This report describes the case of an 81-year-old woman with sudden evisceration of the small intestine through the vagina. It occurred one year after repair of a vaginal vault prolapse, which was initially treated by vaginal hysterectomy and colporrhaphy three years prior to the repair. On examination, we found a 70-80-cm loop of bowel prolapsing through a 3-cm oval defect in the vaginal vault. The patient underwent emergency exploratory laparotomy under general anesthesia. After careful reduction of the eviscerated small intestine, the hernia hiatus was closed and the widened cul-de-sac was o

PURPOSE: Vesicovaginal fistula is a socially debilitating problem with important psychological and medicolegal implications. Complicated fistulas include large fistulas or those with failed prior repair attempts. The key to successful closure of such fistulas is the use of adjuncts such as a Martius, peritoneal or omental flap. Small intestinal submucosa is an acellular collagen matrix graft that is nonimmunogenic, biodegradable and ready to use off the shelf. We evaluated small intestinal submucosa as an interposition patch during complicated vesicovaginal fistula repair.

INTRODUCTION: The appendix has a constant vascular anatomy and provides a small lumen that always maintains its patency because of mucosal secretion and motility; thus, it serves as an ideal conduit structurally. The appendix has been used in urologic surgeries as a pedicled flap and as a free flap in isolated case reports for the reconstruction of the urethra. However, this study proposes more extended applications of the appendix in different kinds of reconstruction.
METHODS: From 2002 to 2011, 11 patients were included in this study retrospectively. Of these cases, 8 were transferred as fr

A 44-year-old woman underwent uncomplicated uterine fibroid embolization (UFE) for menstrual and bulk-related symptoms in an enlarged, myomatous uterus. After surgery, she spontaneously sloughed a large mass of fibroids that arrested in the cervical canal during passage. Four days after gynecological extraction, she developed copious vaginal discharge that contained enteric contents. Contrast-enhanced computed tomography (CT) demonstrated a fistula between the small bowel and the uterus. She subsequently underwent hysterectomy, left oophorectomy, and small-bowel resection. Her postoperative re

Chronic colonic diverticulitis may be associated with typical complications such as local abscesses, stenosis, bleeding, intraperitoneal bowel perforations or fistulas to other organs. Most commonly, fistulas exist between the colon and the bladder; nevertheless, they may also extend to the small intestine, other areas of the colon, ureter, uterus, salpinx, vagina, abdominal wall, portal- and mesenterial venous system, pleura, urachus, biliary system and the hip. We report on a patient with chronic colonic diverticulitis having an unusual sigmoido-gluteal fistula along the sacrum, the piriform

INTRODUCTION AND HYPOTHESIS: The objective was to use an animal model to study different types of interposition grafts for rectovaginal fistula repair.
METHODS: Twelve New Zealand white rabbits underwent surgical creation of a rectovaginal fistula, followed by repair. Four repair techniques were studied; three with interposition grafts and one control group without a graft. Animals were euthanized at 4-week intervals and underwent gross and histologic analysis.
RESULTS: The mean rectovaginal wall thickness was greatest in the control group (5.6 mm) and thinnest in the autologous rectus fasci

In 2009, a 41-year-old Thai woman who had undergone abdominal hysterectomy 4 months earlier was admitted because of bleeding per vagina without pain during coitus for 1 day. She had undergone pelvic examination that disclosed small intestine in vagina and scanty bleeding of vaginal vault. Closure of the rupture of the vaginal vault during laparotomy was completed, resulting in satisfactory condition. Vaginal vault rupture is a rare condition. Prompt surgical and medical intervention are required to prevent complications. Incidence, risk factors, and management for rupture of the vaginal vault

Complex vesicovaginal fistula (VVF) can result in severe destruction of urinary and anal sphincters. We present two cases of patients who underwent retubularized ileal stoma (Monti reconstruction) applied to an augmented bladder for continent urinary diversion. This reconstruction provides another option for a diversion technique in patients with irreparable urethra and bladder damage and a severely damaged anal sphincter. To the best of our knowledge, there have been no published studies of Monti reconstruction in patients with complex VVF.

BACKGROUND: Bowel perforation is a rare complication of tension-free vaginal tape (TVT)-type sling procedures and has been reported in patients with a previous history of abdominal or pelvic surgery.
CASE: A 77-year-old woman with no prior history of abdominal or pelvic surgery underwent elective surgery for pelvic organ prolapse with a vaginal vault colpocleisis and suburethral sling. On postoperative day 2, a computed tomography scan performed for suprapubic discomfort showed an enterocutaneous fistula, which was treated surgically.
CONCLUSION: Bowel perforation is a complication of TVT-ty

BACKGROUND: Ileal pouch strictures that are visually inaccessible by an endoscope may be balloon-dilated by exchange guide wire across the stricture with the aid of fluoroscopy. We present a technique of wire-guided balloon dilation without fluoroscopy to navigate strictures in the ileal pouch.
METHODS: A 50-year-old Caucasian female presented with a 24-year history of ulcerative colitis (UC) with restorative proctocolectomy and ileal pouch anal anastomosis (IPAA) for 7 years. She developed Crohn's disease (CD) of the pouch with multiple strictures at the afferent limb of the pouch and a pouc

OBJECTIVE: We report on the feasibility and outcomes of the N-shaped pouch with an afferent tubular isoperistaltic segment as a new technique for creating a capacious, low pressure bladder substitute following radical cystectomy.
METHODS: Between April 2000 and April 2006, 42 patients (36 male, 6 female) with invasive bladder cancer were considered good candidates for orthotopic urinary diversion. All had radical cystectomy with bilateral pelvic lymphadenectomy and orthotopic bladder substitution by an ileal low pressure reservoir (N-shaped) with an afferent isoperistaltic tubular segment. Of

INTRODUCTION: Closing the pelvic peritoneum to prevent the small bowel dropping into the pelvis after surgery for locally recurrent rectal cancer is important to prevent adhesions deep in the pelvis or complications of adjuvant radiotherapy. Achieving this could be difficult because sufficient native tissue is unavailable; we report on the use of small intestine submucosa extra-cellular matrix mesh in the obliteration of the pelvic brim.
CASE PRESENTATION: We describe two cases in which submucosa extra-cellular matrix mesh was used to obliterate the pelvic brim following resection of a recurr

This case report describes use of a porcine small intestinal submucosa patch to prevent vaginal stiffness and retraction after extensive vaginal resection of an endometriosis nodule. A 32-year-old nulliparous woman was referred for surgical treatment of a large rectovaginal nodule that extended from the vaginal to the rectal mucosa. Surgical treatment was performed in 2 steps. Initially, a laparoscopic rectal resection was performed without opening the vagina to reduce the risk of fistula formation; 6 months later, the patient underwent a laparoscopic second-look combined with the vaginal appr

Meconium peritonitis results from antenatal perforation of the gastrointestinal tract; it presents as gastrointestinal obstruction, intraabdominal masses, or calcification. The presentation with passage of meconium per vagina secondary to meconium peritonitis is rare. We describe the radiologic and surgical findings in a neonate who had passage of meconium per vagina secondary to ileal atresia and meconium peritonitis. Initial clinical and radiologic examination suggested rectal atresia with an associated rectovaginal fistula, although subsequently, this was not the case. Possible explanations

Most rectovaginal fistulas are acquired. Obstetrical trauma and types of surgery such as laparoscopic-assisted vaginal hysterectomy may cause high rectovaginal fistulas. The high fistulas are repaired by abdominal approach, while middle or low fistulas are best approached perineally. There are not many reports of totally-laparoscopic repair available in the literature. We present two patients who had a (Type VI) high rectovaginal fistula following laparoscopicassisted vaginal hysterectomy. Laparoscopic repair was successfully performed by suturing the defects and fixing an omental patch betwee

Ileum or colon interpositions are qualified procedures for functional ureteral replacement in case of extended ureteric lesions. In most cases, a unilateral interposition is sufficient. Rarely, it is necessary to perform bilateral procedures. We report the case of a 41-year-old female patient with bilateral ureter implantation into a singular ileum segment.

BACKGROUND: Vesicovaginal fistulae are well-recognized complications of radiotherapy for the treatment of cervical cancer. These patients often require some form of urinary diversion. Small bowel is often utilized for these purposes.
CASE: A patient with stage IIB cervical cancer presented with a vesicovaginal fistula 6 years post-radiotherapy. An ileal conduit was performed. On day 9 post-surgery breakdown of the small bowel anastomosis was suspected. At laparotomy small bowel anastomotic breakdown was confirmed with round worms at the anastomotic site.
CONCLUSION: Perforation of small bowe

Orthotopic cystoplasty for various diseases of the urinary bladder was made in 58 females aged 22 to 78 years (mean age 52.5 +/- 5.8 years) in 1996-2005. Surgery was indicated in muscular-invasive cancer of the bladder (n = 35, 60.3%), microcystis (n = 21, 36.3%), bladder atonia (n = 1, 1.7%), vesicovaginal fistula (n = 1, 1.7%). The reservoir was made of iliac segment in 50 (86,2%) patients, of gastric body - in 8 (13.8%) patients. Postoperative complications arose in 7 (12.1%) patients, late complications occurred in 4 (6.8%). Postoperative lethality was not registered, 12 months later 3 (5.

BACKGROUND: The laparoscopic radical cystectomy (LRC) with orthotopic ileal neobladder is now applied to treat invasive bladder cancer, however, it has not been well codified and illustrated. We describe in this paper a technique step by step that we have developed in 33 patients and achieved excellent results.
METHODS: The surgical procedure can be divided into eight steps: laparoscopic pelvic lymphadenectomy and mobilization of the distal ureters; exposing Denonvillier's space and the posterior aspect of prostate; exposing retropubic space and anterior surface of the bladder; dividing the l
