Testicular Ischaemia Following Diverticulitis Complicated by Colovesical Fistula: A Case Report.

INTRODUCTION: Colovesical fistula is a recognized complication of diverticulitis, but progression to testicular ischaemia is, to our knowledge, an unreported phenomenon. While colovesical fistulas occur in diverticular disease, the spread of infection to the testes is rare. CASE PRESENTATION: A 47-year-old male with recurrent diverticulitis presented with abdominal pain and was found to have a pelvic abscess. Despite conservative management, the patient developed pneumaturia, dysuria, and right testicular pain. Preoperative scrotal ultrasonography with color Doppler demonstrated globally redu

A study on the correlation between albumin-to-alkaline phosphatase ratio and the severity of pelvic inflammatory disease.

OBJECTIVE: To evaluate albumin-to-alkaline phosphatase ratio (AAPR) as a pelvic inflammatory disease (PID) severity biomarker and determine PID predictors. METHODS: This retrospective study included ninety-nine patients with PID, as the experimental group, further classified by severity into mild and severe PID groups. The control group included forty-nine patients who underwent elective surgery for non-inflammatory tubal-ovarian masses in the same period. Relationships between PID severity and general information, vital signs, laboratory indices, and imaging characteristics among the three g

Dynamic Covalent Dual Prodrug Assemblies for Endometritis Therapy via Inflammation Alleviation, Tissue Repair, and Reproductive Function Restoration.

Endometritis is an inflammatory disorder of the endometrium triggered by microbial infection or immune dysregulation, and current therapies, which are largely single-targeted, fail to coordinate multiple mechanisms. Here, we report a dynamic covalent dual prodrug nanoplatform (termed SFB) that integrates the anti-inflammatory, antiapoptotic, and proautophagic effects of spermidine (SPD) and baicalein (BAI). Using phenylboronic acid-mediated linkage and reaction-induced self-assembly (RISA), SFB formed well-defined nanostructures with an exceptionally high dual-drug encapsulation (exceeding 90%

Influence of Oxidative Stress-Mediated Inflammation on the Pathogenesis of Reproductive Disorders: Exploring the Benefits of Carnosine for Prevention and Treatment of Endometriosis.

Endometriosis is a chronic pathological condition characterized by the growth of endometrial-like tissue outside the uterine cavity and is frequently associated with severe pain, persistent inflammation, and fibrosis within the pelvic region and other parts of the body. The exact causes of endometriosis are not clear, but an innate or adaptive immune response defect has recently been suggested as a factor in the disease's development. Carnosine is a natural dipeptide formed by the ligation of β-alanine and L-histidine and characterized by a multimodal mechanism of action that includes antioxid

Rectal diffuse large B-cell lymphoma misdiagnosed as bleeding cancer in an elderly patient.

BACKGROUND: Primary rectal diffuse large B-cell lymphoma (DLBCL) is a rare malignancy that often closely mimics more common rectal pathologies such as adenocarcinoma or inflammatory bowel disease. This resemblance poses considerable diagnostic difficulty, and misdiagnosis may result in delayed treatment and unnecessary procedures, ultimately affecting patient outcomes. CASE PRESENTATION: An 87-year-old female presented with intermittent hematochezia that acutely worsened two hours before admission. Physical examination identified a rectal mass located 5 cm from the anal verge. Pelvic imaging a

LncRNA MALAT1 affects the progression of endometritis induced by lipopolysaccharide via regulating the expression of miR-142-3p.

BACKGROUND: Chronic endometritis (CE) is a common gynecological disorder linked to infertility, but its pathogenesis remains unclear. AIM: To investigate the role of lncRNA MALAT1 in lipopolysaccharide (LPS)-induced endometritis and its regulatory mechanism involving the miR-142-3p/PIK3R1 axis. METHODS: A total of 133 patients with chronic endometritis (CE) and 100 healthy controls were enrolled. Serum and endometrial tissue samples were collected to detect the expression levels of MALAT1, miR-142-3p, and PIK3R1 using RT-qPCR. LPS-treated human endometrial stromal cells (HESCs) were used to

Amniotic Mesenchymal Stromal/Stem Cell-Derived Extracellular Vesicles for Equine Chronic Degenerative Endometritis Treatment.

BACKGROUND: Equine chronic degenerative endometritis (CDE) is a progressive process characterized by endometrial fibrosis that could be responsible for alterations of uterine environment and foetal-maternal communication. OBJECTIVES: The aim of this study was to try to restore this communication by intrauterine administrations of amniotic cell-derived extracellular vesicles (AMC-EVs) in a case series. STUDY DESIGN: Twelve mares were selected on the basis of their reproductive history of early embryonic loss or abortion and clinical suspicion of CDE subsequently verified with histopathologica

Maternal risk factors associated with complex gastroschisis: Cannabis exposure and recurrent urinary tract infections may be modifiable targets.

AIM OF THE STUDY: Maternal risk factors associated with gastroschisis occurrence have been identified, but predisposing factors to the complex variant are unknown. This study aimed to identify factors associated with complex gastroschisis by comparing the maternal risk factor profiles of simple and complex gastroschisis pregnancies. METHODS: All cases of gastroschisis managed in one tertiary care hospital between 1991 and 2022 were identified from a retrospective clinical database. Cases were classified as 'complex' if severe matting, necrosis, atresia or perforation were present; 'simple' ga

Transcriptomic landscapes of the endometrium of dairy cows with clinical or subclinical endometritis.

Subclinical (SCE) and clinical endometritis (CE) are distinct manifestations of reproductive tract inflammatory disease in dairy cows. The development of both conditions stems from postpartum dysregulation of the inflammatory response or a shift in the composition of the uterine microbiome. To gain further insight into the host responses associated with these distinct conditions, we aimed to identify changes in the endometrial transcriptomic landscape in healthy postpartum dairy cows compared with those diagnosed with SCE or CE. Twenty-four multiparous Holstein cows were evaluated for uterine

Endometriosis and Oocyte Quality: Morphological Alterations, Developmental Competence, and Modifiable Strategies for Reproductive Longevity.

Endometriosis is a chronic, estrogen-dependent inflammatory disorder that is increasingly recognized as a systemic condition with profound implications for female reproductive potential. In addition to pelvic distortion and impaired folliculogenesis, growing evidence indicates that intrinsic alterations in oocyte morphology, mitochondrial function, and developmental competence contribute to infertility. The disease is driven by a multifactorial interplay of somatic mutations, epigenetic remodeling, immune dysregulation, and aberrant steroid signaling, which together create a pro-inflammatory,

Elevated CHI3L1 and COX-2 levels correlate with menstrual pain severity in endometriosis.

OBJECTIVE: Endometriosis is a chronic gynecological disease associated with inflammation and severe pelvic pain in 6%-10% of women of reproductive age. Although the pathophysiology and management of endometriosis are currently understood, further research is needed to develop new diagnostic methods, particularly those involving inflammatory pathways, which could guide targeted therapies. This study aims to analyze the differences in levels of Chitinase-3-like protein 1 (CHI3L1) and Cyclooxygenase-2 (COX-2) in the walls of endometriosis cysts compared to non-endometriosis cysts (controls) and t

Natural products modulate programmed cell death signaling mechanism for treating endometriosis: a review.

Endometriosis (EMs) is a gynecological inflammatory disease that depends on estrogen. Its chief symptoms include dysmenorrhea, chronic pelvic pain, reduced fertility, and pelvic masses. Although various hormonal therapies and surgical treatments are available, their long-term effectiveness is limited, recurrence rates are high, and side effects are significant. Programmed cell death (PCD) is a genetically regulated mechanism of cell clearance that includes apoptosis, autophagy, ferroptosis, pyroptosis, and necroptosis. Numerous studies showed that dysregulation of PCD is strongly associated wi

Dydrogesterone and Letrozole Combination for the Treatment of Endometriosis: A Mechanism-Based Therapeutic Approach.

Endometriosis is a complex, estrogen-dependent disease with limited effective treatments that often focus on symptom management rather than addressing the underlying pathology. Current therapies, such as progestins and GnRH agonists, have significant side effects (weight gain, mood changes, decreased bone mineral density, and menopausal symptoms) and fail to prevent disease recurrence or address fertility concerns. The current study is the first to demonstrate the therapeutic potential of combined treatment with dydrogesterone (a progestin) and letrozole (an aromatase inhibitor) in a preclinic

Therapeutic effects of Gardenia jasminoides Ellis on salpingitis in laying hens: mechanisms of inflammatory regulation and egg quality improvement.

BACKGROUND: Salpingitis (SALP) severely affects egg production and quality in laying hens, and conventional antibiotic therapies are limited by drug residues and antimicrobial resistance. Gardenia jasminoides (GF) is a traditional Chinese medicine that exhibits anti-inflammatory properties. However, its efficacy against SALP remains underexplored. METHODS: A 3-week trial assigned 108 hens to blank, model, positive control, and GF-treated groups (0.5%, 1.0%, and 1.5% dietary supplementation). SALP was induced in all groups, except the blank control group, via intrafallopian infusion of white io

Type 1 Dolichocolon as a Potential Anatomic Co-morbidity in Children with Perianal Crohn's Disease.

BACKGROUND: Perianal Crohn's disease (PCD) represents one of the most severe and refractory forms of pediatric inflammatory bowel disease (IBD). Constipation and colonic redundancy, particularly type 1 dolichocolon (T1-DC), may increase distal rectosigmoid pressure, and exacerbate perianal pathology. We hypothesized that T1-DC is more common in children with PCD than in those with uncomplicated ileocolonic Crohn's disease (CD) or non-IBD controls. METHODS: Retrospective electronic medical record search was performed. DC type was determined radiographically using established criteria, focusing

The cost-effectiveness of male-partner treatment to prevent recurrence of bacterial vaginosis.

Bacterial vaginosis (BV) is a common vaginal condition with high recurrence after standard treatment. Male partners may serve as reservoirs for BV-associated bacteria. Randomised controlled trial (RCT) data show that adding male-partner treatment to standard female care reduces recurrence compared with female-only treatment. We assessed the economic viability of this approach. A cost-effectiveness analysis was conducted using decision-analytic models informed by RCT data. Country-specific models for Australia and South Africa represented high- and middle-income healthcare settings. Simulated c
