Myelin Oligodendrocyte Glycoprotein Antibodies in Breast Milk in a Lactating Woman With Severe Enterovirus-Associated MOGAD.

OBJECTIVES: Myelin oligodendrocyte glycoprotein antibody (MOG-IgG)-associated disease (MOGAD) is a distinct neuroinflammatory disorder, which may be preceded by infections. Transmission of MOG-IgG from blood into breast milk has not been reported previously. METHODS: We present the case of a 38-year-old lactating woman with a severe manifestation of MOGAD after enterovirus gastroenteritis, who was still lactating at that time. Alongside with detailed clinical characteristics, we measured MOG-IgG in breast milk and serum from the breast-fed infant. RESULTS: Our patient experienced a severe pr

Azathioprine Inhibits Hepatitis A Virus Replication In Vitro.

Hepatitis A virus (HAV) infection can occasionally cause acute severe hepatitis. Patients with this disease sometimes need to undergo liver transplantation with immunosuppressants. Although rare, breakthrough HAV infections, despite vaccination, appear to be more common among immunocompromised populations. The effect of immunosuppressants on HAV replication is unclear. In this study, we examined the effects of immunosuppressants on HAV HA11-1299 genotype IIIA replication in human hepatocytes, finding that azathioprine inhibited HAV replication with a half-maximal inhibitory concentration of 0.

Systemic inflammatory and metabolic disease progression persists despite prevention of myocarditis in Isg15-deficient Coxsackievirus B3 infection.

INTRODUCTION: Type I interferon responses are critical determinants of organ-specific antiviral control and metabolic adaptation during Coxsackievirus B3 (CVB3) infection. The interferon-stimulated gene ISG15 integrates antiviral defense with immunometabolic adaptation, and its deficiency aggravates viral myocarditis, systemic inflammation, metabolic wasting, and impaired cardiac performance. Previous studies further demonstrated impaired cardiac energetic adaptation in Isg15-deficient mice during CVB3 infection, raising the possibility that aggravated myocardial infection contributes to infec

Spatial heterogeneity in the temperature-hand, foot, and mouth disease association among children: A multicounty time-series study in western China.

While meteorological and socioeconomic factors are well-documented modifiers of spatial heterogeneity in temperature-hand, foot, and mouth disease (HFMD) associations, substantial unexplained heterogeneity remains. This study investigates underexplored environmental modifiers-including extreme temperature events (heat waves and cold spells), air pollution, and normalized difference vegetation index (NDVI)-by analyzing 484,928 HFMD cases among children under six years old in western China (2013-2019) using distributed lag nonlinear models and meta-regression. We found that cold spells (accounte

Molecular epidemiological and genetic characteristics of coxsackievirus A6 circulating in mainland China from 1992 to 2023.

Hand, foot, and mouth disease (HFMD) is a common global infectious disease in children. Coxsackievirus A6 (CVA6) has emerged as the most important pathogen of HFMD. Molecular epidemiological and evolutionary surveillance of CVA6 is vital for the prevention and control of HFMD outbreaks. This study comprehensively analyzed the molecular epidemiology and genetic characteristics of CVA6 circulating in mainland China during 1992-2023. Phylogenetic analysis based on VP1 sequences revealed that in addition to the previously reported subgenotypes B1, B2, C1, D2, D3a, and D3b, genotype A was first ide

A single residue in the VP3 capsid protein governs virulence and informs live-attenuated vaccine design for coxsackievirus A6.

UNLABELLED: Coxsackievirus A6 (CVA6) is a major pathogen of hand, foot, and mouth disease, yet the viral determinants of its severe pathogenicity remain undefined, hampering intervention strategies. Here, by comparing a highly lethal clinical isolate (CVA6-HeB) with an attenuated strain (CVA6-TW141), we identified the capsid-encoding P1 region as the primary virulence determinant in neonatal mice. Fine-mapping identified a single critical residue at position 238 of VP3 (VP3-238): glutamic acid (E) in virulent CVA6-HeB versus alanine (A) in attenuated CVA6-TW141. Introducing E238 into the atten

String-Powered Microfluidic Chip Integrating Heparin-Mediated One-Pot RT-RPA/CRISPR-Cas12a for Multiplex Detection of HFMD Viruses.

Hand, foot, and mouth disease (HFMD) is a common childhood infection caused by enteroviruses, which exhibit distinct regional and seasonal epidemiological patterns. Wastewater-based epidemiology is a crucial tool for monitoring population infection dynamics and viral subtype distribution. However, the lack of effective on-site viral detection methods limits timely early warning and effective surveillance of infectious disease outbreaks. This study developed a one-pot RT-RPA/CRISPR-Cas12a assay-based, string-powered flywheel microfluidic chip for the multiplex detection of HFMD viruses in waste

Complete genome sequencing and molecular characterization of indigenous enterovirus A71 genogroups D and G of India.

BACKGROUND: Enterovirus 71 (EV-A71), a neurotropic member of the Picornaviridae family, is a positive-strand RNA virus comprising of seven recognized genogroups (A-G) and has caused significant outbreaks of hand, foot, and mouth disease worldwide. Despite prior identification of D and G genogroups in India, comprehensive full-genome characterization of these indigenous lineages remains unreported. METHODS: We performed next-generation sequencing (NGS) and molecular phylogenetic analyses on two Indian isolates from acute flaccid paralysis cases: R13223-IND-02 (genogroup D) and V11-2209-01 (gen

Global Evolution and Phylodynamics of Coxsackievirus A4.

Coxsackievirus A4 (CVA4) is an enterovirus associated with diverse clinical syndromes, yet its global evolution and transmission dynamics remain poorly defined. This study aimed to characterize the molecular epidemiology, evolutionary history, and phylodynamic patterns of CVA4 at both local and global scales. In 2024, 34 CVA4 infections were identified during a localized herpangina outbreak in Beijing, primarily affecting children under 5 years of age, with a pronounced peak in July. High detection and isolation rates, together with temporal clustering in mid-2024, indicate a genuine short-ter

Antigenic characterization and mapping of a conserved neutralizing epitope on VP1 of coxsackievirus A5.

BACKGROUND: Coxsackievirus A5 (CV-A5) has re-emerged as an important pathogen associated with hand, foot, and mouth disease (HFMD) in young children. However, the antigenic structure of CV-A5 and the key determinants responsible for neutralizing antibody responses remain largely undefined, hindering rational vaccine design and antibody-based intervention strategies. METHODS: To investigate the relationship between viral particle composition and humoral immune responses, three forms of CV-A5 particles, including empty particles (EPs), full particles (FPs), A-particles (APs) were purified from

Evolutionary and molecular characteristics of high-Shannon entropy codons in VP1 of Coxsackievirus A6.

Coxsackievirus A6 (CVA6) has emerged as a predominant global pathogen of hand, foot and mouth disease, yet its evolutionary and molecular characteristics remain poorly understood. This study integrated 10 year molecular surveillance data from central China (2013-2024) with global CVA6 sequences to analyse high-entropy codons in the VP1 capsid protein and their co-evolution patterns. Phylogenetic and Bayesian evolutionary analyses indicated that CVA6 strains in China primarily clustered into sub-genotype D3, with an estimated global VP1 substitution rate of 3.62×10⁻³ substitutions/site/year. Fibrillarin-mediated 2'-O-methylation serves as a translation brake on uncapped enterovirus RNA.

2'-O-methylation (Nm) is a key epitranscriptomic mark across diverse RNAs, yet its role in antiviral defense remains largely unexplored. To systematically investigate the landscape and functional significance of internal Nm modifications on enterovirus RNAs, a sensitive sequencing method termed RACE-Nm was established, in combination with the machine learning-based predictor NanoNm, to map Nm sites on low-abundance RNAs at single-nucleotide resolution. A conserved Nm site was identified at nucleotide 307C within the viral internal ribosome entry site (IRES), which was catalyzed by the methyltr

Structure-based macrocyclization of α-ketoamides leads to potent inhibitors of coronaviral and enteroviral proteases.

Viral proteases represent validated targets for direct-acting antivirals and the treatment of associated infections. In co-crystal structures of Mpro of SARS-CoV-2 with peptidomimetic inhibitors, we noticed a spatial proximity of sidechains filling the S1' and S2 pockets, as well as those filling S3 and S1 pockets. To enhance molecular rigidity, the proximal residues were conformationally fixed by macrocyclization. We report the synthesis of two macrocyclic series, i.e. exocyclic nitriles with linked P3 and P1 residues and endocyclic α-ketoamides with linked P1' and P2 residues, and characteri

VP1 molecular evolution of type 2 immunodeficiency-related vaccine-derived polioviruses (iVDPV2) in a patient with primary immunodeficiency disease (PID).

Vaccine-derived polioviruses (VDPVs) arising after use of oral poliovirus vaccine (OPV) are an important impediment to the polio endgame, particularly in settings with low population immunity or prolonged infections in immunodeficient individuals. If immunocompromised individuals such as patients with primary immunodeficiency (PID) receive OPV, the live attenuated vaccine virus will replicate in their intestinal tract and chronically shed in the stool. Long-term proliferation of the vaccine virus in the enteric cells of the PID patients causes several mutations, which eventually culminates in t

Emergence of vaccine-derived poliovirus strains from the novel oral polio vaccine in the Central African Republic.

UNLABELLED: The Central African Republic (CAR) is a high-risk country for poliomyelitis outbreaks because of difficulties in closing immunity gaps due to the lack of resources, logistics issues, and insecurity. The country experienced multiple poliomyelitis cases since 2019. A novel type 2 oral polio vaccine (nOPV2) engineered to lower the risk of reversion of the vaccine strain was deployed in the CAR in 2022-2023 to counter active serotype 2 poliovirus transmission. This study reports routine poliovirus surveillance conducted from November 2021 through October 2023 in the CAR. Polioviruses i

Evaluating the safety profile of poliomyelitis vaccines: insights from post-marketing surveillance.

Poliomyelitis (POLIO) vaccines are widely used because of their high clinical effectiveness and minimal side effects. However, as post-marketing surveillance data have been collected, several serious adverse events (SAEs) linked to poliomyelitis vaccination have been reported.Currently, there is a lack of quantitative studies using real-world data, and detailed information about these adverse events is scarce. To fill this gap, adverse event signals associated with the poliomyelitis vaccine were extracted and analyzed using U.S. data. The Vaccine Adverse Event Reporting System (VAERS) serves as a

A multi-center, open-labelled, randomized controlled extended phase III non-inferiority clinical trial to evaluate the immunogenicity and tolerability of poliomyelitis vaccine (Vero cells), inactivated, Sabin strains administered with or without routine infant vaccines.

BACKGROUND: Oral poliovirus vaccine (OPV) has been reported to cause vaccine-derived poliovirus and vaccine-associated paralytic poliomyelitis, and the limited global supply of conventional IPV has led many countries to rely on fractional-dose IPV regimens alongside OPV. The current study aims to assess the sIPV safety and immunogenicity when given concurrently with or in a staggered manner with routine immunization. METHODS: A multi-country, multi-center, open-label, randomized controlled, extended phase III non-inferiority clinical trial was conducted with 1442 healthy infants aged 6-8 week
