BACKGROUND: Tuberculosis of the endometrium is usually associated with infertility. Once a patient is diagnosed with endometrial tuberculosis, she is almost certain to experience infertility; in vitro fertilization and embryo transfer offer the only realistic treatment for tuberculous infertility. The tumor marker CA 125 was initially thought to be specific for ovarian malignancies. Subsequently it was found to be raised in a variety of benign conditions, including pregnancy, pelvic inflammatory disease, tuberculosis and cirrhosis of the liver. With respect to gynecological tumors, CA 125 may be elevated in benign ovarian cysts, tubo-ovarian abscess, endometriosis, hyperstimulation syndrome, ectopic pregnancy and fibroids. These results demonstrate that CA 125 is a marker of non-specific peritoneal conditions.

We report on a case of dual infection with Cryptococcus neoformans and Streptococcus pneumoniae in the cerebrospinal fluid of a patient with AIDS. To our knowledge this represents the first case to be reported in the English literature. In immunocompromised patients the importance of performing both the Gram- and India-ink stain and cryptococcal antigen test on CSF specimens with completely normal biochemical and cellular parameters is emphasized.

There are several therapeutic procedures for stenosis of the ureter. Ileal plasty is indicated in case of extended stenosis or those located in the pelvis when the poor quality of the bladder prevents use of a bladder flap. Thirteen women were discovered to be positive for human immunodeficiency virus (HIV) infection during pregnancy at the Port Moresby General Hospital from 1988 to 1995; of these, eight were diagnosed in the first half of 1995. OBJECTIVE: To analyse the diagnostic value of laparoscopy in patients with tubal infertility.
METHODS: The morphological appearance of fallopian tubes and surrounding tissues of 1120 cases with proven tubal infertility were observed under laparoscopy. Hydrotubation was performed at the same time in each case.

Exicised tubal segments (including interstial and isthmic portions) from 33 women with proximal tubal occlusive infertility were examined pathologically. OBJECTIVE: To perform an abdominopelvic or pelvic mass biopsy as an office procedure, as an alternative to laparoscopic biopsy.
METHODS: A transvaginal trucut biopsy using a disposable biopsy needle was carried out in 5 cases.
RESULTS: The biopsy gave a clear diagnosis of ovarian cancer in 2 cases, non-tubercular inflammation in 2 cases and tuberculosis in 1 case.
CONCLUSIONS: In the developing world, transvaginal trucut biopsy is an important method for diagnosis.

Combined histological and bacteriological investigations of 800 specimens of nonpregnant endometrial curettings of 15 to 60 years age group of hill women of Darjeeling District were carried out for detection of tuberculous endometritis. A bacteriological study of 114 infertile patients in Northern Nigeria revealed a prevalence rate for genital tuberculosis of 16.7%. The low incidence of a past history of pulmonary tuberculosis may relate to a gastro-intestinal sources of the infective organisms, illiteracy and delay in seeking medical advice.
