Associations of baseline characteristics, patient-reported outcomes, and satisfaction with pain therapy with the patient's global impression of change: a prospective cohort study.

BACKGROUND: Patient-reported outcome measures (PROMs) are key elements of assessing the efficacy of perioperative pain management. Here, we aimed to capture the association of 10 individually reported aspects of patient's specific impression of change since surgery (PSIC) related to four outcome domains of a previously defined core outcome set, relative to the patient's global impression of change (PGIC). We further evaluated the influence of type of surgery, sex, preoperative baseline characteristics, and satisfaction with pain management on PGIC. METHODS: This exploratory analysis used the

Aromatase Inhibitor-Induced Arthralgia Among Patients With Early Breast Cancer: Prevalence, Treatment Nonadherence, and Management in a Prospective Longitudinal Cohort.

BACKGROUND: Aromatase inhibitor (AI)-induced arthralgia (AIA) can lead to treatment discontinuation and worse disease outcomes. This study assessed the prevalence of AIA, nonadherence to AI therapy, and supportive care strategies (SCS) among patients with early-stage breast cancer. METHODS: Patients with early-stage breast cancer treated with adjuvant AI therapy were included from the CANTO cohort. AIA was defined as any-grade articular or muscular pain (CTCAE version 4.0) at years 1, 2, 4, and 6 after diagnosis (corresponding to 3-6 months, 1, 3, and 5 years of AI exposure, respectively). We

Microbe-associated molecular patterns differentially mediate carcinogenic alterations of the breast tissue in the context of obesity.

Obesity is a risk factor for breast cancer. Obesity alters the microbiome and microbiome perturbations are reported in breast cancer patients. Yet, the impact of obesity-mediated microbial shifts on breast cancer risk remains unclear. Here, we investigate the effect of microbial-associated molecular pattern (MAMP) signaling on genome instability and inflammation. We show in human samples that obesity chronically elevates breast tissue levels of two MAMPs: lipopolysaccharide (LPS) and flagellin. In contrast, obesity was not associated with chronic elevations in lipoteichoic acid (LTA). Injectio

Diffuse Dermal Angiomatosis of the Breast Presenting as Diffuse Erythema Mimicking Inflammatory Breast Carcinoma in a Healthy Pregnant Patient.

Diffuse dermal angiomatosis represents a reactive form of microvascular proliferation characteristically linked with vascular insufficiency of diverse etiology. It occurs most commonly on the lower extremity where a frequent association is venous insufficiency; however, it can exhibit breast localization falling under the designation of diffuse dermal angiomatosis of the breast (DDAB). The clinical presentation of DDAB can be striking and resembles a catastrophic thrombogenic vasculopathy such as purpura fulminans, calciphylaxis, and antiphospholipid antibody syndrome. However, a less common p

[Rectal bleeding in infancy; could it be an allergy?].

Rectal bleeding in infants is frequently regarded as an alarming symptom for healthcare providers and often causes significant concern and anxiety among parents. In this article, we discuss two infants who were urgently referred to pediatric services for further evaluation. Both infants showed no signs of illness. The diagnosis in one case was ingested maternal blood and in the other Food Protein-Induced Allergic Proctocolitis (FPIAP), triggered by cow's milk. FPIAP is a non-IgE-mediated food allergy. FPIAP can also occur in exclusively breastfed infants when the lactating mother consumes cow'

Do Combined Resistance and Aerobic Exercise Programs Cause an Interference Effect in Women with Breast Cancer? A Systematic Review and Network Meta-analysis.

BACKGROUND: Exercise medicine has gained significant recognition owing to its demonstrated benefits throughout the breast cancer treatment continuum. While resistance exercise (RE) promotes improvements in lean mass and muscle strength, aerobic exercise (AE) enhances cardiorespiratory fitness, with several studies investigating both exercise modes in patients with breast cancer. However, because of an effect often referred to as interference effect, it is hypothesised that combining resistance and aerobic exercise (COMB) may compromise gains in lean mass and muscle strength and other outcomes

Glucagon-like peptide-1 receptor agonists and the risk of obesity-related cancers: a systematic review and meta-analysis.

BACKGROUND: Type 2 diabetes mellitus (T2DM) and obesity are increasing and are established risk factors for malignancy. Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are widely used for T2DM and obesity, but their association with cancer risk remains uncertain. We assessed the association between GLP-1RA use and obesity-related cancers. METHODS: We searched Embase, PubMed, and the Cochrane Central Register of Controlled Trials (CENTRAL) from inception to 20 November 2025. Observational studies and randomized controlled trials of adults (≥18 years) with T2DM and/or obesity reporting can

Opioid use after breast cancer in Denmark: a nationwide study of social and clinical factors.

BACKGROUND: Pain is a common, long-lasting effect of breast cancer treatment, but adequate pain management can be a challenge in survivorship care. Opioids remain important for acute and chronic pain, yet prolonged use carries risks of dependence and overdose. Denmark has historically had high opioid prescription rates, but the duration, strength, and intensity of use among breast cancer survivors is not well described. METHODS: We conducted a nationwide registry-based study of 84 610 Danish women diagnosed with stage I to III breast cancer between 1997 and 2020. We described temporal trends

Medical thoracoscopy with talc poudrage and indwelling pleural catheter insertion versus medical thoracoscopy with talc poudrage alone for patients with symptomatic malignant pleural effusion (TACTIC): a randomised, controlled phase 3 trial.

BACKGROUND: Patients with suspected malignant pleural effusions (MPE) are often in need of both a confirmatory diagnosis and symptom control. Therapeutic options include talc pleurodesis via chest drain, poudrage during medical thoracoscopy, or alternatively an indwelling pleural catheter (IPC). Combining the diagnostic and therapeutic efficacy of medical thoracoscopy and poudrage with the ambulatory benefits of an IPC has not been studied within a randomised controlled trial. The aim of the TACTIC trial was to determine whether this approach resulted in a reduced length of hospital stay and i

Infant hyperglycemic excursions by saliva 1,5-anhydroglucitol varied by weight status, rapid weight gain, and mode of feeding: an exploratory secondary analysis.

BACKGROUND: Early infant feeding and weight gain can impact the development of chronic diseases. 1,5 anhydroglucitol (1,5-AG) is a measure of hyperglycemic excursions (HEs) and decreases during HEs due to increased urinary excretion. OBJECTIVES: We hypothesized that HEs as measured by 1,5-AG are associated with infant rapid weight gain (RWG). METHODS: Infants from the Maternal and Infant Nutrition Trial, a randomized controlled feeding trial, participated in this exploratory secondary analysis. Saliva from 127 infants aged 5 mo was collected before the introduction of complementary foods. Sa

Need for and design of a trial to test efficacy of weight loss interventions for cancer prevention: an international consensus using expert nominal group and Delphi methods.

Obesity is associated with increased risk of at least 13 cancer types. Evidence from bariatric surgery cohorts and some behavioural intervention trials supports the notion that weight loss can prevent obesity-related cancers. The introduction of glucagon-like peptide (GLP)-1 agonist drugs has rapidly revolutionised pharmacotherapy options. A cancer prevention clinical trial would be complex, lengthy, and costly; therefore, we undertook an international expert consensus to assess the need for and design of a weight-loss intervention cancer prevention trial. We used a combination of two nominal

KNLR: A heterogeneous ensemble learner for predicting Foie gras weight grade in mule ducks (Anas platyrhynchos × Cairina moschata).

Ante-mortem prediction of foie gras weight grade remains an unsolved challenge in commercial duck production. We developed KNLR, a novel heterogeneous ensemble learner that accurately predicts foie gras weight classification in mule ducks using pre-overfeeding morphometric measurements and post-overfeeding live weight, enabling producers to optimize feeding strategies and improve grading consistency. KNLR integrates Heterogeneous Ensemble Feature Selection (HEFS) with Weighted Area Under Curve Evaluation (WAUCE) to enhance predictive robustness. Comparative evaluation with four base learners (

Case report of severe hypocalcemia with atypical symptoms after zoledronic acid in palliative care: a deprescribing pitfall.

BACKGROUND: Symptomatic hypocalcemia is an uncommon metabolic disturbance in palliative care and may therefore be overlooked. While deprescribing is a key principle in this setting, routine discontinuation of calcium and vitamin D supplementation may be harmful in selected high-risk patients, particularly after recent antiresorptive therapy. CASE PRESENTATION: A 74-year-old woman with hormone receptor-positive metastatic breast cancer, extensive bone metastases and chronic kidney disease, was admitted to a German palliative care unit. Four weeks prior, she had received zoledronic acid, and cal
