Introducing Senhance robotic totally extraperitoneal (TEP) hernioplasty: initial experiences, learning curve, and mid-term outcomes from a single center.

Laparoscopic totally extraperitoneal (TEP) inguinal hernioplasty offers less pain and faster recovery than open repair, but it is technically demanding. The Senhance surgical system integrates eye-tracking, haptic feedback, and reusable instruments; however, robust clinical data in TEP remain limited. Between January 2019 and July 2023, we prospectively enrolled 109 adults who underwent Senhance-assisted TEP (S-TEP) at a single center; 57 retrospective laparoscopic TEP (L-TEP) served as an operative-time benchmark. Patients with prior pelvic-malignancy surgery, ascites, peritoneal dialysis, em

Comprehensive Review of Endometriosis Care.

Endometriosis is an estrogen-dependent, chronic inflammatory disorder characterized by the presence of endometrium-like tissue outside the uterus, affecting approximately 10% of individuals of reproductive age. It contributes to chronic pelvic pain, dysmenorrhea, and subfertility, resulting in substantial societal economic burdens. Genetic and environmental risk factors have been identified, and recent research suggests that endometriosis functions as a systemic disease affecting nonreproductive systems and increasing susceptibility to other health conditions. Various phenotypes-superficial pe

Associations between 1400 metabolites and subtypes of endometriosis: a two-sample Mendelian randomisation study.

BACKGROUND: Endometriosis is a chronic inflammatory disease with a prevalence of approximately 10% in women of childbearing age. Metabolic pathways have been demonstrated by previous studies to be potential avenues for the development of new therapeutic strategies and may be used for early diagnosis of the disease. This study aimed to investigate the potential causal relationships between 1400 metabolites and various endometriosis subtypes using Mendelian randomisation (MR) analysis. METHODS: Data from a genome-wide association study were analysed. MR analysis was performed using the inverse-

Postoperative adhesion formation: the role of peritoneal macrophages and targeting therapy.

BACKGROUND: Postoperative adhesion (PA) is the most common complication of abdominal or pelvic surgery, resulting in various complications, including small bowel obstruction, secondary female infertility, chronic abdominal pain and even death. Unfortunately, there are few effective methods for the prevention and treatment of it. Previous studies confirmed that coagulation response, inflammation, fibrosis and fibrinolysis played a central role in PA formation and a variety of cells, such as monocytes, macrophages, fibroblasts, neutrophils and mesothelial cells participated in this process. Peri

Association of white blood cell count to hemoglobin ratio with the life quality after laparoscopic surgery in patients with endometriosis.

BACKGROUND: Endometriosis (EM) is a common hormone-dependent and chronic inflammatory disease affecting women of reproductive age, characterized by pelvic pain, infertility, and reduced quality of life. Laparoscopic surgery is a primary treatment, yet the influence of preoperative factors on postoperative outcomes remains unclear. The white blood cell count to hemoglobin ratio (WHR), a novel marker of systemic inflammation and tissue hypoxia, has shown prognostic value in surgical oncology but its role in predicting postoperative quality of life in EM patients remains to be elucidated. Can hematological and biochemical parameters clinically predict the diagnosis of deep infiltrating endometriosis?

BACKGROUND: To investigate whether preoperative laboratory biomarkers can differentiate deep infiltrative endometriosis (DIE) from other endometriosis (OE) phenotypes (ovarian, tubal, peritoneal) non-invasively. METHODS: This retrospective case-control study analyzed data from patients operated for pathologically confirmed endometriosis at Kartal Dr. Lütfi Kırdar City Hospital between January 2015 and January 2025. Patients were divided into the DIE group (n = 286) and the OE group (n = 230). Demographic, clinical characteristics, and preoperative laboratory parameters, including complete bloo

Cross-Sectional Imaging of Pelvic Inflammatory Disease: Diagnostic Pearls and Pitfalls on CT and MR.

Pelvic inflammatory disease (PID) encompasses a broad range of infection-induced inflammatory disorders of the female upper genital tract, commonly caused by ascending sexually transmitted infections. Diagnosis is often challenging because of nonspecific or absent symptoms and the overlap with other pelvic pathologies. While clinical and laboratory assessments are essential, cross-sectional imaging plays a pivotal role, especially in complicated, atypical, or equivocal cases. This review focuses on the typical and atypical imaging features of PID and highlights the crucial roles of computed to

Entrapped in the Cocoon: A Rare Case of Sclerosing Encapsulating Peritonitis in a Patient on Dialysis.

Sclerosing encapsulating peritonitis (SEP), also known as abdominal cocoon syndrome, is a rare and potentially fatal condition characterized by the encasement of the small intestine within a dense fibrotic membrane. Fewer than 250 cases have been reported worldwide. We present a case of SEP in a patient on chronic peritoneal dialysis (PD). A 66-year-old female with end-stage renal disease on chronic PD presented with worsening abdominal pain, distension, and hypotension. Laboratory evaluation revealed leukocytosis and an elevated peritoneal fluid white blood cell count. Peritoneal fluid cultur

Endometriosis: A Review.

IMPORTANCE: Endometriosis is a chronic, estrogen-dependent, inflammatory disease defined by endometrial-like tissue (lesions) outside the uterine lining. It affects up to 10% of women worldwide, and 9 million women in the US, during reproductive years. OBSERVATIONS: Endometriosis has varying clinical presentations; however, 90% of people with endometriosis report pelvic pain, including dysmenorrhea, nonmenstrual pelvic pain, and dyspareunia, and 26% report infertility. Risk factors for endometriosis include younger age at menarche, shorter menstrual cycle length, lower body mass index, nullip

Rectosigmoid endometriosis, a rare cause of lower GI bleeding in a premenopausal woman: Case report.

INTRODUCTION AND IMPORTANCE: Endometriosis is a common gynecological condition where endometrial-like tissue grows outside the uterus, often affecting the ovaries, peritoneum, and pelvic ligaments. Sigmoid endometriosis is a rare manifestation that can present with nonspecific gastrointestinal symptoms, often mimicking conditions like irritable bowel syndrome (IBS) or inflammatory bowel disease (IBD), posing significant diagnostic challenges. This case report emphasizes the importance of considering sigmoid endometriosis in patients with chronic abdominal pain and gastrointestinal disturbances

A role of the uterine cervix in the pathogenesis of adenomyosis and endometriosis?

Data indicate a link between the cervix and pathologies like adenomyosis and endometriosis. Uterine flexion on the cervix, diameter and stiffness of the internal cervical orifice represent an obstacle to downstream menstrual flow, that when excessive increases the intensity of uterine contractions. Evidence indicates that intense uterine contractions damage the endometrial myometrial junction, favoring endometrial entrance into the myometrium, to induce adenomyosis, and retrograde menstrual flow into the peritoneal cavity, to induce endometriosis. As a consequence of local hyperestrogenism in

Intestinal Endometriosis, A Challenge for Gastroenterologists: A Narrative Review.

Endometriosis is a medical condition where endometrial tissue and glands are found outside the uterine cavity: on ovaries, pelvic peritoneum, rectovaginal septum, bladder, bowel and more. The ectopic tissue is estrogen dependent and cause inflammation and immune response and is responsible for chronic pain, infertility and disability of these patients. Without a specific biomarker and with a wide range of symptoms, the diagnosis is often overlooked or delayed. Abdominal and pelvic pain together with dysmenorrhea, dyspareunia, irregular bleeding, and cyclic alteration of intestinal habits are a

Research progress on the correlation between microbiota and endometriosis.

The microbiota constitutes a vital component of the human microenvironment, dynamically responding to physiological changes and playing crucial roles in maintaining normal bodily functions. Endometriosis (EMs), defined as a chronic, hormone-dependent inflammatory disorder, involves the ectopic implantation of endometrium-like tissue beyond the uterine cavity (e.g., ovaries, peritoneum). This pathology initiates local and systemic inflammatory cascades, leading to characteristic clinical manifestations such as dysmenorrhea (occurring in ∼ 84.9 % of patients), chronic pelvic pain, infertility, a

The Peritoneal Neovagina After Robotic-Assisted Peritoneal Flap Gender-Affirming Vaginoplasty: A Morphologic and Histologic Investigation of the Neovaginal Lining.

OBJECTIVE: To assess histologic changes in the peritoneal neovagina following robotic-assisted peritoneal flap gender-affirming vaginoplasty (RPGAV) to better guide shared decision-making regarding vaginoplasty techniques. RPGAV uses pedicled flaps of pelvic peritoneum to form the proximal neovagina. Early reports of peritoneal vaginoplasty suggested potential for "self-lubrication"; however, long-term maintenance of fluid production remains uncertain. METHODS: Five patients were recruited between August 2024 and March 2025. Patients included were at least 12 months post-RPGAV and had elected

Pilot Clinical and Radiomic Analysis of Deep Infiltrating Endometriosis of the Parametrium Using Shannon Entropy: A Retrospective Cohort Study.

OBJECTIVE: Ultrasound techniques for diagnosing deep infiltrating endometriosis (DIE) currently lack a quantitative method to assess microstructural heterogeneity in relation to diagnosis and clinical symptoms. This study evaluates Shannon entropy-based radiomics for differentiating DIE lesions from adjacent tissue and correlating these features with pain severity. METHODS: In this retrospective cohort study (2020-2024), fertile women with histologically confirmed parametrial endometriosis and high-quality ultrasound images were evaluated. Pain was measured using a 10-cm visual analog scale.

Endometriosis: new insights and opportunities for relief of symptoms.

Endometriosis is a chronic neuroinflammatory disorder believed to impact on the wellbeing of more than 190 million women and people assigned female at birth. The defining hallmark of endometriosis is the growth of endometrial-like tissue as "lesions" outside the uterus. Most lesions are found in the pelvis and referred to as peritoneal (superficial), ovarian (endometrioma) or deep depending on location. Patients often suffer from persistent pelvic pain which can be worse during menstruation as well as fatigue, gastro-intestinal and urinary symptoms and mood disorders that impact quality of lif

Endometriosis and ovulatory menstruation: beyond the Sampson principle.

Endometriosis is an estrogen-dependent chronic inflammatory syndrome characterized by viable endometrial tissue outside the uterine cavity and associated with pain and infertility. Endometriosis, as tissue or a pathological process, is dynamic in that its establishment and progression require repeated episodes of retrograde travel of shed endometrial tissue, which implants in the lower abdominal cavity following ovulatory cycles and survives. Estrogen-rich follicular fluid released onto peritoneal surfaces during ovulation may also support endometriotic implants. DNA evidence indicates that en

Perineal endometriosis on an episiotomy scar: diagnosis based on clinical, radiological, and hormonal criteria (case report).

Endometriosis is a chronic, non-cancerous gynecological disorder that is typically observed in women of fertile age. It describes the presence of functional endometrial components, glands and stroma outside the endometrial cavity, most often involving pelvic structures such as the ovaries, peritoneum, and uterine ligaments. Perineal endometriosis is a rare form of extrapelvic endometriosis, with an estimated incidence of between 0.3% and 1%. We report the case of a 36-year-old woman, gravida 1 para 1(G1P1), with a history of vaginal delivery and no known history of endometriosis, who presented

Towards comprehensive management of symptomatic endometriosis: beyond the dichotomy of medical versus surgical treatment.

Except when surgery is the only option because of organ damage, the presence of suspicious lesions, or the desire to conceive, women with endometriosis-associated pain often face a choice between medical and surgical treatment. In theory, the description of the potential benefits and potential harms of the two alternatives should be standardized, unbiased, and based on strong evidence, enabling the patient to make an informed decision. However, doctor's opinion, intellectual competing interests, local availability of specific services and (mis)information obtained from social media, and online
