decomposition (DMD), on the 2024-2025 influenza, respiratory syncytial virus (RSV), and COVID-19 seasons in the United States and Canada. Using Hankel-DMD, we generated weekly forecasts that were submitted to the Center for Disease Control and Prevention's (CDC) FluSight Forec

Impact of the COVID-19 pandemic and control measures on type 2 diabetes care among South Asians: the perspective of healthcare providers in British Columbia, Canada.

BACKGROUND: British Columbia is one of the most ethnically diverse provinces in Canada, with South Asians emerging as one of the fastest-growing ethnic minority groups, burdened by a high incidence of type 2 diabetes. During the COVID-19 pandemic, many healthcare services shifted to virtual care to minimize infection transmission. Given the limited understanding of how primary care physicians in BC adapted their diabetes care approaches and the consequent impact on patient experience, this qualitative study aims to explore the effects of pandemic-related control measures on primary care diabet

The hidden burden of viral meningitis in ethiopia: Etiology and clinical outcomes.

BACKGROUND: Viral meningitis remains a significant global public health problem, particularly in resource-limited settings where the diagnosis of viral etiology is mainly based on clinical assessment. This study aimed to identify the common viral etiologies of meningitis, describe their associated clinical features, and assess patient outcomes among individuals suspected of meningitis without evidence of bacterial meningitis. METHODS: We conducted a multicenter study involving 430 patients clinically suspected of meningitis. Clinical data and cerebrospinal fluid samples (CSF) were collected f

The detection paradox: How COVID-19 surveillance patterns shifted from lower-income to more affluent neighborhoods in Miami-Dade County.

Passive disease surveillance systems may systematically undercount burden in low-resource minority neighborhoods, yet fine-scale evidence of this phenomenon remains limited. We examined spatiotemporal patterns of reported COVID-19 cases across Miami-Dade County's diverse neighborhoods and investigated which socioeconomic and demographic characteristics predicted areas of elevated and suppressed case rates. In an ecological, cross-sectional analysis of 491,769 geocoded cases across 1,831 census block groups from March 2020 to May 2021, we applied negative binomial regression with spatial eigenv

Precancerous cervical lesions and associated factors among HIV-positive women in Southern and Eastern African countries: A systematic review and meta-analysis.

BACKGROUND: Women living with HIV (WLHIV) face a sixfold increased risk of cervical cancer compared to HIV-negative women. Most WLHIV residing in the Southern and Eastern African regions have a substantial HIV-related cervical cancer burden. Studies on precancerous cervical lesions (PCCL) and associated factors in WLHIV from these regions have inconsistent and inconclusive findings. Therefore, we aimed to synthesize the pooled prevalence of PCCL and associated factors among WLHIV in Southern and Eastern Africa. METHODS: This study was registered under PROSPERO (CRD420251123583) and conducted

Public Awareness, Attitudes, and Practices Toward Digital Health Surveillance in Malawi: Cross-Sectional Study.

BACKGROUND: Digital surveillance systems are increasingly recognized as essential tools for enhancing early outbreak detection, reporting, and response, especially in low-resource settings. The COVID-19 pandemic unveiled the critical role of digital health surveillance systems in timely detection, monitoring, and responses to public health threats. However, successful implementation depends on public awareness, acceptance, and willingness to participate. Limited evidence exists on how the public perceives and interacts with digital surveillance in Malawi. OBJECTIVE: The aim of the study was t

Barriers and Enablers to Asynchronous Secure Messaging Use in Hospital-Based Ambulatory Specialty Clinics in Ontario, Canada: Convergent Mixed Methods Study.

BACKGROUND: The COVID-19 pandemic accelerated the adoption of virtual care, including patient-to-provider asynchronous secure messaging (ASM). However, while ASM offers access and convenience, the surge in message volume can increase physician workload and contribute to burnout. Furthermore, it has raised additional concerns about message appropriateness, medicolegal liability, and sustainability of this communication virtual care modality. OBJECTIVE: This study aims to explore the experience with ASM in Ontario in specialty ambulatory care to inform current practice support strategies. We so

Interferon-λ signaling to maternal dendritic cells protects against congenital Zika virus infection.

Interferon-λ (IFN-λ, type III IFN) mediates antiviral immunity at anatomic barriers, including the maternal-fetal interface. To investigate the effects of IFN-λ during congenital Zika virus (ZIKV) infection, we infected mice lacking the IFN-αβ receptor (Ifnar1-/-) or both the IFN-αβ and IFN-λ receptors (Ifnar1-/-Ifnlr1-/-) at embryonic day 9 and found that loss of maternal IFN-λ signaling resulted in greater transplacental transmission, highlighting a protective effect of IFN-λ at the maternal-fetal interface. To investigate the spatial relationship between ZIKV infection and antiviral respons

Opposing cell-type preferences for binding and replication shape influenza A virus infection in human airways.

Influenza A viruses (IAVs) pose a threat to human health. Although receptor specificity, polymerase adaptation, and airway tropism have been studied extensively, it remains unresolved how they combine to shape tropism in airway epithelia. Here, we show that avian IAVs infect ciliated and secretory cells but preferentially bind to ciliated cells because of higher apical α2,3-linked sialic acid levels. As replication levels are comparable between cell types, this translates into overall preference for ciliated cells. Human IAVs also preferentially bind to ciliated cells despite comparable α2,6-l

Zinc supplementation counteracts the age-related impairment of interferon-α response by reconstituting basal interferon Regulatory factor 3 levels.

BACKGROUND & AIMS: Immunosenescence increases the risk of infectious diseases in the aged population. A health issue partially caused by a reduced interferon (IFN)-α response. With zinc playing a crucial role in antiviral immunity and zinc-deficiency being highly prevalent in the elderly, this study examined the association between the age-related decline in IFN-α production and zinc status. The aim of this study was to compare the IFN-α response of young controls to geriatric patients stratified by their zinc status, to examine the zinc dependency of the transcription factor Interferon Regula

In-Hospital Outcomes of Acyclovir With or Without Corticosteroid for Herpes Simplex Encephalitis: A Nationwide Retrospective Cohort Study.

BACKGROUND AND OBJECTIVES: Acyclovir is the standard therapy for herpes simplex virus (HSV) encephalitis, yet mortality remains high. Secondary inflammatory responses triggered by viral infection potentially contribute to additional brain injury. Given their anti-inflammatory properties, corticosteroids are considered a biologically plausible adjunctive treatment for HSV encephalitis. However, evidence regarding their effects remains inconclusive, highlighting the need for large-scale evidence. METHODS: We conducted a retrospective cohort study using the Japanese national inpatient Diagnosis
