The use of mechanical insufflation-exsufflation in invasively ventilated critically ill children.

BACKGROUND: Invasively ventilated, critically ill children are at risk of retained respiratory secretions, which can obstruct airways, impair gas exchange, and cause lung collapse or ventilator-acquired pneumonia. Endotracheal suctioning partially removes secretions, but additional airway clearance interventions may be required. Mechanical insufflation-exsufflation (MI-E), is a cough augmentation device used to improve secretion clearance in adult intensive care, however it is not well characterised in paediatrics. AIM: To explore MI-E use in invasively ventilated, critically ill children. M

Intraoperative Contrast-Enhanced Ultrasound to Assess Lymph Node Resection and Involvement in Patients Undergoing Robotic-Assisted Thoracic Surgery Lobectomy for Management of Non-Small Cell Lung Cancer.

BACKGROUND: Mediastinal staging is crucial in patients undergoing lung resection for cancer to plan further adjuvant. Current guidelines recommend resecting at least three mediastinal lymph node stations, including Group7. Herein, we evaluated the role of intraoperative Contrast-enhanced ultrasound (CEUS) to guide lymph node resection in patients undergoing RATS lobectomy for management of non-small cell lung cancer. METHODS: It was a retrospective study including 24 consecutive patients underwent RATS lobectomy for management lung cancer. The hilar and mediastinal lymph node was firstly eval

After-Hours Versus In-Hours Percutaneous Coronary Intervention in Patients With Acute Myocardial Infarction Complicated by Cardiogenic Shock.

BACKGROUND: Acute myocardial infarction complicated by cardiogenic shock (AMI-CS) remains a time-critical emergency with persistently high mortality despite advances in reperfusion and mechanical circulatory support. Clinical outcomes may be influenced by the timing of percutaneous coronary intervention (PCI), with after-hours procedures potentially affected by logistical and staffing constraints. The impact of PCI timing on AMI-CS outcomes remains unclear. METHOD: A retrospective cohort study was conducted using data from the Victorian Cardiac Outcomes Registry including all adults undergoin

Masked ABCA3-Related Interstitial Lung Disease in a Neonate With Prenatally Diagnosed Total Anomalous Pulmonary Venous Connection.

Obstructive total anomalous pulmonary venous connection (TAPVC) and ABCA3-related interstitial lung disease (ILD) rarely coexist. We report a neonate with TAPVC complicated by compound heterozygous ABCA3 variants (c.4180dup/p.V1394fs maternal, c.3446A>G/p.D1149G paternal). Overlapping neonatal respiratory symptoms mask the underlying surfactant disorder, easily causing misdiagnosis and delayed targeted management. This case highlights that the genetic screening for ABCA3 deficiency is mandatory for infants with persistent severe hypoxemia and ventilator dependence after complete TAPVC surgical

Total intravenous anesthesia for open fetal myelomeningocele repair: a case series from a single-center in Peru (2017-2025).

BACKGROUND: Open fetal myelomeningocele repair requires maternal general anesthesia and profound uterine relaxation. Total intravenous anesthesia may be an alternative to volatile anesthetics. We describe our anesthetic approach and maternal, fetal, surgical, and neonatal outcomes. METHODS: We retrospectively reviewed 25 cases of open fetal myelomeningocele repairs performed from January 2017 to July 2025. The anesthetic protocol comprised target-controlled infusions of propofol and remifentanil guided by bispectral index monitoring, neuromuscular blockade, intravenous uterine relaxation, and

Modelling Tetralogy of Fallot: insights and limitations of animal models.

Tetralogy of Fallot (TOF) is the commonest cyanotic congenital heart disease, arising from antero-cephalad deviation of the outlet septum during outflow tract (OFT) morphogenesis, producing a ventricular septal defect, overriding aorta, pulmonary stenosis, and right ventricular hypertrophy. Despite advances in surgical correction, the developmental origins of TOF remain incompletely understood, and the genetic architecture of the majority of non-syndromic cases is yet to be resolved. Experimental models have been central to progress in this field, yet each model may capture only part of the di

A Predictive Model for Pulmonary Embolism in Patients with Atrial Fibrillation Based on Thromboelastography and Coagulation Parameters.

BACKGROUND: Patients with atrial fibrillation (AF) face an elevated risk of pulmonary embolism (PE), yet existing prediction tools demonstrate limited accuracy. OBJECTIVES: This study aimed to develop and validate a novel predictive model integrating thromboelastography (TEG) parameters with conventional coagulation markers for PE risk assessment in AF patients. METHODS: We conducted a retrospective study of 271 hospitalized AF patients who underwent CTPA for suspected PE (41 with PE, 230 without). The mean age was 76.5 years, 56.5% were male, and hypertension was the most common comorbidity

Integrated Cardiopulmonary Profile as an Additive Signal in Patients Evaluated for Suspected Newly Detected Atrial Fibrillation.

BACKGROUND: Atrial fibrillation (AF) is associated with atrial remodelling, hemodynamic stress, and complex cardiopulmonary interactions. While natriuretic peptides and echocardiographic abnormalities are established markers of AF-related substrate, the contribution of spirometry changes, as a landmark of pulmonary impairment, remains less clearly defined. This study evaluated whether AF is associated with an integrated cardiopulmonary profile characterized by altered spirometry, increased NT-proBNP, and conventional echocardiographic markers of left atrial structure and function. METHODS: We

Multiple cardiac malformations in a calf.

This paper describes the morphopathological aspects of a case of multiple cardiac malformations in a calf. A two-day-old male calf of undefined breed was born with an ectopic heart, presenting with dyspnoea and in lateral recumbency. The owner had repositioned the exposed heart beneath the adjacent skin, which was suclosed, without additional incisions or suturing of the heart or pericardial sac. The animal died the following day and was submitted for necropsy. Macroscopically, the heart was observed to be ventrally pendulous, covered by the pericardial sac, external to the thoracic cavity, wi

Spurious Fingertip Oxygen Desaturation During the 6-Minute Walk Test in a Patient With NSIP and Raynaud Phenomenon: The Value of Forehead Pulse Oximetry and Transcutaneous Oxygen Tension.

Reduced digital perfusion associated with Raynaud phenomenon may cause spuriously low fingertip pulse oximetry readings during the 6-minute walk test (6MWT), potentially leading to overestimation of exercise-induced hypoxaemia in patients with interstitial lung disease. We report a 39-year-old woman with non-specific interstitial pneumonia and Raynaud phenomenon who underwent repeated 6MWTs. Fingertip SpO2 progressively decreased from 96% to 82%, whereas forehead SpO2 remained stable (93%-98%). Simultaneously, transcutaneous oxygen tension (TcPO2) measured on the volar forearm remained unchang

Entrectinib-related Myocarditis Causing a Triangular QRS-ST-T Waveform Electrocardiographic Pattern: A Case Report.

INTRODUCTION: Entrectinib is a kinase inhibitor used in ROS1-positive non-small cell lung carcinoma. Cardiovascular toxicity is rare, with only one prior report of myocarditis related to entrectinib use. The triangular QRS-ST-T waveform electrocardiographic pattern is a ST-elevated myocardial infarction equivalent and is rarely associated with myocarditis. CASE REPORT: A 42-year-old male with metastatic non-small cell lung carcinoma presented with presyncope, palpitations, and dyspnea three days after initiating entrectinib. Electrocardiograph (ECG) revealed a "shark-fin" T-wave morphology wi

The Questionnaire for O2 Therapy Evaluation (QuOTE): a simple O2 therapy assessment.

BACKGROUND: Despite the established use of long-term oxygen therapy (LTOT) for chronic respiratory diseases, a simple tool to assess LTOT use, clinical effectiveness and side-effects is not currently available. We therefore designed and developed the Questionnaire for Oxygen Therapy Evaluation (QuOTE). METHODS: Delphi methodology was used by a group of international respiratory specialists to identify the domains and questions to include in the questionnaire. The questionnaire was beta-tested by a group of patients using LTOT. RESULTS: In the first round, 13 clinical and academic respiratory

Montelukast and Bilastine Combination Therapy in Allergic Rhinitis: A Narrative Review of Evidence.

Allergic rhinitis (AR) is a chronic inflammatory disorder of the nasal mucosa triggered by immunoglobulin E (IgE)-mediated responses to allergens. The release of histamines and leukotrienes results in nasal irritation, excessive secretions, and frequent sneezing, affecting both children and adults. This review aims to provide an updated understanding of AR by evaluating the prevalence, treatment approaches, and clinical effects of bilastine and montelukast in AR management. A comprehensive literature search was conducted to identify research papers published between January 2003 and September

Prevalence, burden and manifestation of atrial fibrillation: a large-scale analysis of cardiac patch monitors.

BACKGROUND: Atrial fibrillation (AF) is frequently paroxysmal and asymptomatic, making its detection and burden quantification challenging. The distribution of AF burden across clinically monitored populations remains poorly characterised. We sought to describe the distribution of AF burden and its variation by monitoring indication, symptomatology, arrhythmia characteristics and geography. METHODS: This study analysed cardiac patch monitor reports collected between May 2017 and July 2023. Analyses were stratified by monitoring indication, AF burden grouping and geography. AF burden subgroups
