Initial Results of FAPI PET/MRI to Assess the Extent of Endometriosis.

Endometriosis is a common gynecologic disorder often associated with chronic pelvic pain, infertility, and reduced quality of life. Conventional imaging modalities, such as transvaginal ultrasound and MRI, may underestimate the extent of disease, particularly in deep infiltrating endometriosis. Fibroblast activation protein (FAP) is highly expressed in remodeling endometrium and has been implicated in endometriotic lesions. This study investigates the feasibility of integrating FAP-targeted PET/MRI to improve endometriosis detection. Methods: This retrospective study included 18 premenopausal

Malakoplakia mimicking pelvic malignancy with liver metastasis and peritoneal carcinomatosis.

Malakoplakia is a rare chronic inflammatory condition characterised by defective macrophage function, most commonly affecting the genitourinary tract. Although usually localised, it may occasionally present as a mass-forming lesion mimicking malignancy. We report the case of a 46-year-old woman presenting with a pelvic mass, acute kidney injury, and hydroureteronephrosis. Imaging demonstrated a necrotic 10.3cm pelvic mass with lymphadenopathy and hepatic lesions, raising suspicion of metastatic gynaecological malignancy. Surgical exploration identified a fibrotic, thickened pelvic peritoneum w

Late Cervical Recurrence of Invasive Lobular Carcinoma Ten Years After Primary Breast Cancer: A Case Report and Review of the Literature.

Invasive lobular carcinoma (ILC) accounts for approximately 15% of breast cancers and the most common neoplasm in the female population. Cervical involvement is exceptionally rare and often underrecognized. This relationship is well-defined in the context of breast and ovarian cancer syndrome related to BRCA gene mutations. However, it is also observed in rare but underreported cases of cervical metastases originating from breast cancer. The objective of this manuscript is to describe a rare case of cervical recurrence of invasive lobular carcinoma and summarize comparable case to guide future

The endometriosis-adenomyosis spectrum: shared pathophysiology and microenvironment-driven disease divergence.

Endometriosis and adenomyosis are common gynecologic disorders associated with dysmenorrhea, chronic pelvic pain, and infertility. Although they share several molecular features, the mechanisms by which endometrium-derived tissues develop distinct pathological phenotypes in different tissue environments remain incompletely understood. This review summarizes shared and divergent pathogenic mechanisms, focusing on lesion-specific microenvironments. This narrative review was based on a PubMed literature search from the year of the first publication through December 2025 using terms related to end

The "White line sign": A novel sonographic soft marker of pelvic endometriosis.

INTRODUCTION: Ultrasound assessment of superficial endometriosis (SE) is difficult, and it is still mainly diagnosed via surgery. We hypothesized that a thickening of the subperitoneal layer of tissue overlying the anterior pouch of Douglas (POD), labeled the "White line sign" (WLS), could indicate the presence of SE. The aim of the study was to investigate the association between the WLS and the finding of endometriosis at histology. MATERIAL AND METHODS: This prospective diagnostic accuracy study included 100 premenopausal women with chronic pelvic pain who underwent transvaginal ultrasound

Human Endometriotic Lesion-Derived Small Extracellular Vesicles Impair Macrophage Function in the Peritoneal Microenvironment.

Endometriosis (EM) is a chronic inflammatory disease that affects ∼10% of women during reproductive age. It is characterised by ectopic (ECT) growth of endometrial-like tissue mainly in the pelvic cavity. Small extracellular vesicles (sEVs) mediate cellular interactions, but their function remains poorly understood in the pathogenesis of EM. 3D endometrial epithelial organoids (EEOs) from ECT lesions and eutopic (EUT) endometrium from EM patients and controls were established to investigate sEVs. Multiplex bead-based flow cytometry revealed CD133/1 and EpCAM as dominant markers on EEO-sEVs, wi

Symptom Burden in Adenomyosis: A Comparative Analysis With Peritoneal and Deep Infiltrating Endometriosis.

BACKGROUND/AIM: Adenomyosis frequently coexists with endometriosis and shares overlapping symptoms; however, the symptom burden of isolated adenomyosis remains insufficiently defined. The aim of this study was to compare the severity and distribution of symptoms in patients with isolated adenomyosis versus those with peritoneal/ovarian endometriosis and deep infiltrating endometriosis (DIE). PATIENTS AND METHODS: This retrospective study analyzed standardized preoperative symptom questionnaires from women who underwent surgery for suspected endometriosis at a tertiary referral center in 2021.

Targeting TLR4 Attenuates Endometriosis Progression by Suppressing NF-κB/NLRP3 Inflammasome Activation and Angiogenesis.

Endometriosis is a chronic inflammatory disorder affecting approximately 10% of reproductive-age women, yet non-hormonal therapeutic options remain limited. This study investigates the role of the TLR4/NF-κB/NLRP3 inflammasome axis in endometriosis pathogenesis and evaluates the therapeutic potential of pharmacologic TLR4 inhibition. Ectopic endometriotic tissues, eutopic endometrium, and peritoneal fluid were collected from 15 patients with ovarian endometriosis and 15 control subjects. The endometriotic epithelial cell line 11Z was stimulated with LPS and ATP with or without the TLR4 inhibit

BACKGROUND: Small bowel adenocarcinoma (SBA) is considered an unusual malignancy with vague presentations thus risk of delayed diagnosis. Despite being rarely reported in the literature, SBA can present with gynecological manifestations, especially adnexal mass, misleading surgeons in dealing with it as a primary gynecological malignancy. METHODS: We report a case of a 60 year old female patient who had SBA with pelvic mass as initial presentation. Identification of intestinal origin was done intraoperatively. ESPriT1: the effectiveness of laparoscopic treatment of isolated superficial peritoneal endometriosis for managing chronic pelvic pain in women: a randomised controlled feasibility trial.

Endometriosis is a chronic pain condition that affects >190 million people worldwide. It is recommended in clinical guidelines to remove the most common type of endometriosis, 'superficial peritoneal endometriosis' (SPE), if it is diagnosed at laparoscopy (keyhole surgery). However, the evidence to support removal of SPE is based on small studies with short-term follow-up. The aim of this small feasibility trial was to determine what proportion of women with pelvic pain undergoing laparoscopy for suspected endometriosis would be willing to be randomly allocated to have surgical removal of SPE

Patient reported outcome measures in intra-peritoneal adhesive disease: a scoping review.

BACKGROUND: Intra-peritoneal adhesive disease (IPAD) can lead to debilitating symptoms including bowel obstruction, chronic abdominopelvic pain, and infertility. Standard tools to evaluate patient experiences for this disease process have not been described. This scoping review aims to (1) identify existing Patient Reported Outcome Measures (PROMs) related to IPAD and (2) evaluate the utility and quality of available IPAD-related PROMs, measured by predefined adhesive disease domains. METHODS: Systematic searches were completed in PubMed using terms specific to IPAD-related symptoms and Quali

Peritoneal Incretin Deficiency and Tirzepatide as a Multi-Axis Adjuvant Hypothesis in Treatment-Refractory Endometriosis: A Mechanistic Framework Linking Metabolism, Immunity, Fibrosis, and Nociception.

Endometriosis is increasingly recognized as a chronic systemic disorder extending beyond the classical estrogen-dependent paradigm, integrating metabolic, immune, fibrotic, and nociceptive pathways that sustain lesion persistence and refractory pelvic pain. We propose a mechanistic, translational hypothesis in which tirzepatide, a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist, may modulate four interconnected pathological axes of refractory endometriosis-Warburg-type metabolic reprogramming with lactate accumulation, peritoneal imm

A case of omental endometriosis complicated by hemorrhagic ascites leading to pelvic encapsulated effusion.

BACKGROUND: Endometriosis is an estrogen-dependent benign inflammatory disorder characterized by the implantation of active endometrial tissue (glands or stroma) outside the uterus, with pelvic adhesions as a core hallmark. Omental endometriosis is an extremely rare subtype of extra-pelvic endometriosis (accounting for < 5% of all extra-pelvic endometriosis cases), and its concurrent presentation with massive hemorrhagic ascites and de novo pelvic encapsulated effusion without predisposing factors (e.g., pelvic surgery, pelvic inflammatory disease) remains scarcely reported in existing clinica

Low-energy extracorporeal shock wave therapy is associated with reduced inflammation and fibrosis in an E2-induced rat model of endometriosis and remains effective under menopause-mimicking conditions.

BACKGROUND: Endometriosis is an estrogen-dependent inflammatory disorder marked by ectopic lesions, chronic pelvic inflammation, and frequent recurrence. Extracorporeal shock wave therapy (ESWT) is a non-invasive modality with anti-inflammatory and regenerative potential, but its efficacy under varying estrogen conditions remains unclear. METHODS: Endometriosis-like lesions were induced in female rats by suturing uterine fragments to the peritoneum, followed by daily 17β-estradiol (E2) or PBS for 28 days. Sprague-Dawley rats were assigned to control, E2, E2 + ESWT (0.1 or 0.25 mJ/mm2), or ova

Modulating the Estrobolome and Inflammatory Microenvironment in Endometriosis: The Role of Microbiome-Targeted Interventions and Nutritional Compounds.

Endometriosis is a chronic estrogen-dependent systemic inflammatory disorder characterized by ectopic implantation of endometrial-like tissue, primarily on the ovaries, pelvic peritoneum, and bowel. According to the World Health Organization (WHO) estimate updated in 2025, endometriosis affects approximately 10% (about 190 million) of reproductive-age women worldwide and is a major contributor to chronic pelvic pain, dysmenorrhea, dyspareunia, and infertility. Standard medical therapies focus on ovarian suppression, which alleviates symptoms but precludes conception and carries significant met

Peritoneal Inclusion Cyst in an Unusual Location in a "Virgin" Abdomen: A Diagnostic Dilemma Expanding the Clinical Spectrum.

Peritoneal inclusion cysts (PICs) are benign, multilocular fluid-filled lesions that predominantly affect women of reproductive age, often arising after abdominal surgery or chronic inflammation. We report a case of a 28-year-old female presenting with chronic left upper-quadrant abdominal pain for 1 year. Radiological evaluation revealed a large cystic lesion closely abutting the left adrenal gland and adjacent organs, creating a diagnostic dilemma regarding its tissue of origin. The patient was planned for diagnostic laparoscopy, which revealed a large, thin-walled cyst containing clear sero

Superficial Peritoneal Endometriosis Beyond Surgical Diagnosis: A Narrative Review of Emerging Functional and Molecular Perspectives.

Background and Objectives: Superficial peritoneal endometriosis (SPE) remains one of the most difficult endometriosis phenotypes to diagnose non-invasively, because lesions are frequently small, multifocal, and poorly detectable using conventional imaging. Diagnostic laparoscopy therefore remains the reference standard for direct visualization of superficial peritoneal lesions. However, the inconsistent relationship between visible lesion burden and pain severity, together with the multifactorial nature of chronic pelvic pain, highlights the limitations of a purely lesion-based diagnostic mode
