Socio-medical consequences of living with obstetric fistula: a phenomenological study of the lived experiences of women in the Tamale Metropolis, Ghana.

BACKGROUND: Obstetric fistula is a distressing condition affecting mothers in Ghana and elsewhere. Most obstetric fistulas are caused by long-term obstructed labor. The majority of women suffering from this condition and their families go through challenging experiences with medical treatment and social integration. This study explored the lived experiences of women with obstetric fistula regarding the socio-medical consequences of the condition in terms of treatment-seeking challenges and socio-cultural integration in the Tamale Metropolis of the Northern Region of Ghana. METHODS: The study

Social reintegration of women after obstetric fistula surgery: Evidence from a longitudinal multilevel mixed-effects study in Zambia.

INTRODUCTION: Obstetric fistula remains a persistent maternal health challenge in sub-Saharan Africa, severely impairing women's physical, social, and psychological well-being. Although surgical repair restores continence, less is known about the factors influencing women's social reintegration after treatment. This study examined the association of continence status and follow-up time on social reintegration among women who underwent obstetric fistula repair in Zambia. MATERIAL AND METHODS: This prospective cohort study included 2172 women who underwent fistula repair between 2017 and 2023 a

Robot-Assisted Laparoscopic Transvesical Closure of a Vesicovaginal Fistula: First Report in a Teenage Girl.

Vesicovaginal fistula (VVF) is rare in children and is most commonly associated with retained vaginal foreign bodies. Surgical repair is technically demanding and carries a significant risk of recurrence, which has limited the application of minimally invasive approaches. We report the first pediatric case of robot-assisted laparoscopic transvesical repair of a VVF. A 14-year-old girl presented with longstanding urinary incontinence, malodorous vaginal discharge, and recurrent urinary tract infections, since early childhood. Initial imaging revealed a 30-mm bladder stone. Cystoscopy identified

State of the Art: Single-Port Robot-Assisted Extraperitoneal Transvesical Surgery-Technique, Indications, Outcomes, and Pitfalls.

BACKGROUND: The transvesical route provides direct access to the bladder and prostate whereas avoiding peritoneal entry. With the advent of the da Vinci Single-Port (SP) platform, the extraperitoneal (EP) transvesical procedure has reemerged as a minimally invasive option for benign, oncologic, and reconstructive urologic procedures. However, guidance regarding setup, patient selection, outcomes, and technical challenges remains limited. OBJECTIVES: To provide a procedure-oriented overview of the SP robot-assisted EP transvesical procedure, focusing on access, indications, outcomes, and techn

Obstetric Fistula in Zambia: A Population-Based Cross-Sectional Analysis of Associated Factors Using the 2018 DHS Data.

BACKGROUND: Obstetric fistula remains an understudied severe maternal health outcome in Zambia. This study examines the socioeconomic, reproductive, and healthcare access factors associated with obstetric fistula among women in Zambia. METHODS: We conducted a cross-sectional analysis of 10 296 women using data from the 2018 Zambia Demographic and Health Survey (ZDHS). The outcome variable was self-reported lifetime experience of continuous leakage of urine or stool from the vagina. Survey-weighted proportions were used to describe the prevalence and distribution of obstetric fistula. Associat

Robotic pneumovesical vesicovaginal fistula repair using the da vinci sp system: technical feasibility and early outcomes.

OBJECTIVE: To evaluate the outcomes and technical feasibility of robotic-assisted pneumovesical repair of vesicovaginal fistulas (VVFs) using the da Vinci single-port (SP) robotic system. METHODS: Between April 2024 and January 2026, ten patients underwent robotic-assisted pneumovesical VVF repair. Following diagnostic cystoscopy, a single 2.5-3 cm suprapubic incision was created, and a robotic port was placed directly into the bladder. A pneumovesicum was established using CO₂ insufflation, and the fistula tract was dissected and closed in layers using the da Vinci SP system. RESULTS: The m

Bladder stone as the first clue to a hidden vesicovaginal fistula: diagnostic and surgical insights from a multidisciplinary approach-a case report.

BACKGROUND: Women presenting with urinary tract symptoms such as bladder stones are often presumed to have primary urological disease. However, urogynecologic conditions-including vesicovaginal fistula (VVF) and retained vaginal foreign bodies-can rarely manifest with predominant urological symptoms, leading to diagnostic delay. CASE DESCRIPTION: A 20-year-old female presented with persistent lower urinary tract discomfort and urinary leakage for 2 years. She denied abdominal pain, distension, or menstrual irregularities. No psychiatric or surgical history was reported, and renal function was

A novel four-step protocol using intracavitary contrast-enhanced ultrasound combined with pelvic floor ultrasound for the diagnosis and preoperative planning of vesicovaginal fistulas.

BACKGROUND: Vesicovaginal fistula (VVF) is a debilitating condition that significantly impairs quality of life. Accurate preoperative assessment of fistula characteristics is crucial for surgical planning. We aimed to evaluate the diagnostic efficacy of a novel four-step protocol combining transperineal pelvic floor ultrasound with intracavitary contrast-enhanced ultrasound (IC-CEUS) for VVF assessment. METHODS: This retrospective study included 17 patients with suspected VVF treated at our institution between March 2015 and May 2025. All patients underwent the proposed four-step IC-CEUS prot

Vesicovaginal reflux: a case report and literature overview.

Vesicovaginal reflux (VVR) is a functional voiding disorder characterized by the retrograde flow of urine into the vagina during micturition, often presenting with daytime incontinence or postvoid dribbling. It is commonly underrecognized and may be mistaken for anatomical abnormalities such as vesicovaginal fistula or obstructive hydrocolpos, which require surgical intervention. We report a case of an 8-year-old girl who presented with dysuria, urinary urgency, and fever. Transabdominal and transrectal ultrasounds revealed an anechoic area measuring 7.3 × 3.3 × 4.0 cm in the vagina while urin

Addressing complicated vesicovaginal fistula post-intercourse in MRKH: the critical imperative for comprehensive sex education and awareness.

INTRODUCTION: Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome, a rare congenital disorder, affects approximately 1 in 4500 to 5000 females, leading to the underdevelopment or absence of the uterus and upper vaginal segment. While the physical manifestations of MRKH are substantial, the associated psychological and social repercussions, exacerbated by societal norms, can be profound. Inadequate sex education further compounds the challenges faced by these women, particularly in postmarital contexts. This report highlights an unprecedented case of a young woman with MRKH type II who developed a c

Vesicovaginal Fistula Repair Using Amniotic Membrane Interposition: A Case Report.

Vesicovaginal fistula (VVF) is a debilitating condition associated with significant physical, psychological, and social morbidity. Surgical repair remains challenging because recurrence is often related to poor tissue quality and impaired wound healing. Human amniotic membrane (AM) possesses anti-inflammatory, anti-fibrotic, antimicrobial, and regenerative properties that may enhance tissue repair. We report the case of a 51-year-old woman who developed continuous urinary leakage following total hysterectomy for uterine fibroids. Clinical examination and cystoscopy revealed a 10-mm high vesico

Calcified Foley Catheter and Vesicovaginal Fistula: A Case Report.

Calcification of indwelling urinary catheters is a known complication of prolonged urinary catheterization. It is caused by the formation of a bacterial biofilm, which promotes the accumulation of calcium deposits around the balloon. These deposits can lead to recurrent urinary tract infections, bladder stones, and, more rarely, bladder wall damage that can progress in the long term to a vesicovaginal fistula. We report the case of a 56-year-old woman living in a rural area who presented with persistent vaginal urinary incontinence. The patient reported having a urinary catheter that had been

St Mark's protocol for standardised examination under anaesthesia for rectovaginal fistulae.

Rectovaginal fistulae are uncommon but highly morbid, with presentations ranging from vaginal defaecation to subtle symptoms such as flatus, discharge, or recurrent infection. Imaging and contrast studies may be inconclusive for short, low-lying tracts, and ultrasound techniques can be limited by false-negative results. A structured examination under anaesthesia therefore remains central to definitive diagnosis and operative planning. The St Mark's protocol standardises this assessment using a reproducible five-stage sequence: (1) direct proctovaginal inspection with evaluation of the rectovag

Bridging Gaps: Insights from a Rectal Buttonhole Tear Case Series.

INTRODUCTION AND HYPOTHESIS: Obstetric rectal buttonhole tears (ORBT) are isolated rectal mucosal lacerations with intact anal sphincters. These injuries are frequently overlooked during delivery, resulting in delayed diagnosis, rectovaginal fistula formation, and the need for surgical referral. METHODS: A descriptive case series was conducted on seven women referred for ORBT repair between X across Y under Z (60,000 deliveries). All patients had intact anal sphincters with confirmed rectal mucosal tears identified at the time of delivery through systematic digital rectal examination and subs
