Beyond the Target Sign: Two Cases of Low-Type Anal Atresia Diagnosed Prenatally With Fetal Perineal Imaging.

Prenatal diagnosis of low-type anorectal malformations (ARMs) can be challenging, particularly when the target sign appears displaced or misleading. We report two fetuses-one male and one female-with isolated low-type anal atresia, both diagnosed prenatally through targeted axial and midsagittal transperineal ultrasound and confirmed postnatally. In the male fetus, an eccentrically located target sign and a hyperechoic tract suggested a perineoscrotal fistula. In the female fetus, the anal canal was anteriorly displaced, with a fistulous tract extending to the posterior vaginal introitus. Abse

Delayed coloanal anastomosis for rectovaginal fistulas: Insights from a multicenter cohort (GRECCAR).

BACKGROUND: Delayed coloanal anastomosis for treating rectovaginal fistulas focuses on optimizing healing by postponing the coloanal anastomosis until after the vaginal repair. This study aimed to evaluate the success rate, morbidity, and functional outcomes of this strategy. METHODS: This retrospective study included all patients who underwent delayed coloanal anastomosis for rectovaginal fistula between 2010 and 2023 across 16 GRECCAR-affiliated colorectal centers. RESULTS: A total of 78 patients (median age: 54.5 years) were included. The main cause of rectovaginal fistula was chronic lea

Management of rectovestibular fistula in infants and children: Our experience and insights regarding primary versus staged anorectoplasty.

Vestibular fistula is the most common anorectal malformation in females. Traditionally managed with a staged posterior sagittal anorectoplasty (PSARP) following colostomy, concerns remain regarding colostomy-related morbidity. Intra-operative, early, and late postoperative complications, and functional outcomes (voluntary bowel movements, soiling, and constipation) were compared between secondary PSARP (after colostomy) and primary PSARP (without) using appropriate statistical tests. Functional outcomes were comparable, with >90% achieving voluntary bowel movements. Thus, we conclude that prim

Anatomical Correction and Early Outcomes of One-step Ventral and Dorsal Proctoplasty in Girls with Low Anorectal Malformations.

Rectoperineal fistula (RPF) and rectovestibular fistula (RVF) are the most common forms of low anorectal malformations (ARMs) in girls, and lead to difficult stooling, thus demanding early surgical correction. This study's aim was to assess early outcomes associated with one-step ventral and dorsal proctoplasty in RPF/RVF.All female infants who consecutively underwent one-step proctoplasty for RPF/RVF at our institution (2012-2022) were retrospectively included. Reviewed data included: age at procedure, congenital anomalies, fistula location, preoperative symptoms, intraoperative findings, ope

Versatility of contrast enhanced colosonography for pre-operative evaluation of anorectal malformation with fistula.

INTRODUCTION: Anorectal malformations (ARM) with fistulae require anatomic definition of the fistula and distal extent of the rectum relative to the coccyx prior to posterior sagittal anorectoplasty (PSARP). We previously validated contrast enhanced colosonography (CECS) as a safe and accurate modality to characterize ARM. The purpose of this study is to evaluate the application of CECS in various clinical settings and its reliability in the newborn population. METHODS: We performed a single-center retrospective case series of 50 patients with ARM who underwent evaluation with CECS from 2016-

Anorectal Malformation with Rectoperineal Fistula in Females Treated with a Posterior Rectal Advancement Anoplasty: Report of Early Outcomes.

OBJECTIVE: The posterior rectal advancement anoplasty (PRAA) is a novel approach for females with rectoperineal fistulas in which the distal fistula tract lies within the anterior limit of the anal sphincter complex. PRAA eliminates the risk of vaginal injury and perineal body dehiscence while allowing for an appropriately sized and centered anal opening. This study aims to examine the complications and outcomes following PRAA. METHODS: A retrospective, single-institution study was performed of female patients with an anorectal malformation with a rectoperineal fistula between January 2020 an

Comprehensive Management of Obstetric Rectal Buttonhole Tears: Insights From a Case Report and Literature Analysis.

BACKGROUND Obstetric rectal buttonhole tears (ORBT) are rare and underreported complications during vaginal births. They offen go undiagnosed, as they appear with intact perineum or with a low-degree perineal laceration. If missed, they can progress to a rectovaginal fistula. They are not a part of the Sultan classification and there is no consensus on their management. Various repair techniques have been proposed, but the limited evidence and experience make it challenging to standardize an approach. CASE REPORT We present a case of a 2-cm ORBT with intact anal sphincters revealed after a dig

T2-based magnetic resonance genitography in children with complex anorectal malformations: experience at a tertiary pediatric hospital in Latin America.

In children with anorectal malformations (ARMs), it is essential to have a diagnostic imaging method that helps with the evaluation of the internal anatomy. In patients with a persistent cloaca, an ARM variant, in which the measurement of the urethral channel and common channel determines surgical management, there are multiple options for imaging. Magnetic resonance imaging (MRI) is an excellent method for this purpose, from which accurate measurements of channel length can be obtained. Additionally, the use of volumetric/isotropic sequences allows multiplanar reformatting. We present our exp

Treatment techniques for rectovaginal fistulas after low rectal resection for deep endometriosis.

Endometriosis is a benign gynecologic affection that may lead to major surgeries, such as colorectal resections. Rectovaginal fistulas (RVF) are among the possible complications. When they occur, it is necessary to adapt the repair surgery as best as possible to limit their functional consequences. This video shows three different techniques for correcting RVF after rectal resection for endometriosis, with a combination of perineal surgery and laparoscopy: a mucosal flap, a transanal transection and single stapled anastomosis (TTSS) and a pull through. Supplementary file1 (MP4 469658 KB).

Chronic lower abdominal pain in an elderly female patient: a case report about sigmoid colon fistula and pelvic abdominal infection.

An elderly woman patient presented with a history of recurrent right lower abdominal pain accompanied by fever and abnormal vaginal discharge for 36 years worse for two weeks. Conservative medical treatment was ineffective, by laparoscopic exploration combined with intraoperative colonoscopy, the presence of a sigmoid colon fistula and pelvic abdominal infection with foreign bodies were confirmed. It was hypothesized that the occurrence of recurrent right lower abdominal pain and intestinal fistula may be potentially associated with tubal injection sterilization performed 36 years ago.
