Integrating Behavioral Interventions into the Routine Immunization System in Chad: Implementation Lessons from a Fragile Setting.

Despite global progress, COVID-19 pandemic disruptions increased immunization inequities, particularly in fragile contexts like Chad. In 2022-2023, Chad reported 124,544 zero-dose children, in addition to 122,935 children who were classified as under-vaccinated. Low immunization uptake reflects persistent barriers related to access, knowledge, misinformation, and hesitancy. Behavioral interventions, adapted to low-resource settings, can help address these challenges. Anchored in the WHO Behavioral and Social Drivers (BeSD) framework and aligned with the Immunization Agenda 2030 (IA2030), we de

Comparative serum cytokine and chemokine profiles across tuberculosis, latent TB, HIV, and COVID-19 infections in Namibian adults.

BACKGROUND: The devastating pandemic of coronavirus disease 2019 (COVID-19) highlighted the importance of early detection and global collaboration in combating infectious diseases. Namibia is one of the countries that bears a high burden of tuberculosis (TB) and human immunodeficiency virus (HIV) infection. The overlap of these diseases underscores the need to better understand host immune responses, which play a critical role in disease pathogenesis and may differ across infections. This study aimed to profile selected cytokines and chemokines among Namibian individuals with TB, latent TB inf

Breastfeeding, Breastmilk Feeding and Formula Feeding Within the Closed, Temporary Accommodation of Australia's COVID-19 Quarantine.

During 2021-2022, families with infants and young children were among the > 450,000 people who spent 2 weeks in Australia's institutional COVID-19 quarantine. Given pre-existing lack of emergency planning for children, there were concerns about whether parents would be adequately supported in this temporary accommodation. This paper reports on research considering the breastfeeding, breastmilk feeding and formula feeding of infants and young children in Australia's COVID-19 quarantine. Two-hundred and sixteen parents who quarantined with children aged 0-36 months completed an online survey wit

Telehealth-related education and training: a systematic review of recent developments since the COVID-19 pandemic.

The pandemic showed the potential of existing telehealth to increase healthcare access but also the need for healthcare workers to be competent and skilled in delivering care through this modality. We undertook a systematic literature review to identify telehealth-related courses developed since the start of the COVID-19 pandemic and summarised study characteristics, learning objectives, and course content. Articles describing telehealth-related training and education programs implemented between 2020 and 2024 were included. Papers were required to list specific program content or learning obj

Safety and immunogenicity of a first-generation modified vaccinia virus Ankara-vectored SARS-CoV-2 vaccine candidate with mRNA-based booster vaccination: A phase 1 trial.

The rapid development of effective COVID-19 vaccines was enabled by the parallel assessment of multiple candidates early in the pandemic, including candidates based on the Modified Vaccinia virus Ankara (MVA) vector platform. This Phase 1 trial (NCT04569383) evaluated the MVA-based vaccine candidate MVA-SARS-2-S, expressing the full-length native SARS-CoV-2 spike glycoprotein (Wuhan-HU-1). Between October 2020 and May 2021, 30 healthy, SARS-CoV-2-naive adults received two doses of either 107 or 108 international units (IU) of MVA-SARS-2-S 28 days apart, with 6-month follow-up. From the operating room to the cloud: the evolving continuum from live surgical broadcasting to remote tele-surgery.

In the last four decades, the transmission of a live surgical operation to a remote audience has evolved from closed-circuit educational broadcast into a fully remote, robot-assisted surgery performed across different continents. This review maps that journey systematically, defining a six-rung Digital Surgical Spectrum - from passive live broadcast to artificial-intelligence-augmented autonomous surgery - grounding each stage in published clinical evidence across multiple surgical domains. We demonstrate that each rung of this spectrum is no longer theoretical. Live broadcasts have reached au

An efficient and versatile reverse genetics system for bovine coronavirus facilitates antiviral evaluation.

Bovine coronavirus (BCoV), a primary etiological agent of calf diarrhea, exhibits dual tropism for the respiratory and enteric tracts. However, research on BCoV gene function and pathogenesis has been limited by the lack of convenient and flexible reverse genetics tools. Herein, we report a robust reverse genetics system for BCoV based on an optimized circular polymerase extension reaction (CPER) methodology. We successfully rescued recombinant BCoV (rBCoV) in HRT-18G cells and engineered recombinant BCoV expressing ZsGreen fluorescent protein (rBCoV/Zs-T2A-ns2). Phenotypic analysis demonstrat

Pulmonary Herpes Simplex Virus Reactivation in Invasively Mechanically Ventilated Patients With COVID-19 in Intensive Care Unit.

Patients in intensive care units (ICU) under mechanical ventilation, particularly those with SARS-CoV-2 infection, are highly susceptible to Herpes simplex virus (HSV) reactivation. Given the lack of data on the SARS-CoV-2 Omicron and Delta variants, this study aimed to assess the prevalence of pulmonary HSV reactivation in mechanically ventilated patients with SARS-CoV-2 pneumonia. The impact of HSV replication on SARS-CoV-2 replication, and conversely, was investigated, along with the susceptibility of HSV strains to antiviral therapies. This retrospective study included 381 mechanically ven

The human gut microbiome primes fever after vaccination.

Fever is a common adverse reaction to vaccination, contributing to vaccine hesitancy and reduced uptake. To understand variation in fever risk, we longitudinally profiled fecal microbiota, oral temperature, and serological markers in 171 healthy adults receiving Severe acute respiratory syndrome coronavirus 2 mRNA vaccines. Fever risk correlated with low-grade intestinal inflammation, increased abundance of flagellated Lachnospiraceae bacteria, and increased flagellin expression prevaccine. Microbiomes from fever-high donors triggered stronger inflammation in human intestinal organoids and dro

Quantification of 20-Hydroxyecdysone From Drosophila Larvae at the Onset of Metamorphosis.

Metamorphosis in class Insecta is defined as the drastic transition of an organism from a larval stage to a morphologically very different adult form. Insects such as Drosophila melanogaster undergo metamorphosis, which requires precise temporal control, exerted by ecdysteroids, the primary regulators of these developmental transitions. The active form of ecdysone is 20-hydroxyecdysone (20E), which acts in pulses that trigger the onset of different developmental stages. A massive pulse of 20E occurs between the prepupal and pupal stages, triggering pupariation. Precise quantification of 20E le

Exogenous IFN-α delivered as an intranasal adjunct to systemic vaccination directs vaccine-specific T-cell responses to the upper respiratory tract.

Respiratory infectious diseases remain a major global health burden, sustained by ongoing community transmission. Licensed respiratory vaccines are predominantly systemically administered, which elicit vaccine-induced antigen-specific circulating immunity, but fail to protect the primary site of initial infection and onward transmission, the upper respiratory tract (URT). The URT is a highly compartmentalised anatomical site that is not directly accessed by systemic vaccination. Consequently, conventional systemic respiratory vaccines do not reliably prevent acquisition of infection or its onw

Impact of and recovery after Covid-19 on breast cancer diagnosis and care in Johannesburg, South Africa.

BACKGROUND: COVID-19 negatively impacted cancer diagnostic and treatment services worldwide. These challenges were further compounded in low- and middle-income countries, where longstanding resource constraints limited the capacity to maintain standard treatment pathways. We aimed to assess the impact of COVID-19 on (i) stage at diagnosis and time intervals from symptom recognition to a pathological confirmed breast cancer (BC) diagnosis and (ii) treatment initiation at two academic hospitals in Johannesburg; Chris Hani Baragwaneth Academic Hospital (Site 1) and Charlotte Maxeke Johannesburg A

Hypoxia Inducible Factor 1α-dependent Induction of Alveolar miR-147b Dampens SARS-CoV-2-associated Acute Respiratory Distress Syndrome via Direct Targeting of Viral RNA.

BACKGROUND: Acute respiratory distress syndrome (ARDS) causes significant morbidity and mortality during viral pneumonia, including SARS-CoV-2 infections. Nevertheless, most patients with SARS-CoV-2 infections recover seamlessly without developing ARDS, suggesting the existence of endogenous pathways to protect the lungs. Since microRNAs (miRNAs) can regulate endogenous molecular pathways involved in lung protection, the authors hypothesized that alveolar miRNAs could function to dampen SARS-CoV-2-associated lung injury. METHODS: Screening studies in human alveolar epithelial cells and SARS-C

Post-Infectious Syndromes in the Post-Pandemic Phase Among the German General Population.

BACKGROUND: Persistent symptoms after a respiratory infection can impair daily life, as has been shown, for example, for "long COVID." In this study, we determined the frequency and impact of symptoms that persisted for 12 weeks or more after a respiratory infection. A further endpoint was health care utilization for such symptoms in the general population. METHODS: A cross-sectional study was conducted within the population-based digital cohort DigiHero in Germany (DRKS00025600). 39.3% of invited participants responded by completing an online questionnaire. 46 915 adults were included in the

Computational identification of B- and T-cell epitopes: a unified task taxonomy and review of databases, datasets, predictive pipelines, and gaps.

The immune system leverages B and T cells to recognize specific molecular patterns, known as epitopes, on pathogens and cancer cells to effectively combat infections and diseases. The critical success of immunotherapies in cancer treatment and COVID-19 vaccine development has established precise epitope identification as a central and rapidly growing priority in therapeutic design. Because traditional wet-lab based identification of B- and T-cell epitopes is expensive and timeconsuming, our systematic literature search (2015-2026) identified 148 Artificial Intelligence (AI) based linear B-cell
