Surgical treatment of apical prolapse Number 9 - 2026.

Apical genital prolapse represents a failure of DeLancey's level I support. Isolated vaginal hysterectomy does not correct apical prolapse. All surgery for uterine or vaginal vault prolapse should include apical suspension. Sacrocolpopexy is the most durable technique for correcting vaginal vault prolapse, with the exception of colpocleisis. Vaginal techniques remain valid options in selected patients. Uterine preservation, when based on shared decision-making, is possible and safe for carefully evaluated patients. Recognition of the apical component in advanced cystocele is essential to preve

Beyond POP-Q: A Scoping Review of Pelvic Floor Ultrasound for Anatomical Phenotyping of Anterior Compartment Prolapse.

Background/Objective: Anterior compartment prolapse is commonly described as cystocele or anterior vaginal wall descent, although these terms do not identify the underlying mechanism of support failure. Similar POP-Q findings may result from different anatomical defects, including central cystocele, paravaginal support loss, apical descent, levator ani injury, hiatal ballooning, or combined abnormalities. Pelvic floor ultrasound may improve anatomical characterization, but its role in defect-specific phenotyping remains unclear. Our objective was to map the current evidence on the use of pelvi

An AI-assisted Clinical Decision Support System for Green Classification of Cystocele on Dynamic Transperineal Ultrasound.

Green classification of cystocele on dynamic transperineal ultrasound (TPUS) remains operator-dependent because it requires manual frame selection and landmark-based assessment of the Valsalva maneuver. We developed a workflow-oriented AI-assisted clinical decision support system for automated urethrovesical junction localization and dynamic Green classification and prospectively evaluated its standalone and reader-support performance. This diagnostic accuracy and reader study included 881 patients from a tertiary referral hospital, comprising a retrospective development cohort (n = 688) and a

Congenital Agenesis of the Obturator Foramen in an Asian Population and Its Clinical Significance.

BACKGROUND: The obturator foramen, an ovoid opening in the hip bone formed by the ischium and pubis, allows passage of the obturator artery, vein, and nerve through the obturator canal. Mostly covered by the obturator membrane, its congenital agenesis or partial underdevelopment is extremely rare and scarcely documented, yet it carries significant implications for pelvic surgery, orthopedic, vascular procedures, and radiological interpretation. OBJECTIVE: This study aimed to determine the prevalence, morphological characteristics, and clinical relevance of congenital agenesis of the obturator

Clinical and Urodynamic Predictors of Urinary Urgency Improvement After Cystocele Repair.

PURPOSE: Overactive bladder (OAB) symptoms may improve following surgical correction of cystocele. This study aimed to evaluate changes in urgency, the primary symptom of OAB, and identify factors associated with improvement in urgency after surgical treatment for cystocele. METHODS: We conducted a retrospective analysis of medical records for patients who underwent surgical treatment for cystocele and had preoperative urgency measuring ≥3 on a 5-point urinary sensation scale. Patients were categorized into 2 groups based on their urgency status 3 months postsurgery: those with improved urgen

Is occult stress urinary incontinence truly "occult"?

Stress urinary incontinence (SUI) and pelvic organ prolapse (POP) frequently coexist due to shared pathophysiological mechanisms. In some cases, SUI becomes evident only after the reduction of prolapse, a condition referred to as occult stress urinary incontinence (OSUI). Conventional understanding attributes OSUI to urethral obstruction caused by mechanical kinking or compression from the descending bladder base. This perception has led to ongoing controversy regarding the necessity of concomitant anti-incontinence procedures during POP repair. However, is this conventional interpretation acc

The risk factors of anatomical cystocele recurrence: the hiatal area and the lowest vaginal mesh location observed by pelvic floor ultrasound.

OBJECTIVES: To explore the risk factors for the anatomical cystocele recurrence (ACR) after transvaginal mesh (TVM) implantation. METHODS: Patients with a TVM between 2015 and 2023 were enrolled. ACR is determined as the bladder descended ≥ 10 mm below the pubic symphysis or a cystocele stage II or higher. The lowest mesh position (LMP) determined by ultrasound corresponded to the caudal mesh end relative to the bladder neck at maximum descent. Positions below the bladder neck were noted as negative values. The hiatal area (HA) was measured using four-dimensional pelvic floor ultrasound. The

[A CASE OF PYOGENIC SPONDYLITIS CAUSED BY MESH INFECTION AFTER LAPAROSCOPIC SACROCOLPOPEXY].

We report a case involving an 81-year-old woman who developed pyogenic spondylitis following laparoscopic sacrocolpopexy (LSC) for a cystocele in November year X. During concomitant uterine resection, purulent discharge was observed, and oral levofloxacin was administered for 5 days postoperatively. Pathological examination revealed xanthogranulomatous endometritis, which was considered the source of the mesh infection. The patient was discharged on postoperative day 7 without complications. In early January year X+1, she developed buttock pain that persisted for approximately 2 weeks without

A novel description of dynamic organ-specific trajectories in pelvic organ prolapse based on magnetic resonance imaging traces.

BACKGROUND: The dynamic motion trajectories of pelvic organs during prolapse remain incompletely characterized. Detailing these trajectories could enhance the understanding and treatment of pelvic organ prolapse (POP). This study aimed to generate and visualize generalized dynamic trajectories for the bladder, uterus, and rectum. METHODS: This observational study enrolled 63 symptomatic patients with Stage III or worse POP (57 with cystocele, 35 with uterine prolapse, and 29 with rectocele). Patients underwent dynamic pelvic MRI scans during three consecutive maximum Valsalva maneuvers. Pelvi

[Deep learning models based on fused ultrasound images for the assessment of cystocele in women].

OBJECTIVES: Cystocele is a common type of pelvic organ prolapse in women. Ultrasound is the preferred imaging modality for the diagnosis of cystocele; however, the examination procedure is relatively complicated and dependent on the operator's experience, resulting in subjective interobserver differences in diagnosis. This study aims to develop deep learning models based on two-dimensional (2D) ultrasound images, three-dimensional (3D) ultrasound images, and fused 2D and 3D ultrasound images for the diagnosis and ultrasonographic grading of cystocele in women, and to compare the diagnostic per

Finite element analysis-based evaluation of different pessaries for female cystocele intervention.

Based on finite element analysis (FEA), this study compared the effects of vaginal pessaries of different shapes and sizes on pelvic floor biomechanics in a patient with cystocele, providing a theoretical basis for clinical selection of pessaries. Pelvic floor finite element models were constructed using the patient's magnetic resonance images (MRI) data to simulate the conditions of wearing ring and Gellhorn pessaries, and the biomechanical parameters of pelvic organs were analyzed. After pessary insertion, bladder neck displacement (BND), uterine neck displacement (UND), and anterior vaginal

Small bowel obstruction induced by migrated sacropexy mesh: a case report.

INTRODUCTION: Small bowel obstruction (SBO) is a frequent cause of surgical emergency, most commonly due to adhesions, hernias, or tumors. Mesh-related SBO following laparoscopic sacropexy is an exceptionally rare complication, often presenting years after the initial procedure and difficult to diagnose preoperatively. CASE PRESENTATION: We report a 69-year-old Caucasian woman with a history of laparoscopic sacropexy for cystocele repair using mesh eight years prior, presenting with abdominal pain, bilious vomiting, and distension. Laboratory tests revealed leukocytosis and elevated inflammat

Post-Hysterectomy Vaginal Vault Prolapse: Current Perspectives and Decision-Making Considerations for Surgical Treatment.

Background and Objectives: Following the removal of the uterus, due to prolapse or other indications, disruption of the apical support structures may result in descent of the vaginal apex. As the condition progresses, the cystocele, rectocele, and/or enterocele are formed, finally creating a complex vaginal eversion that generally requires surgical multi-compartmental repair. This review aimed to provide a comprehensive overview of cur-rent surgical modalities for post-hysterectomy vaginal vault prolapse, which could sup-port personalized treatment.
