Rectal Diverticulum as a Rare Complication Following the Longo Procedure: A Case Report and Review of Therapeutic Challenges.

Iatrogenic rectal diverticulum is an exceptionally rare complication following the Longo procedure (stapled hemorrhoidopexy), with only a handful of cases reported in the literature. This case is noteworthy due to its rarity, the diagnostic challenges it presents, and its therapeutic implications for patients undergoing stapled hemorrhoidopexy. Reporting this case aims to raise awareness among clinicians about this uncommon complication and contribute to the understanding of its clinical management. A 60-year-old woman underwent the Longo procedure in 2021 for symptomatic Grade III hemorrhoida

Magnetic resonance defecography assessment of obstructed defecation syndrome in patients with chronic constipation in a tertiary care hospital.

BACKGROUND: Obstructed defecation syndrome (ODS) is a subtype of constipation that is considered one of the major pelvic floor dysfunctions affecting the aging population, particularly women over 50 seeking medical care. The condition is characterized by the urge to defecate but an impaired ability to expel the fecal bolus. ODS is associated with various anorectal abnormalities, which are not always apparent during a standard physical examination, requiring specialized imaging techniques for proper diagnosis. AIM: To study the distribution of causes of ODS in patients with chronic constipatio

Comparative clinical outcomes of ventral rectopexy versus STARR in the management of obstructed defecation syndrome: a clinical review.

Obstructed defecation syndrome (ODS) is a multifactorial pelvic floor disorder commonly affecting women, characterized by straining, incomplete evacuation, and dependence on manual maneuvers. Among various surgical options, stapled transanal rectal resection (STARR) and laparoscopic or robotic ventral mesh rectopexy (LVMR/RVMR) are frequently employed. However, the comparative effectiveness of these procedures remains debated. This narrative review synthesizes evidence from 43 studies including randomized trials, cohort studies, registries, and systematic reviews published between 2004 and 202

Role of magnetic resonance defecography in the assessment of obstructed defecation syndrome.

Obstructed defecation syndrome (ODS) represents an important cause of constipation, primarily arising from dysfunctions within the pelvic floor. Characterized by an inability to complete defecation or effectively evacuate fecal material despite the urge to defecate, ODS results in a persistent sensation of incomplete evacuation and often requires excessive straining during defecation. Conventional clinical examinations fail to adequately assess the complex dynamic dysfunctions of the pelvic floor and anorectal region. Magnetic resonance defecography (MRD), a sophisticated form of dynamic pelvi

Rectocele.

Rectocele is one of the more common pathologies of the pelvic floor. It is a clinical problem that significantly worsens the quality of life by causing obstructed defecation symptoms such as anal digitation, incomplete emptying, and dyschesia. Proper differential diagnosis, distinguishing rectocele from enterocoele or perineal body descent, allows for making proper therapeutic decisions and consists of medical history, ultrasound examination, dynamic magnetic resonance imaging (MRI), and defecography. The most common treatment procedures are vaginal native tissue repair, transanal repair, and

Patient preference of integrated total pelvic floor ultrasound versus defaecatory proctography.

AIM: Pelvic floor defaecatory dysfunction is traditionally investigated with defaecatory proctography, which examines rectocele, enterocele, intussusception and functional aspects including dyssynergy and completeness of evacuation. Integrated total pelvic floor ultrasound offers a means to investigate these anatomical features as well as muscle co-ordination and propulsion, without the requirement of rectal contrast expulsion on a commode. We explore patient preference for each procedure. METHOD: A prospective study was taken of women who had undergone both defaecatory proctography and total

Unusual presentation of aggressive angiomyxoma arising from the posterior vaginal wall.

A multiparous woman presented with a large bulge protruding from the posterior wall of the vagina, initially diagnosed as a rectocele. Imaging revealed a cystic lesion measuring 9 cm × 6 cm × 3.5 cm suspected to be a developmental cyst. The lesion underwent surgical excision with significant improvement in symptoms. Histological analysis reported an aggressive angiomyxoma (AAM); postoperative hormonal therapies were considered and rigorous follow-up initiated. The authors discuss challenges in investigating and managing AAM with proposals made for improving preoperative diagnosis and optimisin

Predictive Factors of Pelvic Organ Prolapse and the Quality of Life Among Jordanian Women: A Case-Control Study.

BACKGROUND: Pelvic organ prolapse (POP) is a common gynecological problem. Several risk factors have been identified to contribute to the main pathophysiological mechanism of prolapse development. However, the POP data in the Eastern Mediterranean region, including Jordan, are limited, and the risk factors associated with POP were mainly obesity and high number of vaginal deliveries. Since Jordan has a high prevalence of obesity, parity, and type 2 diabetes, the present study aimed primarily to identify the modifiable and non-modifiable factors associated with POP in Jordanian women, and secon
