Obturator Hernia: A Critical Appraisal of Treatment Strategies Based on 10 Cases and Current Literature: Towards a Pragmatic Surgical Algorithm.

Obturator hernia is a rare pelvic hernia that predominantly affects elderly, thin women. Despite advances in imaging and surgical techniques, the optimal surgical approach remains controversial. We retrospectively analyzed 10 patients who underwent emergency surgery for an obturator hernia at our institution between January 2010 and August 2024. All patients were female with a median age of 88.9 years (range: 84-97). Six patients (60%) required bowel resection. Seven underwent open repairs, and three underwent laparoscopic transabdominal preperitoneal (TAPP) repair. One patient with perforated

Current Status of PSMA-Targeted Agents Based on Small Molecules for Prostate Cancer in Preclinical Studies.

BACKGROUND: National conversation has justifiably been concerned with firearm-related deaths and much less attention has been paid to the consequences of surviving a firearm injury. We assessed the risk of hospital readmission, length of stay (LOS) during hospitalization, and costs within 90-days after surviving an index firearm injury and compared these data with pedestrians and occupants involved in motor vehicle crash (MVC). METHODS: Nationwide Readmission Database, a nationally representative readmission database from 2013 and 2014 was used to create a retrospective cohort study. The prim

Multifocal small bowel gastrointestinal stromal tumor causing perforation peritonitis: A rare cause of acute abdomen.

A female patient in late 40s presented with severe abdominal pain, distension and fever for a week. Ultrasound abdomen revealed ascites and a large indeterminate mass in the pelvis, but bowel loops were not dilated. Contrast-enhanced CT (CECT) abdomen revealed a large, exophytic, heterogeneously enhancing intraperitoneal mass, arising from a jejunal loop in the pelvis; showing fistulous communication with an adjacent small bowel loop. Multiple smaller enhancing rounded, serosal masses were also seen in the jejunum and ileum along with pneumoperitoneum, moderate ascites and peritoneal enhanceme

Early US Prenatal Diagnosis and Serial Imaging Confirmation of Hydrometrocolpos with Uterine Didelphys and Cloacal Malformation: A Rare Case of Combined Müllerian Duct and Anorectal Anomaly.

BACKGROUND: Hydrometrocolpos associated with uterine didelphys and cloacal malformation represents an exceptionally rare prenatal finding. These anomalies arise from complex developmental disruptions of the Müllerian ducts and anorectal structures. Early recognition through first-trimester ultrasound is critical to ensure accurate diagnosis, guide follow-up imaging, and support parental counseling and perinatal planning. CASE PRESENTATION: A female fetus was identified in the first trimester with a midline cystic pelvic mass posterior to the bladder. The finding was initially suspicious for h

Laparotomy After Total Pelvic Exenteration: Current Status and Techniques for Ileal Conduit Preservation.

BACKGROUND/AIM: Advances in chemotherapy have shifted the treatment paradigm for colorectal cancer from surgical intervention to medical treatment. We report the outcomes of laparotomy following total pelvic exenteration (TPE) and describe specific methods for preventing ileal conduit injury. PATIENTS AND METHODS: Eighteen patients underwent a total of 29 laparotomies after TPE. Laparotomies were categorized into three types "Recurrence surgery", "Complication surgery (early complications and late complications)", and "Palliative surgery". Ileal conduit preservations were categorized into Cat

Comparison of morbidity and mortality of abdominoperineal resection vs low anterior resection in rectal cancer.

Postoperative sepsis is a serious complication of rectal cancer surgery and contributes to increased morbidity and mortality. This study aimed to identify risk factors and etiologies associated with postoperative sepsis in patients undergoing rectal surgery. A retrospective cohort study was conducted at a single center, including patients with rectal cancer who underwent surgery between November 2018 and February 2023. Tumors located <5 cm from the anal verge were treated with abdominoperineal resection (APR). Recorded variables included age, sex, surgical approach, tumor location, comorbiditi

Diagnostic Pitfalls in Obturator Hernia: A Case Report and Literature Review.

Obturator hernia is a rare clinical entity that predominantly affects elderly, thin women due to anatomical factors such as a wider pelvis and the loss of preperitoneal fat. Despite its classic description, the preoperative diagnosis remains a major challenge due to its deep location and non-specific presentation as a mechanical small bowel obstruction. This report describes the case of a 66-year-old female whose history of prior abdominal surgeries created a diagnostic bias toward adhesive obstruction, resulting in an initial period of conservative management. The most significant clinical si

Jejunal Diverticulitis With Contained Perforation and Abscess Successfully Managed With Conservative Therapy: A Case Report and Review of the Literature.

Jejunal diverticulosis is a rare condition that predominantly affects elderly individuals and is often asymptomatic. However, complications such as diverticulitis, perforation, and abscess formation can result in significant morbidity. Perforated jejunal diverticulitis has traditionally been managed surgically, although increasing evidence supports conservative treatment in selected patients. We report a case of a 74-year-old female with hypothyroidism, hyperlipidemia, and Alzheimer's dementia who presented with acute bilateral lower abdominal pain and nausea. Laboratory evaluation demonstrate

Uterine Leiomyosarcoma Incidentally Diagnosed After Sigmoid Colon Perforation: A Case Report and Review of the Literature Highlighting Individualized Surgical and Oncologic Decision-Making.

Introduction: Uterine leiomyosarcoma (uLMS) is a rare, highly aggressive malignancy arising from the smooth muscle tissue of the uterine wall. It typically presents with abnormal vaginal bleeding, pelvic pain, or a pelvic mass. In rare instances, symptoms may result from local invasion or metastasis. We report a case of uLMS initially diagnosed following colonic perforation due to direct tumor invasion, accompanied by a comprehensive narrative review of the literature on the incidental identification of uterine sarcomas during emergency general surgery to further emphasize the diagnostic chall

Global and Regional Impact of Pelvic Inflammatory Disease in Females Across All Age Groups (1990-2021) with Projections to 2046: Findings from the 2021 Global Burden of Disease Study.

BACKGROUND: Pelvic inflammatory disease (PID) is a common condition among women of childbearing age, referring to a group of infectious diseases of the upper female reproductive tract, tubo-ovarian abscess, and pelvic peritonitis, primarily caused by sexually transmitted infections (STIs). Delays in diagnosis and treatment can lead to sequelae associated with PID reproductive tract infections such as tubal factor infertility, ectopic pregnancy (EP), and chronic pelvic pain. This study assessed the global burden, trends, and cross-national disparities in PID among women, with projections extend

Cryptic clues from an acute abdomen: a case report of peritonitis turning into disseminated cryptococcosis.

BACKGROUND: Cryptococcosis is a life-threatening opportunistic fungal infection that primarily affects immunocompromised individuals, most commonly presenting as meningitis. Peritoneal involvement is exceedingly rare and can closely mimic common acute surgical conditions, often resulting in delayed diagnosis and treatment. This case describes an unusual presentation of disseminated cryptococcosis as acute peritonitis with subsequent progression to cryptococcal meningitis; underscoring the diagnostic challenges posed by atypical intra-abdominal infection. Infectious mononucleosis mimicking Fitz-Hugh-Curtis Syndrome: The diagnostic complexity of concurrent Chlamydia trachomatis infection and false-positive syphilis serology.

Infectious mononucleosis (IM), typically caused by the Epstein-Barr virus, presents with a wide range of clinical manifestations. This diversity includes rare organ complications and the potential for false-positive syphilis serology results, making the diagnosis challenging. We report a rare and instructive case of IM presenting as acute acalculous cholecystitis (AAC). This case was complicated by concurrent false-positive syphilis serology results, prompting strong clinical suspicion for Fitz-Hugh-Curtis syndrome (FHCS). A 21-year-old woman presented with fever, right upper quadrant pain, an

Peritoneal Tuberculosis Complicated by Abdominal Abscesses: Diagnostic Challenges and Clinical Insights From Two Cases.

Peritoneal tuberculosis (PT) is a rare form of extrapulmonary Mycobacterium tuberculosis infection that may closely mimic Crohn's disease (CD) or intra-abdominal malignancy, leading to diagnostic delay, particularly in endemic areas. We report two illustrative cases complicated by localized abscess formation. The first case involved a 40-year-old woman presenting with chronic right lumbar pain. Computed tomography (CT) revealed a large right psoas abscess associated with circumferential thickening of the cecal base and terminal ileum, along with necrotic mesenteric and iliac lymphadenopathy. Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Midgut Volvulus with Necrosis: A Complication of Endometriosis.

BACKGROUND: Deeply infiltrating endometriosis (DIE) can involve the bowel leading to necrosis with septic shock, a rare complication occurring in 5% of endometriosis patients. This case report aims to illustrate a life-threatening complication of endometriosis, emphasizing the need for strong clinical acumen in atypical cases of endometriosis. This also highlights the need for improved noninvasive diagnostic modalities to enable earlier detection. Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Fusobacterium necrophorum associated with pelvic infections: A case series and literature review.

Fusobacterium necrophorum is a gram-negative anaerobic bacillus most commonly implicated in oropharyngeal infections and their subsequent complications, including septic thrombophlebitis of the internal jugular vein, or Lemierre syndrome. However, limited case reports document evidence of pelvic infections associated with Fusobacterium as well, such as pelvic peritonitis and pelvic septic vein thrombophlebitis. In this case series, we discuss two occurrences of Fusobacterium necrophorum bacteremia from a presumed gynecologic source and present a literature review of other reported cases of Fus

A neonate with meconium peritonitis, first-trimester hepatitis A affected pregnancy: A case report.

Congenital intra-abdominal masses can stem from various etiologies. However, the differential diagnoses in the presence of meconium peritonitis are primarily narrow to gastrointestinal disorders such as cystic fibrosis, Hirschsprung's disease, and colonic atresia. Additionally, literature has documented rare associations, including maternal hepatitis A infection. We present a case of a premature female infant born via cesarean section to a 25-year-old mother with adequate prenatal care. The mother had a history of flu-like symptoms and diarrhea during the first trimester and routine third-trim
