Spontaneous Tumour Lysis Syndrome in Metastatic Endometrial Carcinoma.

Tumour lysis syndrome (TLS) is a potentially fatal oncological emergency characterised by rapid cellular breakdown resulting in hyperkalaemia, hyperphosphataemia, hyperuricaemia, and hypocalcaemia, with subsequent acute kidney injury, cardiac dysrhythmias, and multi-organ dysfunction. It most commonly occurs following initiation of anti-cancer therapy in highly proliferative haematological malignancies. Spontaneous tumour lysis syndrome (STLS), occurring in the absence of a therapeutic trigger, is considerably rarer, particularly in solid tumours. We report a 55-year-old female presenting with

The pedfix technique: a new minimally invasive method of peritoneal dialysis catheter insertion with secure fixation to the abdominal wall-a preliminary experience.

PURPOSE: Peritoneal Dialysis (PD) is the most common modality of renal replacement therapy in children with acute kidney injury (AKI). Various open and laparoscopic techniques for the placement of peritoneal dialysis (PD) catheter, with or without intra-abdominal fixation, have been described. The incidence of catheter migration after PD is 0-19%. We present a new, modified technique which minimizes the risk of catheter migration and blockage. MATERIALS AND METHODS: For 18 years a laparoscopic assisted PD catheter insertion has been used by the first author with a single 5 mm Port at the umbi

Multifocal small bowel gastrointestinal stromal tumor causing perforation peritonitis: A rare cause of acute abdomen.

A female patient in late 40s presented with severe abdominal pain, distension and fever for a week. Ultrasound abdomen revealed ascites and a large indeterminate mass in the pelvis, but bowel loops were not dilated. Contrast-enhanced CT (CECT) abdomen revealed a large, exophytic, heterogeneously enhancing intraperitoneal mass, arising from a jejunal loop in the pelvis; showing fistulous communication with an adjacent small bowel loop. Multiple smaller enhancing rounded, serosal masses were also seen in the jejunum and ileum along with pneumoperitoneum, moderate ascites and peritoneal enhanceme

"Incidentaloma" as Occasional Finding During Laparoscopic Surgery: How Laparoscopy Makes The Difference!

BACKGROUND: In the scientific nomenclature, "incidentaloma" is the medical term to describe incidentally found asymptomatic tumors. We adapted the term "incidentaloma" to describe the finding of additional pathology during laparoscopic surgery conducted for a different indication. This study aimed to highlight the role of laparoscopy in identifying and addressing hidden concurrent conditions, thereby preventing the need for further surgical interventions. METHODS: Five hundred ninety-five patients undergoing laparoscopic procedures from January 2018 to January 2023 were enrolled. Patients wer

A comparative analysis of retroperitoneal small-incision open pyeloplasty and laparoscopic pyeloplasty for infantile hydronephrosis.

BACKGROUND: With the development of minimally invasive surgery, laparoscopic pyeloplasty has become the mainstream surgery for ureteropelvic junction obstruction(UPJO), while open surgery is used less frequently. However, in regions where endoscopic techniques are not yet well-developed or in very young infants, open pyeloplasty still plays a significant role. This study aims to evaluate the clinical efficacy and safety of the retroperitoneal small-incision open pyeloplasty in infants by comparing it with laparoscopic pyeloplasty. METHODS: This retrospective cohort enrolled 117 infants diagno

Pseudo-renal failure with hyponatremia and proteinuria after gynecologic surgery: a case report with literature comparison.

BACKGROUND: Acute kidney injury (AKI) is defined by an increase in serum creatinine according to the Kidney Disease: Improving Global Outcomes criteria. In contrast, pseudo-renal failure is a rare condition characterized by azotemia and electrolyte imbalance that mimic AKI, while the actual renal function remains preserved. Because of its rarity and nonspecific presentation, pseudo-renal failure is often misdiagnosed as true AKI. We report a rare case of pseudo-renal failure caused by intraperitoneal bladder rupture following gynecologic surgery. This case is unique in that pseudo-renal failur

Malakoplakia mimicking pelvic malignancy with liver metastasis and peritoneal carcinomatosis.

Malakoplakia is a rare chronic inflammatory condition characterised by defective macrophage function, most commonly affecting the genitourinary tract. Although usually localised, it may occasionally present as a mass-forming lesion mimicking malignancy. We report the case of a 46-year-old woman presenting with a pelvic mass, acute kidney injury, and hydroureteronephrosis. Imaging demonstrated a necrotic 10.3cm pelvic mass with lymphadenopathy and hepatic lesions, raising suspicion of metastatic gynaecological malignancy. Surgical exploration identified a fibrotic, thickened pelvic peritoneum w

Late Cervical Recurrence of Invasive Lobular Carcinoma Ten Years After Primary Breast Cancer: A Case Report and Review of the Literature.

Invasive lobular carcinoma (ILC) accounts for approximately 15% of breast cancers and the most common neoplasm in the female population. Cervical involvement is exceptionally rare and often underrecognized. This relationship is well-defined in the context of breast and ovarian cancer syndrome related to BRCA gene mutations. However, it is also observed in rare but underreported cases of cervical metastases originating from breast cancer. The objective of this manuscript is to describe a rare case of cervical recurrence of invasive lobular carcinoma and summarize comparable case to guide future

Disseminated peritoneal leiomyomatosis incidentally diagnosed during surgery for torsion of a subserosal uterine fibroid: a case report.

BACKGROUND: Disseminated peritoneal leiomyomatosis is a rare condition characterized by the dissemination of histologically benign leiomyomas within the peritoneal and retroperitoneal cavities. Intra-abdominal findings can resemble the peritoneal dissemination of gynecologic or gastrointestinal malignancies. Disseminated peritoneal leiomyomatosis has been predominantly reported in patients with a history of gynecological treatment or during pregnancy. Herein, we present a case of disseminated peritoneal leiomyomatosis diagnosed incidentally following the torsion of a uterine fibroid in a patie

Primary peritoneal clear cell carcinoma with metastasis mimicking ovarian carcinoma: a case report and literature review.

BACKGROUND: Primary peritoneal clear cell carcinoma (PPCCC) is an exceptionally rare malignancy that closely resembles gynecologic epithelial cancers clinically and histologically. Its pathogenesis is poorly understood, with possible origins from Müllerian metaplasia or malignant transformation of endometriosis. CASE PRESENTATION: A 48-year-old woman with no history of endometriosis or hormone therapy presented with acute lower abdominal pain. Imaging revealed a pelvic mass and a lesion in the pancreatic tail. The patient underwent hysterectomy with bilateral salpingo-oophorectomy and distal p

Abdominal screening with ultrasound in children with suspected acute appendicitis.

OBJECTIVES: Ultrasound (US) is the first-line modality for the assessment of acute appendicitis (AA) in children. Because alternative diagnoses in these patients are common, some centers have expanded US to include screening of the entire abdomen and pelvis. However, insufficient data prevent consensus on whether this practice improves the detection of abdominal pathologies in the differential diagnosis for AA. To determine whether comprehensive US screening of the abdomen/pelvis improves the detection rate of abdominal pathologies among children presenting with symptoms consistent with AA, an

Delayed posterior rectus sheath rupture presenting as intestinal obstruction: A case report and review.

Extended-view totally extraperitoneal repair (eTEP) is one of the recently introduced, advanced minimal access approaches for the repair of ventral hernias. Posterior sheath rupture (PRSR), though rare, is a lethal complication of eTEP, exposing the polypropylene mesh to the viscera. Few cases of PRSR have been reported following eTEP ventral hernia; however, delayed presentations as acute small bowel obstruction, has not been reported so far. We present the case of acute intestinal obstruction following eTEP ventral hernia repair after 10 months following the primary surgery. A 67-year-old fe

Jejunal Diverticulitis With Contained Perforation and Abscess Successfully Managed With Conservative Therapy: A Case Report and Review of the Literature.

Jejunal diverticulosis is a rare condition that predominantly affects elderly individuals and is often asymptomatic. However, complications such as diverticulitis, perforation, and abscess formation can result in significant morbidity. Perforated jejunal diverticulitis has traditionally been managed surgically, although increasing evidence supports conservative treatment in selected patients. We report a case of a 74-year-old female with hypothyroidism, hyperlipidemia, and Alzheimer's dementia who presented with acute bilateral lower abdominal pain and nausea. Uterine Leiomyosarcoma Incidentally Diagnosed After Sigmoid Colon Perforation: A Case Report and Review of the Literature Highlighting Individualized Surgical and Oncologic Decision-Making.

Introduction: Uterine leiomyosarcoma (uLMS) is a rare, highly aggressive malignancy arising from the smooth muscle tissue of the uterine wall. It typically presents with abnormal vaginal bleeding, pelvic pain, or a pelvic mass. In rare instances, symptoms may result from local invasion or metastasis. We report a case of uLMS initially diagnosed following colonic perforation due to direct tumor invasion, accompanied by a comprehensive narrative review of the literature on the incidental identification of uterine sarcomas during emergency general surgery to further emphasize the diagnostic chall

A strengthening yet less extreme Atlantic-European jet during winter.

It is known that beta-lactam antibiotics can conjugate to lysine and histidine residues on proteins via the carbonyl group of the opened beta-lactam ring. However, it is not known which proteins these drugs target and there is little work addressing whether conjugation is preferential for some proteins over others or if conjugation has functional consequences for the protein. We have previously shown that the beta-lactam antibiotic benzylpenicillin (BP) conjugates to IFN-gamma and reduces its activity. This interaction demonstrates selectivity, as BP does not bind to IL-4. Here, we extend our

Cryptic clues from an acute abdomen: a case report of peritonitis turning into disseminated cryptococcosis.

BACKGROUND: Cryptococcosis is a life-threatening opportunistic fungal infection that primarily affects immunocompromised individuals, most commonly presenting as meningitis. Peritoneal involvement is exceedingly rare and can closely mimic common acute surgical conditions, often resulting in delayed diagnosis and treatment. This case describes an unusual presentation of disseminated cryptococcosis as acute peritonitis with subsequent progression to cryptococcal meningitis; underscoring the diagnostic challenges posed by atypical intra-abdominal infection. CASE PRESENTATION: A previously health

Infectious mononucleosis mimicking Fitz-Hugh-Curtis Syndrome: The diagnostic complexity of concurrent Chlamydia trachomatis infection and false-positive syphilis serology.

Infectious mononucleosis (IM), typically caused by the Epstein-Barr virus, presents with a wide range of clinical manifestations. This diversity includes rare organ complications and the potential for false-positive syphilis serology results, making the diagnosis challenging. We report a rare and instructive case of IM presenting as acute acalculous cholecystitis (AAC). This case was complicated by concurrent false-positive syphilis serology results, prompting strong clinical suspicion for Fitz-Hugh-Curtis syndrome (FHCS). A 21-year-old woman presented with fever, right upper quadrant pain, an

Residential radon in Peru: challenges for health surveillance and lung cancer prevention.

The Journal of Genetics, started by William Bateson in 1910, played a distinguished role in the early years of genetics. However, it stopped publishing in 1978. The Indian Academy of Sciences revived it in 1985, and has published it regularly since then. To commemorate this landmark, I highlight one of the 17 articles published that year.
