Comparison of the Long-term Treatment Outcome Between Pubovaginal and Transobturator Suburethral Sling for Stress Urinary Incontinence in Women.

PURPOSE: Stress urinary incontinence (SUI) affects women worldwide, and surgery remains important for those who do not respond to conservative management. METHODS: We retrospectively reviewed the medical records of 533 female patients with mixed urinary incontinence and predominant SUI treated at a medical center. Some patients may have had stage 3 or higher cystocele and underwent concomitant anterior colporrhaphy. Patients were divided into 4 groups: pubovaginal sling (PVS) alone, PVS with colporrhaphy, transobturator suburethral sling (TOT) alone, and TOT with colporrhaphy. The primary out

Short- and long-term outcomes of patients with anterior vaginal wall prolapse after abdominal paravaginal cystocele repair.

OBJECTIVE: Pelvic organ prolapse (POP) is a common condition for women, causing urinary dysfunction, discomfort due to the lump at the introitus, sexual dysfunction and difficulty with bowel emptying. The lifetime risk for surgical intervention is 12.6 % by age 80, with 19 % of women requiring a secondary operation due to recurrence. Abdominal paravaginal cystocele repair is an option of treatment for women with Baden-Walker Grade 4 or recurrent cystocele in our centre. Therefore, the short- and long-term outcomes were investigated to evaluate the efficacy of treatment. MATERIALS AND METHODS:

Impact of Cystocele Classification and Surgical Method on Treatment Outcome: A Defect-Oriented Surgical Treatment for Cystocele.

Background/Objectives: Cystocele remains a prevalent condition with high recurrence rates following conventional native tissue repair. While mesh-based techniques may reduce anatomical recurrence, they are associated with increased complications and regulatory limitations. Our study proposes a defect-oriented approach to cystocele repair to assess whether individualized surgical management based on defect type can improve outcomes, particularly recurrence rates. Methods: A single-center retrospective analysis of 317 women undergoing cystocele repair (2019-2020) was performed. Patients were cla

Laparoscopic Surgery in a Patient With Endometrial Cancer After Laparoscopic Sacrocolpopexy.

In patients undergoing laparoscopic sacrocolpopexy (LSC) for pelvic organ prolapse (POP) repairs, occult uterine endometrial cancer may be discovered later. However, its detection poses unique surgical and oncological challenges, particularly when a mesh is involved. This case report describes a laparoscopic pelvic reconstruction with trachelectomy and bilateral salpingo-oophorectomy for occult uterine endometrial cancer in resected specimens after LSC. A 51-year-old female with stage III uterine prolapse and cystocele presented at our center. She subsequently underwent LSC and a laparoscopic

Excision of urethral caruncle using a sealing device (LigaSure™): A case report.

We report a case of a 74-year-old postmenopausal woman with stage III cystocele and a non-circumferential urethral caruncle presenting with bulge, voiding difficulty, occasional bleeding, and meatal discomfort. She underwent anterior colporrhaphy followed by excision of the urethral caruncle using the LigaSure™ device. The procedure was completed within 1 min, with minimal blood loss and no suturing. Postoperative follow-ups showed good wound healing without recurrence or urethral stricture. This case suggests that LigaSure™ may enable rapid, sutureless excision in selected non-circumferential

A rare case of female urinary retention caused by urethral leiomyoma: A case report.

INTRODUCTION AND IMPORTANCE: Female urethral leiomyoma is a rare benign tumor that originates from the smooth muscle cells in the urethra's wall. Surgical resection is often the primary treatment option. However, the tumor's location and size can present challenges for complete removal while preserving urethral function. Here, we report a case of female urethral leiomyoma causing chronic urinary retention and bladder diverticulum and provide a review of the existing literature on this rare condition. PRESENTATION OF CASE: Following a hysterectomy, a 49-year-old Asian woman was referred to our

Quantitative Cystocele Assessment in Clinical Pelvic Floor Ultrasound Diagnosis.

PURPOSE: Cystocele is a pelvic floor dysfunction disease to which women are prone after childbirth. The accuracy of pelvic floor ultrasound as the most commonly used examination method is influenced by subjective factors such as doctor experience and fatigue level, making it challenging to achieve high accuracy, consistency, and repeatability of diagnosis. This study aims to propose a high-precision and fully automatic cystocele evaluation method based on pelvic floor ultrasound video images. MATERIALS AND METHODS: This study retrospectively collected pelvic floor ultrasound images of 158 fem

Application of Transperineal Ultrasound in Assessing the Impact of the Degree of Uterine Prolapse on the Anterior Compartment.

This study was performed to assess how the degree of uterine prolapse affects the anterior pelvic compartment using transperineal ultrasound. This single-center, retrospective study included 287 patients with uterine prolapse as assessed by transperineal ultrasound. The patients were categorized into mild and severe groups based on the severity of uterine prolapse. All patients underwent pelvic floor ultrasonography, during which the bladder neck descent, lowest point of the bladder, retrovesical angle, urethral tilt angle, and urethral rotation angle were observed to classify cystocele types.

T‑shaped sling with modified posterior pelvic reconstruction: a technical note with video vignette.

To address the lack of standardized procedures for concurrent stress urinary incontinence (SUI), cystocele, and vaginal laxity, we developed a novel, integrated approach that employs a customized inverted T-shaped sling for bladder neck-to-midurethral suspension combined with modified posterior pelvic reconstruction. The widened sling design with posteriorly shifted suspension vector prevents postoperative voiding dysfunction associated with traditional slings. The modified reconstruction technique employs high-strength absorbable barbed sutures for bilateral levator ani plication and perineal

De-Novo Stress Urinary Incontinence After Apical Prolapse Surgery: Potential Link with the Zone of Critical Elasticity.

Background/Objectives: Pelvic organ prolapse (POP) surgery can lead to postoperative stress urinary incontinence (SUI) in previously continent women, termed de novo SUI. This study assessed the incidence and risk factors of de novo SUI after apical POP repair, hypothesizing that reduced bladder neck elasticity-particularly within the zone of critical elasticity (ZCE) described by the Integral Theory-contributes to its development. Methods: A retrospective single-center analysis was performed in 206 postmenopausal women (≥60 years) who underwent apical POP surgery without concomitant anti-incon

Gynecological Morbidity in Geriatric Women: Findings From a Cross-Sectional Study in a Tertiary Care Center in Northern Sri Lanka.

Background and objective Gynecological disorders are prevalent among elderly women due to ageing and hormonal changes in the post-reproductive period. However, there is limited evidence in Sri Lanka to guide the provision of optimal care for geriatric women with these conditions. Therefore, this study aimed to describe the gynecological disorders among elderly women. Methods A hospital-based descriptive cross-sectional study was carried out among 327 women aged 60 years and above who attended the gynecology and urology clinics at the Teaching Hospital Jaffna, Jaffna, Sri Lanka. I saw the "hourglass" sign: CT diagnosis of inguinoscrotal bladder herniation with pseudo-diverticula.

Bladder herniation into the inguinoscrotal region is an uncommon condition that can present with lower urinary tract symptoms and scrotal swelling. Early recognition is critical to avoid serious complications during surgical repair. We report a case of a 75-year-old man with a history of multiple left inguinal hernia repairs who presented with urinary urgency, urge incontinence, and hematuria. Physical examination revealed a persistent scrotal mass. Contrast-enhanced computed tomography (CT) showed a cystic lesion within the left scrotum, directly communicating with the urinary bladder. Can we use integrated total pelvic floor ultrasound as a screening tool in defaecatory pelvic floor dysfunction? A prospective evaluation of the accuracy of integrated total pelvic floor ultrasound compared with defaecation proctography.

AIM: Pelvic floor dysfunction is common and includes symptoms such as urinary incontinence, pelvic pain, faecal incontinence, obstructive defaecation syndrome symptoms and pelvic organ prolapse. It is investigated with defaecation proctography (DP) and integrated total pelvic floor ultrasound (TPFUS). Whilst DP is currently the gold standard, TPFUS is efficient and less invasive, offering additional sphincter function assessment. This study aimed to compare TPFUS accuracy to DP in the evaluation of pelvic floor dysfunction. METHODS: From 2015 to 2016, a prospective observational study was con

Martius flap in recurrent rectovaginal fistula: a video vignette.

PURPOSE: We aimed to demonstrate the Martius flap treatment in a patient who had a recurrent iatrogenic rectovaginal fistula following cystocele repair. METHOD: We performed a Martius flap procedure for the reconstruction. A vascularized flap was harvested from the labial fat pad, ensuring preservation of its blood supply via the internal pudendal vessels. The flap was then transposed to the defect site to provide adequate vascular support and promote healing. RESULTS: The patient initially underwent a primary repair of the rectovaginal fistula, which did not achieve fistula healing. A diver
