Functional outcomes and quality of life after vesicovaginal fistula repair: beyond anatomic closure.

PURPOSE OF REVIEW: Vesicovaginal fistula (VVF) repair has been historically measured by anatomical closure rates alone. This review summarizes recent evidence showing that successful treatment should also account for urinary continence, sexual function, psychological well-being, and social reintegration, highlighting a multidisciplinary approach to post-repair recovery. RECENT FINDINGS: Although anatomic closure rates exceed 80-95% across surgical approaches, post-obstetric fistula repair incontinence affects 16-55% of women even after successful anatomic closure. Vaginal foreign body for contraceptive purposes resulting in vesicovaginal fistula in an unmarried nulliparous woman: a case report.

INTRODUCTION: Vaginal foreign bodies are an uncommon cause of vesicovaginal fistula (VVF). In resource-limited settings, modern contraceptive methods may be unavailable, and self-insertion of foreign bodies may be attempted as a form of contraception. This case highlights the urgent need for culturally sensitive, community-based sexual and reproductive health education to prevent harmful behaviors and their serious complications. CASE PRESENTATION: A 28-year-old, unmarried, nulliparous woman presented to our facility with foul-smelling vaginal discharge and severe lower abdominal pain. Vagina

Giant Vesicovaginal Calculi Secondary to a Forgotten Vaginal Tampon Complicated With Vesicovaginal Fistula: A Rare Case Report.

Vesicovaginal fistulas in developing countries are mostly of obstetric origin. The etiology involving calcified vaginal foreign bodies is rare. We report an exceptional case of a giant vesicovaginal stone in a 67-year-old woman, secondary to a forgotten and calcified vaginal tampon, complicated by a vesicovaginal fistula. Treatment consisted of stone removal and fistula repair during the same surgical procedure as part of a health campaign for the treatment of urogenital fistulas. The postoperative course was uneventful; the patient was dry upon catheter removal. This observation shows the imp

Retrospective study comparing robotic versus open approaches for genitourinary fistula repair.

INTRODUCTION: Genitourinary fistulas are abnormal communications between the urinary or genital tract and adjacent structures, often requiring surgical repair to restore function and quality of life. Robotic-assisted surgery has gained acceptance due to improved visualisation and surgical precision. This study compares the outcomes of robotic-assisted and open genitourinary fistula repair. PATIENTS AND METHODS: A retrospective review of patients undergoing genitourinary fistula repair between 2010 and 2020 was performed. Patients were divided into robotic (n = 26) and open (n = 24) groups. Su

Teaching Vaginal Surgery Through Simulation.

As the volume of vaginal surgery declines, residents have fewer opportunities to learn, and training programs are incorporating simulation into their curricula to address this deficiency. We conducted a scoping review to identify literature on simulation training for gynecologic vaginal surgical procedures. Data were extracted from each study on the following parameters: model or assessment tool description, study objectives, target skill level, learning objectives, intervention, aims of the study, study outcomes, performance metrics, performance cutoff scores, simulator assessments, Medical E

Forceps-Assisted Delivery Could Reduce the Iatrogenic Fistula Crisis: A Clinical Commentary.

In an era where the aetiology of childbirth-related pelvic floor fistula is rapidly shifting from ischaemic obstetric fistula to iatrogenic childbirth-related fistula related to complications of caesarean section, strategies must be implemented to minimise the ongoing burden of fistula-related pathologies and surgical morbidity and mortality. Despite growing access to caesarean section in low- and middle-income countries (LMICs), this mode of delivery has introduced new maternal complications such as ureterovaginal and vesico-uterine fistulae that often require abdominal repair. The reintroduc

Vaginal Repair of Post-hysterectomy Vesicovaginal Fistula With a Martius Flap: A Case Report.

A vesicovaginal fistula (VVF) is a devastating condition often resulting from gynecological surgery. We present the case of a chronic VVF treated via a vaginal approach with a Martius flap. A 38-year-old woman presented with three years of urinary incontinence following a total abdominal hysterectomy. Diagnosis of a VVF was confirmed by physical exam, methylene blue test, and cystoscopy. The patient underwent vaginal repair with a Martius flap. The Martius flap procedure restored urinary continence and improved the patient's quality of life. This case highlights the importance and surgical ste

Early Versus Delayed Laparoscopic Repair of Surgically Induced Vesicovaginal Fistula.

IMPORTANCE: The optimal timing of surgical repair for vesicovaginal fistula (VVF) remains debated, with traditional recommendations favoring delayed intervention. Whether early laparoscopic repair can achieve comparable outcomes without increasing morbidity is clinically relevant. OBJECTIVES: This study aimed to compare the clinical efficacy and safety of early (within 3 weeks) versus delayed (≥3 months) laparoscopic VVF repair following surgical injury. STUDY DESIGN: A retrospective cohort study was conducted involving 23 patients with surgically induced VVF treated between January 2021 and
