During a 10 years period at the Department of Urology of the Surgical Clinic KBC "Bezanijska kosa" 32 patients were hospitalised with a single functional kidney, who were operated on and derivation operations of the ureter were performed. The reason for a derivating operation on the ureter was an infiltrative cancer of the urinary bladder in 24 patients. Ileum neobladder after cystectomy is mainly reserved for male patients. Female patients are normally excluded from this kind of urinary diversion for fear of incontinence following orthotopic bladder substitution. The excellent results experienced with the use of seromuscular intestinal grafts in certain types of urological plastic operations encouraged us to use it (as a patch) also in the surgical management of vesicovaginal and rectovaginal fistulas. This paper presents the results of a series of 10 continent urinary valves performed in the urology department of Ibnou-Rochd hospital in Casablanca according to an original technique between 1991 and 1993. These 10 patients (6 men and 4 women) suffered from various diseases of the lower urinary tract: 4 bladder tumrous, 3 complex vesicovaginal fistulae, two of which were associated with a rectovaginal fistula, one bladder exstrophy, one neurogenic bladder and one extensive urethral stricture. One women developed a pouch-vaginal fistula after reconstructive proctocolectomy for ulcerative colitis. This study describes the risk factors which predispose to the development of this rare complication, as well as the various treatment options available. A diverting ileostomy was maintained and gracilis muscle interposition flap was used to treat the patient. An excellent result was achieved. This procedure can be useful in this rare but distressing complication.

The hydraulic ileal valve ensures urinary continence and can be employed in many different reservoirs. The valve is constructed by isolating a 14 cm long intestinal loop with its mesentery. The isolated ileal segment is then folded inward on itself throughout its length. The Authors present a case of bile peritonitis due to a biliary fistula developed after T tube removal from the common bile duct. Highlighting poor clinical manifestations they report an alternative conservative treatment, by drainage of the bile leakage with culdocenthesis and successive endoscopic spincterotomy and transpapillary drainage.

Between 1984 and 1989 21 patients underwent proctocolectomy and were reconstructed with ileal-pouch-anal anastomosis using a J-pouch. 18 patients had ulcerative colitis and 3 adenoma of the colon. In 16 of 21 patients the operation was performed in a three-stage way: (1.) total colectomy; (2.) proctomucosectomy, ileal-pouch-anal anastomosis and protective ileostomy; (3.) ileostomy closure. 5 patients had a two-stage operation (3 patients with adenoma of the colon, 2 patients with low or no steroid medication). The pelvic-pouch procedure has become a standard operation for selected patients with ulcerative colitis, but is contraindicated in patients with Crohn's disease at our institution. Ileum has been used to reconstruct the lower urinary tract in 15 patients (10 women, five men) after treatment for bladder or gynaecological cancer. Seven patients had previously received radical pelvic irradiation. Little information exists on the use of continent urinary reservoirs in patients with previous pelvic irradiation. We report the use of the Indiana pouch urinary reservoir in ten women with a history of pelvic irradiation for cervical cancer, of whom eight underwent a total pelvic exenteration for recurrent pelvic tumor and two had diversion for radiation-induced vesicovaginal fistula. Twenty-eight patients with vaginal fistulas complicating Crohn's disease, seen between 1970 and 1987, are described. Twelve required early operation; five of them had rectal excision. Conservative management was used in 16 patients but in none of these did the fistula close spontaneously. Between 1984 and 1988, 10 patients with ulcerative colitis underwent surgery involving total colectomy and ileoanal reconstruction with a J-pouch. In 8 of the 10 we selected the three-stage surgical procedure: 1. total colectomy, 2. proctomucosectomy, ileoanal pouch anastomosis and double path protective ileostomy, 3. ileostomy closure. Some of the initial problems associated with the ileoanal reservoir have been solved. In their place, other complications have been recognized. Among these, the ileal pouch vaginal fistula stands out as a recently recognized difficult management problem. Construction of continent colonic urinary reservoir was performed in 44 patients after exenteration for invasive bladder carcinoma or various gynecologic tumors. A variety of methods for urinary diversion are employed in patients with gynecologic malignancies depending on whether there is an obstruction or fistula formation due to either tumor growth or radiotherapeutic injury.
