Intraoperative discovery of bladder herniation in a known inguinoscrotal hernia during open cystolithotomy, prostatectomy and hernioraphy : a case report and review of diagnostic pitfalls.

BACKGROUND: Inguinoscrotal herniation of the urinary bladder (ISHB) is a rare but underdiagnosed condition, accounting for 1-4% of inguinal hernias and discovered intraoperatively in more than 80% of cases. Recognised predisposing factors include advanced age, obesity, and chronic infravesical obstruction, of which benign prostatic hyperplasia (BPH) is the most common. Failure to identify ISHB preoperatively exposes the surgeon to the risk of inadvertent vesical injury, reported in up to 12% of unprepared herniorrhaphies. CASE PRESENTATION: A 61-year-old man was admitted for obstructive lower

Test order, timing, and multidisciplinary review predict management change in sequential fluoroscopic and MR defecography.

PURPOSE: To determine whether sequential fluoroscopic defecography (FLD) and MR defecography (MRD) change clinical management of patients with pelvic floor dysfunction and to identify factors associated with management change. METHODS: Multi-institutional retrospective study including adults who underwent both FL and MR defecography within 3 years, without interval surgery. Primary outcome was management change attributed to the second examination. Clinical information, test order & timing, multidisciplinary review, and imaging variables were compared between patients with and without managem

Evaluation of anatomical changes dynamically by magnetic resonance defecography according to pelvic floor dysfunction types.

BACKGROUND: Pelvic floor dysfunction, resulting from the weakening of pelvic support structures, leads to the loss of support and prolapse of intrapelvic organs. Magnetic resonance defecography (MRD) is effective in demonstrating changes during increased intra-pelvic pressure, such as defecation. In our study, we aimed to investigate changes occurring during defecation in different types of pelvic floor dysfunction. MATERIALS AND METHODS: Magnetic resonance defecography images of 206 female patients with and without pelvic floor dysfunction were retrospectively evaluated and categorized into

Improving Diagnostic Accuracy for Surgical Pelvic Organ Prolapse: A Sequential Protocol Combining POP-Q Examination and Transperineal Ultrasound.

Background/Objectives: The POP-Q system is conventionally used to evaluate pelvic organ prolapse (POP). Nevertheless, differences between clinical examination and intraoperative findings can hinder appropriate surgical planning. We aimed to assess the accuracy of a sequential protocol involving clinical POP-Q assessment and, in cases of uncertain diagnosis, transperineal ultrasound. Methods: We conducted a prospective observational study with 314 women scheduled for POP surgery from January 2021 to December 2024. A pelvic floor specialist assessed all patients using the POP-Q system. Transperi

Comparable Performance of ORIHIME Mesh Versus Gynemesh PS in Laparoscopic Sacrocolpopexy: A Retrospective Comparative Study.

INTRODUCTION: In Japan, laparoscopic sacrocolpopexy (LSC) is performed using either the commonly used polypropylene (PP) mesh or the domestically produced polytetrafluoroethylene (PTFE) mesh, ORIHIME. Despite its clinical adoption, the comparable performance of this material relative to conventional options such as PP mesh has not been sufficiently investigated. This study aimed to compare clinical outcomes of LSC using PTFE mesh (ORIHIME) versus PP mesh (Gynemesh PS) through a retrospective study. METHODS: A total of 211 patients who underwent LSC at our hospital and for whom follow-up was p

Tele-Robotic Pectopexy Across Cities Using an Indigenous Robotic Platform: A Case Report.

Tele-robotic surgery enables surgeons to operate from distant locations using robotic platforms and secure communication networks, helping reduce geographic barriers to specialized surgical care. Its application in gynaecologic pelvic reconstructive surgery is still limited, particularly in low- and middle-income countries. We present a case demonstrating the feasibility of long-distance tele-robotic pectopexy in India. A 57-year-old woman with third-degree uterovaginal prolapse, third-degree cystocele, and first-degree rectocele underwent tele-robotic pectopexy at Her Health Hospital, Bhopal,

Salvage pectopexy using detached lateral suspension mesh arms.

BACKGROUND: Laparoscopic lateral suspension is an alternative to sacrocolpopexy when access to the sacral promontory is restricted or unsafe. However, prolapse recurrence may occur due to mesh arm detachment or progressive fascial attenuation. Pectineal fixation is an alternative approach that may provide a stronger, more stable anchoring vector and improved force distribution. OBJECTIVES: To describe a minimally invasive salvage surgical strategy for managing recurrent anterior compartment prolapse in a patient with a history of laparoscopic lateral suspension and inaccessible sacral promont

[A Case of Pelvic Organ Prolapse Treated with Laparoscopic Sacrocolpopexy and Bladder Diverticulectomy at the Same Time].

We report a case of bladder diverticulum accompanied by pelvic organ prolapse treated with laparoscopic sacrocolpopexy and bladder diverticulectomy at the same time. A literature search showed this is the first report of simultaneous laparoscopic bladder diverticulectomy and sacrocolpopexy. A 63-year-old woman was referred to our department for voiding dysfunction and incomplete bladder emptying. She had a POP-Q stage III cystocele. Magnetic resonance imaging showed a large bladder diverticulum. Laparoscopic sacrocolpopexy and bladder diverticulectomy were performed at the same time. After the

Spina Bifida Occulta, Syringomyelia, and Diastematomyelia in a Toddler: A Two-Year Case Report.

Spinal dysraphisms are a heterogeneous group of congenital spinal cord anomalies resulting from defective neurulation. They include both open and closed forms, with spina bifida (SB) being the most clinically recognized. These conditions frequently coexist with other anomalies, such as syringomyelia and diastematomyelia, significantly complicating clinical presentation and management. We report the case of a male neonate born at term with a lumbar spinal defect consistent with SB occulta. Initial evaluation revealed a closed, skin-colored lesion with preserved neurological function. Early spin

Vaginal hysterectomy in patients with pelvic organ prolapse: An eight-year experience from a tertiary care center.

OBJECTIVE: Pelvic organ prolapse (POP) impairs quality of life in aging women. Vaginal hysterectomy (VH) is a standard approach for advanced POP, yet large single-center series are limited. We evaluated concomitant procedures and perioperative outcomes of VH and explored associations of age, parity, and concomitant adnexal or reconstructive surgery with perioperative measures. STUDY DESIGN: This retrospective cohort included 416 patients who underwent VH for POP between 2017 and 2025. Demographics, prolapse stage, concomitant procedures, perioperative complications, length of hospital stay, p

A Dual-Task Deep-Learning Model with Fused Ultrasound Images for Simultaneous Typing and Grading of Cystocele.

INTRODUCTION AND HYPOTHESIS: We developed a dual-task deep-learning model, termed FD-Net, which utilizes fused two-dimensional (2D) and three-dimensional (3D) ultrasound images to simultaneously automate cystocele typing and grading, and evaluated its diagnostic performance. METHODS: We retrospectively included 625 patients (467 cystocele, 158 normal). The model fused preprocessed two-dimensional (2D, resting and Valsalva) and three-dimensional (3D, levator hiatus) images as input. On the basis of a ResNet50 backbone, FD-Net performed both typing (normal, type I/II/III) and grading (normal, m
