Attention to the Editor - Case series: "Laparoscopic robot-assisted extravesical repair of vesicovaginal fistula post-hysterectomy: a study of peritoneal flap inlay technique and outcomes".

Vesicovaginal fistulas occur in approximately one out of 1800 hysterectomy cases in Western world countries. Various surgical techniques for fistula repair are described in the literature. This research seeks to introduce our uniform operative technique and assess the results of minimally invasive robot-guided vesicovaginal fistula closure employing a peritoneal flap insert. From 2011 to 2022, surgery was performed on a total of 21 individuals. Nineteen patients developed vesicovaginal fistulas following hysterectomy, while two presented with more complex fistula formations. We evaluated surgi

Management of two variants of the OEIS complex (Omphalocele, Exstrophy, Imperforate anus, Spinal defects) : case reports and literature review".

BACKGROUND: There is a wide spectrum of anorectal malformations (ARMs). The majority of ARM types can be diagnosed thorough perineal examination and radiological investigations such as MRI and distal loopogram. The most important aim of ARM repair is keeping the rectum in the muscle complex to protect continence, but still there are important factors that influence continence such as spine anomalies and associated urological anomalies. CASE PRESENTATION: In our report we describe two patients with ARM and abnormal anal sphincteric locations. The first case was a two-year-old female patient who

Sexual and bowel function in women with obstetric anal sphincter injuries after perineoplasty and sphincterorrhaphy.

Obstetric anal sphincter injury (OASIS) may lead to persistent sexual and bowel dysfunction, even after surgical repair. Evidence on the functional outcomes after sphincterorrhaphy and perineoplasty remains limited, particularly in Indonesia. The aim of this study was to analyze the associations of perineal muscle strength and anal sphincter tone with sexual and bowel function in women with OASIS after sphincterorrhaphy and perineoplasty. This cross-sectional study was conducted at Hasan Sadikin Hospital, Bandung, Indonesia. Women with anal incontinence secondary to OASIS or fecal incontinence

Successful Closure of Multiple Post-bariatric Surgery Fistulas Using Modified Internal and External Endoscopic Vacuum Therapy: A Case Report.

Leaks and fistulas following bariatric surgery are serious complications associated with sepsis, malnutrition, and high morbidity. Endoscopic vacuum therapy (EVT) has emerged as an effective minimally invasive alternative for the management of transmural defects of the upper gastrointestinal tract. Recently, low-cost modifications using conventional hospital supplies have been developed to expand their applicability in resource-limited settings. We present the case of a 46-year-old female patient with a history of multiple bariatric surgeries: sleeve gastrectomy (2017), Roux-en-Y gastric bypas

Colorenal Fistula in a Patient With Recurrent Urinary Tract Infections and Urinary Urgency: A Case Report.

Colorenal fistula is a rare condition resulting from chronic inflammation, malignancies, diverticulitis, or infection. This case examines a 68-year-old female with a history of urinary urgency, left back pain, and recurrent urinary tract infections. Anteroposterior non-contrast computed tomography scan showed a large stone in the renal pelvis and gas in the collecting system. Left ureteroscopy revealed multiple ureteral polyps. Retrograde pyelogram revealed contrast filling the left colon. Ureteral polyp biopsies were benign, and colonoscopy was negative for malignancy. This case describes the

Predictors of recurrence after vesicovaginal fistula repair: a systematic review of surgical and patient-related factors.

BACKGROUND: Repair of vesicovaginal fistula (VVF) is highly effective, with primary success rates of 80-95%; however, recurrence is reported in 10-30% of cases. Factors influencing recurrence include fistula size and tissue characteristics. METHODS: This systematic review identifies predictive factors for VVF recurrence by reviewing 21 studies published between 1994 and 2025 that met PRISMA criteria, and by analyzing the surgical outcomes from PubMed/MEDLINE data for various etiologies of VVF. Eligible studies reporting predictors of VVF repair were assessed for bias using the modified Newcas

Microsurgery in mucosal advancement flap repair: a microscope-assisted, tissue-sparing approach for managing complex perianal fistulas within the IDEAL Stage 1 framework.

BACKGROUND: The management of complex perianal fistula remains challenging, requiring a balance between effective tract eradication and preservation of anal sphincter function. Despite multiple surgical options, recurrence rates and functional outcomes remain suboptimal. Microsurgical techniques, widely adopted in other surgical fields, may offer enhanced precision and tissue preservation. This study aims to describe the implementation of a microsurgical approach for complex perianal fistula within an IDEAL Stage 1 framework. METHODS: A prospective single-center case series was conducted incl

Impaired keratinization activity in perianal Crohn's fistulas is associated with poor prognosis.

BACKGROUND: Perianal fistulas remain a major complication in Crohn's disease, profoundly impacting quality of life. Although often therapy refractory, strong heterogeneity exists between patients, both in clinical behavior and therapy response. In this study, we aimed to identify biological factors involved in the clinical behavior of perianal fistulas, allowing better stratification. METHODS: Forty-nine patients were included (Crohn's n = 36, cryptoglandular n = 13). Gene expression was analyzed in internal fistula openings and tracts by RNA sequencing. Data were verified using spatial trans

Robot-Assisted Sigmoid Colon Conduit for Urinary Diversion: Avoiding Bowel Anasto-mosis in a Patient with Prior Loop Colostomy and Pelvic Radiation.

PURPOSE: Late radiation-induced genitourinary and gastrointestinal complications after multimodal treatment for pelvic malignancies frequently require complex reconstructive surgery, often necessitating both urinary and fecal diversion (1). In this setting, a colonic segment may provide an effective option for urinary diversion while avoiding bowel anastomosis (2, 3). Minimally invasive robotic approaches have increasingly expanded the feasibility of these complex reconstructions and may reduce morbidity, particularly in frail or previously irradiated patients (4-7). We describe a single-stage

Palliative Management of Inoperable Vesicovaginal Fistula in Cervical Cancer Patient Using Bilateral Single-J Stents and Bladder Stoma: A Pilot Study.

INTRODUCTION AND HYPOTHESIS: Vesicovaginal fistula (VVF) is characterized by the formation of an abnormal connection between the bladder and the vagina, leading to continuous, involuntary leakage of urine through the vaginal canal. For patients with inoperable fistulas, individualized urinary diversion becomes necessary to alleviate incontinence and pelvic pain, thereby enhancing the patient's quality of life. This pilot study aims to evaluate the feasibility and clinical benefits of an innovative use of single-J (SJ) stents in conjunction with bladder stomas for palliative management of VVF s

Mid-term Clinical and Radiological Outcome of the BioBall Adapter System in Revision Total Hip Arthroplasty: A Retrospective Study.

AIM AND BACKGROUND: Revision total hip arthroplasty (rTHA) is a technically demanding procedure, particularly when the femoral stem is well-fixed. Removal of a stable stem increases operative time, morbidity, and risk of complications. The BioBall modular head-neck adapter system allows intraoperative restoration of leg length, offset, and version without stem removal. This study evaluates the mid-term clinical and radiological outcomes of the BioBall system in revision THA. METHODS: A retrospective, single-center observational case series was conducted including all rTHAs performed at Wright

Rapunzel Syndrome Complicated by Cholecystoduodenal Fistula Secondary to Biliary Compression: A Case Report of a Patient With Cerebral Palsy.

Rapunzel syndrome is a rare form of gastric trichobezoar that extends into the small intestine, leading to intestinal obstruction. Biliary-enteric fistulas are abnormal communications between the biliary system and the gastrointestinal tract, generally occurring spontaneously. We report the case of a 26-year-old female patient with a history of cerebral palsy who presented to the emergency department with an acute abdomen. Computed tomography revealed a mass occupying the stomach and intestine. A laparotomy was performed, identifying a gastric and duodenal trichobezoar complicated by an acute
