Urinary diversion in patients undergoing pelvic exenteration.

Between October, 1969, and April, 1981, gynecologic oncologists at the University of Alabama Medical Center in Birmingham have performed 119 pelvic exenterations. One hundred fifteen of these patients had a concurrent supravesical urinary diversion. Fifty-six patients (48.7%) had an anterior exenteration and 59 patients (51.3%) had a total exenteration. An ileal segment was used as a conduit in 97 patients while the segment of transverse colon was used in 16 patients. Two patients had sigmoid conduits. Eighty-five patients (73.9%) had the intestinal anastomosis and conduit constructed with gas

Gastrointestinal tract complications following radiotherapy of uterine cervical cancer: past and present.

A retrospective analysis of the gastrointestinal tract complications in 298 patients with cervical cancer treated with radiotherapy at the State University of New York at Buffalo and Albert Einstein College of Medicine affiliated hospitals was carried out. Fifty-two patients had pretreatment surgical staging (39 transperitoneal and 13 extraperitoneal). Twenty-four percent had varying degrees of radiation sickness. They all responded to conservative therapy. Seven percent developed Stage I radiation proctitis. In the clinical staging group late complications consisted of: Three small bowel inju

Repair of postirradiation damage to colorectum: a progress report.

The results of 21 operations for repair of rectovaginal fistula and/or stricture secondary to irradiation for pelvic cancer are presented. The operations rely on the use of proximal nonirradiated colon with normal blood supply for effecting the repair. In patients having had a previous colostomy, it is possible to use the proximal end of the bypassed colon for this purpose. There is minimal dissection of the rectal ampulla and the presacral space is never entered. Continuity is established by anastomosis to the anterior rectal wall via an abdominal approach alone, or by a combined abdominovagi

[X-ray morphology of Crohn's disease (author's transl)].

The article reports on the x-ray morphology in 52 patients with Crohn's disease (part of a multicenter study for the purpose of therapy control). The most frequent morphological sign was the so-called "string sign", also known as Kantor's sign (55%). Changes in the mucosal relief were also very frequent (45%). Chief localisation was the terminal ileum (64%), exclusion affection of the colon being rare (8%). The formation of a fistula is described as the most important complication; a rare one occurred between rectum and vagina. In two cases pseudopolypous changes were seen. The article also di

Elective resection for diverticular disease.

A series of 119 patients undergoing elective resection for diverticular disease has been reviewed. The indications for resection were classified into two major groups--those with severe infections ("complicated" diverticulitis), comprising 58 patients, and those with minimal infections, comprising 61 patients. The majority of the resections were limited to the sigmoid colon (101 patients). Fifteen patients underwent left hemicolectomy, whilst three had total colectomy. Thirty-six patients (30%) had a proximal defunctioning stoma--18 prior to resection and 19 at the time of resection. Anastomot

Effects of radiation on the human gastrointestinal tract.

Radiation therapy directed at the abdomen may damage the digestive tract, the type and extent of injury depending on the dose of the radiation and the radiation sensitivity of the gut. Characteristic early changes are manifest in the mucosa of the gut: for later ulceration, changes in the collagen tissues and particularly in the vascular channels occur. This paper describes and characterizes injuries to the esophagus, stomach, small intestine and colon. It emphasizes the importance of recognizing radiation-induced damage to the gut which may occur early or late after radiation.

Operative management of radiation injuries of the intestinal tract.

A review of forty cases of radiation-induced gastrointestinal injuries is presented. Based on this experience and reports in the literature, preoperative management and operative technics are discussed. The increased risk of radiation bowel injury is recognized in patients who have had previous operations. Preradiation contrast studies are advised to identify trapped loops of intestine in the pelvis. Small bowel resection is recommended with localized segments of disease. Bypass operations are preferable to avoid any extensive dissections. Bypass operations have anastomotic dehiscence rates si

Surgical treatment of the radiation injured bowel.

Over the last 10 years, 9 patients treated by surgical procedure for radiation injuries of the bowel were studied with the following conclusions: The damage to the small intestine caused by external irradiation leads to adhesion of the bowel, perforation and postoperative anastomotic dehiscence if the irradiated bowel is used in the anastomosis. Surgical treatment for the small intestine is resection of the damaged loop. In order to determine the extent of the resection it is important that during the operation fibrosis and obstruction of vessels in the submucosa and subserosa is examined by b

An improved non-intubated cutaneous ureterostomy technique for the normal and dilated ureter.

Cutaneous ureterostomy is a simple procedure to perform but is attended by high morbidity owing to recurring abscesses, strictures and stenosis of the stoma. An improved technique applied to the diversion of 4 normal renal units and 3 dilated ureters is described. We have not experienced the complications mentioned in previous studies and none of the patients has required ureteral intubation in this small series with a 3-year followup.

Complications following operation in the previously irradiated abdominopelvic cavity.

Twenty-three patients underwent 32 operations in a previously irradiated abdominopelvic cavity. The overall postoperative morbidity rate was 62% and the mortality rate was 6.2%. The data from this series do not support small bowel bypass as the only acceptable method of management of surgical radiation enteritis. Wide resection with primary anastomosis reduces the long-term morbidity of the "blind-loop syndrome", perforation and fistulization of the isolated loop, and does not have an increased mortality.

[Surgical treatment of radiation injuries of the intestinal tract after radiotherapy of malignant tumors].

From 1962-1975 in our hospital 44 patients were operated because of complications of the bowel after irradiation of malignant tumors. 3 patients were operated in the region of subsequent irradiation. While at the small intestine stenoses were the most frequent complication, at the great gut fistula of rectum, colon sigmoideum or vagina besides stenoses were frequent. The time between irradiation and operation differed between 5 weeks and 22 years. The peak of frequency in the region of the great gut was seen about the first year after termination of irradiation, in the region of the small inte

Jejunal loop interposition in patients with ileal conduit failure after pelvic exenteration.

Although the ileal conduit is currently the most widely used method for urinary diversion in patients undergoing pelvic exenteration, reports continue to accumulate indicating the frequency of long-term complications. Four patients with failure of ileal urinary diversion after pelvic exenteration for invasive pelvic malignancy are presented. All four patients had either multiple pelvic surgical procedures and/or pelvic radiation. Interposition of jejunum between the proximal ureters or renal pelvises and skin was performed with satisfactory results in all four. In patients with pelvic exentera

Sequelae of abdominal radiation and their medical management.

As various chemotherapeutic agents are added to treatment routines, the possibility of adverse effects is appreciably increased, particularly in those organs for which the agents have specific toxicity. Symptoms most commonly associated with radiation sickness, such as malaise, anorexia, nausea, vomiting, diarrhea, dysphagia, dermatitis, and depleted hemopoietic elements, are usually seen late in the course of radiation therapy or shortly thereafter. Consequently, they are managed by the physician in charge of radiation or the patient's referring physician. The general physician may be concern

[Rare fistulation in enterocolitis regionalis (Crohn's disease) (author's transl)].

The transmural inflammation in Crohn's disease, the variable localization of the disease and the tendency to a descending spread, all lead to a multitude of possible fistulae, the presence of which is often difficult to demonstrate. This is a report on 5 unusual fistula systems: in 2 cases entero-enteral (one of which with hydronephrosis), 1 case of rectovaginal fistula, 1 case of a branched fistula system in the right buttock, 1 case of fistulae between an abscess system in the spleen "bed" and the greater curvature of the stomach. A radiological and endoscopic diagnosis is important since su
