[Vaginal pelvic floor reconstruction techniques].

Pelvic organ prolapse (POP) is a common condition affecting women. It results from weakening of the pelvic floor muscles and connective tissues and is particularly common among postmenopausal women. This weakening can lead to descent of the pelvic organs, including the bladder, uterus, and rectum, into or beyond the vaginal canal, causing significant discomfort and impairment of quality of life. Vaginal prolapse surgery is a key therapeutic approach for severe cases of POP, aiming to restore pelvic anatomy and improve functional outcomes. Surgical interventions for vaginal prolapse can be broa

The autologous vaginal patch plastron associated with the anterior sacrospinous ligament fixation for the treatment of anterior and apical pelvic organ prolapse: Technical note with video demonstration.

Following transvaginal mesh interdiction, native tissue repair (NTR) emerged as the standard surgical approach for anterior pelvic organ prolapse (POP) requiring vaginal intervention, despite high recurrence rates with traditional methods. This study presents a novel NTR technique combining vaginal patch plastron (VPP) with anterior sacrospinous ligament fixation (SSLF-A) to address anterior and apical POP. Incorporating apical suspension during anterior repair reduces recurrence compared to non-apical procedures. The VPP technique involves creating a vaginal strip on the anterior colpocele, s

Study of the relationship between mediolateral episiotomy in vaginal delivery and pelvic organ prolapse in pregnant mothers.

Pelvic Floor Disorders (PFDs) are a group of disorders of the female reproductive system that can cause a variety of problems for women. PFDs can include Urinary Incontinence (UI) and Pelvic Organ Prolapse (POP). This study evaluated the association between mediolateral episiotomy during pregnancy and POP in patients. A cross-sectional study with prospective follow-up was conducted on 150 pregnant women admitted for vaginal delivery at Furqani Hospital in Qom, Iran. Participants were divided into episiotomy and non-episiotomy groups based on clinical indications. Pelvic organ prolapse was asse

Automated Evaluation of Female Pelvic Organ Descent on Transperineal Ultrasound: Model Development and Validation.

INTRODUCTION AND HYPOTHESIS: Transperineal ultrasound (TPUS) is a widely used tool for evaluating female pelvic organ prolapse (POP), but its accurate interpretation relies on experience, causing diagnostic variability. This study aims to develop and validate a multi-task deep learning model to automate POP assessment using TPUS images. METHODS: TPUS images from 1340 female patients (January-June 2023) were evaluated by two experienced physicians. The presence and severity of cystocele, uterine prolapse, rectocele, and excessive mobility of perineal body (EMoPB) were documented. After preproc

Colpocleisis for recurrent pelvic organ prolapse after tension-free vaginal mesh repair: a case report.

Tension-free vaginal mesh repair surgery (TVM) has decreased due to complications related to mesh use and the lack of superiority compared to native tissue repair (NTR). In japan it is still performed as a treatment for pelvic organ prolapse (POP). However, recurrent prolapse after TVM does occur, necessitating appropriate management. The patient was underwent TVM-A surgery at another hospital 12 years ago for cystocele. POP-Q stage II rectocele was observed, and the LeFort colpocleisis was performed. Although TVM surgery is effective via the vaginal approach, recurrent posterior vaginal wall

Predictive Factors of Pelvic Organ Prolapse and the Quality of Life Among Jordanian Women: A Case-Control Study.

BACKGROUND: Pelvic organ prolapse (POP) is a common gynecological problem. Several risk factors have been identified to contribute to the main pathophysiological mechanism of prolapse development. However, the POP data in the Eastern Mediterranean region, including Jordan, are limited, and the risk factors associated with POP were mainly obesity and high number of vaginal deliveries. Since Jordan has a high prevalence of obesity, parity, and type 2 diabetes, the present study aimed primarily to identify the modifiable and non-modifiable factors associated with POP in Jordanian women, and secon

A Case of Vesicovaginal Fistula After Transvaginal Mesh Surgery Cured by Placement of a Urethral Catheter With Low-Pressure Intermittent Suction.

INTRODUCTION: We report a case of vesicovaginal fistula (VVF) following transvaginal mesh (TVM) surgery that was successfully treated conservatively using a urethral catheter with low-pressure intermittent suction. CASE PRESENTATION: A 78-year-old woman underwent TVM surgery for cystocele. During the surgery, bladder perforation occurred, which was repaired transvaginally. After the urethral catheter removal, she was diagnosed with VVF. We reinserted a new urethral catheter and added low-pressure intermittent suction to the catheter, which gradually decreased the amount of incontinence, and t

Influence of Pubovisceral Avulsion on Perineal Body Descent and Prolapse Development.

INTRODUCTION AND HYPOTHESIS: Pubovisceral muscle (PVM) avulsion affects over a third of women post-vaginal childbirth and is a significant risk factor for pelvic floor dysfunction (PFD). Prior studies show that even a 5 mm posterior-inferior shift of the perineal body (PB) enlarges the genital hiatus, contributing to prolapse. Building on this, we investigated the relationship between PVM avulsion and PB movement, correlated prolapse metrics, and sacrum/coccygeal movement with the injury. Moreover, we used statistical shape modeling (SSM) to see shape variation among different groups. METHODS

Risk factors for postoperative urinary retention following pelvic organ prolapse surgery: a meta-analysis.

Postoperative urinary retention (POUR) is one of the most common problems following pelvic organ prolapse (POP) surgery, the risk factors associated with its occurrence have been reported inconsistently across studies. We aim to synthesize the risk factors of urinary retention after POP surgery using a meta-analysis approach. Databases of the PubMed, The Cochrane Library, EMBASE, Web of Science, CBM, CNKI, WanFang and VIP were systematically searched through December 31, 2024. A total of 2065 studies were identified, 19 of which were included in this meta-analysis and 14 risk factors were anal

Bladder prolapse (cystocele) among African, Asian, and Middle Eastern women: a clinical and socio-cultural perspective.

Bladder prolapse, also known as cystocoele, is the most common form of pelvic organ prolapse (POP), significantly affecting women's physical, sexual, and psychosocial health, particularly in low- and middle-income countries (LMICs). Despite its widespread prevalence and disabling consequences, bladder prolapse remains under-recognised and inadequately addressed in global health strategies. Prevalence estimates vary widely across regions, from 3%-64.6%, influenced by diagnostic methods and cultural reporting biases. Common risk factors include high parity, early childbirth, prolonged labour, po
