Clinical characteristics of different prolapse phenotypes in severe pelvic organ prolapse: a single-centre retrospective study.

BACKGROUND: Severe pelvic organ prolapse (POP) shows heterogeneous patterns of compartment involvement. We compared three major severe POP phenotypes and explored the adjusted association between manual labour and total pelvic organ prolapse (TPOP). METHODS: This retrospective study included 417 women with POP-Q stage III-IV POP treated between 1 June 2022 and 30 June 2025. The main comparison included isolated anterior compartment prolapse (n = 128), anterior and apical compartment prolapse (n = 162), and TPOP (n = 67); the remaining 60 cases were used only to describe overall phenotype dist

Apical Support After Benign Laparoscopic Hysterectomy: A Randomized Controlled Trial Comparing Vaginal Cuff Closure Techniques.

INTRODUCTION AND HYPOTHESIS: The route of vaginal cuff closure after laparoscopic hysterectomy has not been standardized. This study was designed to compare pelvic floor support after vaginal or laparoscopic cuff closure. The authors hypothesized that laparoscopic closure would be superior. METHODS: This randomized controlled trial included women undergoing a benign total laparoscopic hysterectomy. Exclusion criteria included concurrent prolapse procedure, malignancy, and a pelvic organ prolapse quantitation system (POPQ) point C greater than -4. Preoperative POPQ exams were performed by blin

Factors Associated With Self-reported Family History of Pelvic Floor Disorders.

IMPORTANCE: Although useful to drive prevention efforts, awareness of family history (FH) of pelvic organ prolapse (POP) or urinary incontinence (UI) may be influenced by factors other than the actual presence or absence of POP/UI in a family member. This analysis identifies demographic and clinical factors associated with positive self-reported family history (FH POP/UI +) during the first pregnancy and through 1-year postpartum (1-yr PP). OBJECTIVES: Based on self-reported FH during the third trimester, the objectives were to evaluate differences between nulliparous women with and without F

Post-Hysterectomy Vaginal Vault Prolapse: Current Perspectives and Decision-Making Considerations for Surgical Treatment.

Background and Objectives: Following the removal of the uterus, due to prolapse or other indications, disruption of the apical support structures may result in descent of the vaginal apex. As the condition progresses, the cystocele, rectocele, and/or enterocele are formed, finally creating a complex vaginal eversion that generally requires surgical multi-compartmental repair. This review aimed to provide a comprehensive overview of cur-rent surgical modalities for post-hysterectomy vaginal vault prolapse, which could sup-port personalized treatment. Management and Outcomes of Uterine Smooth Muscle Tumour of Uncertain Malignant Potential (STUMP).

BACKGROUND: Uterine smooth muscle tumours of uncertain malignant potential (STUMP) is a mesenchymal tumour which does not fulfil the criteria for leiomyoma, benign histological subtypes, or leiomyosarcoma. The risk factors for recurrence and the rates of recurrence vary amongst different studies; as a result, there are currently no consensus guidelines for optimal management. METHODS: Patients diagnosed with STUMP that were treated in a tertiary medical centre in Hong Kong were included in the study. The clinical and histological features were analysed and reviewed. Any adjuvant treatment was

The effects of spatial structure on a host-parasite model with horizontal and vertical transmission.

Different strategies have been developed to refine the prediction of the risk of preterm delivery in asymptomatic patients. Transvaginal sonography has been used for this reason to measure and examine the length and shape of the cervix. In this review, we focus on clinical studies involving transvaginal sonographic assessment of the cervix in asymptomatic women at high risk of preterm delivery and in the general pregnant population. Three ultrasound signs are suggestive of cervical incompetence, namely, dilatation of the internal os, sacculation or prolapse of the membranes into the cervix (wi

Post-surgical Comparison of Volumetric Analysis in Orbital Fractures with Conventional and Advanced Management.

BACKGROUND: Pelvic floor dysfunction is a disorder predominantly affecting females. It is common and undermines the quality of lives of at least one-third of adult women and is a growing component of women's health care needs. Identifying and supporting these needs is a major public health issue with a strong psychosocial and economic basis. The importance of the interdependence of mechanical, neural, endocrine and environmental factors in the development of pelvic floor dysfunction is well recognized. There is a paucity of data investigating the true prevalence, incidence, specific risk facto

A rare case of chronic complete uterine inversion in a postmenopausal woman: a case report and literature review.

Uterine inversion is a rare condition that refers to the collapse of the fundus into the uterine cavity and occurs in puerperal and non-puerperal conditions. Non-puerperal uterine inversion is particularly infrequent. Diagnosing non-puerperal uterine inversion is often challenging because it resembles vaginal or cervical tumors and pelvic organ prolapse. Furthermore, this condition alters the anatomical structure of pelvic organs, thereby complicating diagnosis and treatment, and potentially leading to misdiagnosis with grave consequences. We report a case of a postmenopausal woman who present

Optimizing the regenerative potential of vaginal fibroblasts: The role of autologous platelet-rich plasma and hyaluronic acid in vitro.

OBJECTIVE: Many postmenopausal women suffering from vulvovaginal atrophy are looking for non-hormonal treatments. Platelet-rich plasma (PRP) therapy has emerged as a novel and promising approach for gynecological applications. PRP is an autologous blood product rich in growth factors used to stimulate tissue regeneration. On the other hand, hyaluronic acid (HA) is used as a treatment for vaginal dryness as it improves tissue hydration thanks to its strong capacity to retain water. This study examines the in vitro effects of PRP alone or combined with HA on vaginal fibroblasts (VFs) isolated fr

Incorporating Perineorrhaphy into the LeFort Colpocleisis: A Modified Surgical Approach.

INTRODUCTION AND HYPOTHESIS: Perineorrhaphy with levator myorrhaphy is considered a standard part of colpocleisis. Typically, this is done through a separate incision after colpocleisis is completed. We present a video to demonstrate a modified approach to LeFort colpocleisis incorporating perineorrhaphy into the procedure. METHODS: A rectangular piece of anterior vaginal mucosa is dissected off the underlying fibromuscular tissue starting from 2-3 cm distal to the cervix and ending proximal to the urethrovesical junction. Similarly, a symmetrical posterior vaginal mucosa is dissected startin

Profiling of the macrophage response to polypropylene mesh burden in vivo.

Pelvic organ prolapse (POP) surgical repair with polypropylene mesh (PPM) offers improved anatomical outcomes compared to reconstruction using native tissue. However, PPM repair is hampered by complications, most commonly pain or mesh exposure, occurring in over 10 % of cases. This maladaptive response is, in part, attributed to the host response to a foreign material. Previous studies have demonstrated that mesh properties, such as weight, pore size, and porosity, influence downstream outcomes. In addition, computational models and in vivo mechanistic studies demonstrate that mesh deforms aft
