Trichophyton rubrum Phenotypic Virulence Factors in Mexican Strains.

(1) Background: T. rubrum is the most important agent in tinea pedis, tinea manuum, tinea cruris, tinea corporis, and even in subcutaneous dermatophytosis. T. rubrum must overcome several obstacles to adhere, grow, and invade the host, for which their virulence factors are important. Previous studies have demonstrated the capability of T. rubrum strains to produce proteases, phospholipases, hemolysins, and elastases. The aim of this work was the genotypic identification of clinical isolates of T. rubrum to subsequently determine production of the main phenotypic virulence factors associated wi

Atypical tinea corporis with Id reaction: A case report and successful treatment with upadacitinib.

Atypical tinea corporis, characterized by clinical manifestations that resemble other skin disorders, can lead to misdiagnosis and delayed treatment. This 67-year-old male had a seven-month history of pruritic skin lesions resembling subcorneal pustular dermatosis. Dermatological exam and fungal culture confirmed Trichophyton rubrum-induced atypical tinea corporis. After oral itraconazole treatment, he developed symmetrical eczematous lesions (Id reaction) on his extremities. Initial treatment was ineffective, so upadacitinib was administered, resulting in significant improvement at eight week

The Importance of Histopathology in the Diagnosis of Annular Lichenoid Dermatitis of Youth: A Case Report.

INTRODUCTION: Annular lichenoid dermatitis of youth (ALDY) is a rare and underreported dermatologic condition. It primarily affects children and adolescents. Clinically, ALDY presents as erythematous annular macules or plaques with raised borders and hypopigmented centers, predominantly affecting the trunk and flexural areas. The etiology remains unknown, and the condition poses a diagnostic challenge due to its resemblance to other annular dermatoses. CASE PRESENTATION: A 5-year-old female presented with asymptomatic annular erythematous lesions on the trunk, abdomen, lumbar region, and flan

[Trichophyton mentagrophytes ITS genotype VIII/Trichophyton indotineae in Germany-revisit after 5 years].

Trichophyton (T.) mentagrophytes ITS genotype VIII/T. indotineae (TMVIII/TINDO) is a new, anthropophilic dermatophyte from the T. mentagrophytes/T. interdigitale complex that has gained increasing importance worldwide in recent years. This pathogen is characterized by frequent terbinafine resistance and a clinical picture of pronounced, inflammatory, and therapy-resistant dermatophytoses that predominantly affect the groin, trunk, extremities, and face. Between 2018 and the end of 2024, all TMVIII/TINDO cases detected in patients from Germany by culture and/or molecular biology were systematic

Diagnosis of Onychomycosis and Real-World Data of Oral Fosravconazole in Japan.

In Japan, two topical drugs and three oral antifungal drugs are prescribed for onychomycosis. Fosravuconazole L-lysine ethanolate (F-RVCZ) is an azole antifungal drug that has been prescribed in Japan since 2018 for the oral treatment of onychomycosis. Herein, we review the real-world evidence of F-RVCZ treatment for cutaneous fungal infection in Japan. The number of prescriptions for F-RVCZ has been increasing. It is more effective than topical drugs and has a favorable safety profile. On the other hand, side effects include elevation of liver enzymes, such as γ-GTP, and renal dysfunction in

Annular Erythema in a Nine-Year-Old Girl: A Rare Type of Psoriasis.

Annular erythema is a nonspecific clinical sign arising from various conditions, including infections, cutaneous lymphomas, connective tissue diseases, and inflammatory dermatoses. Psoriasis vulgaris typically presents as sharply demarcated erythematous plaques with silvery scales; however, annular forms are rare and may complicate diagnosis. We report a nine-year-old girl with an annular erythematous lesion on the anterior chest that gradually expanded over two months, spreading to the extremities and trunk with mild pruritus. Despite suspicion of tinea corporis, repeated fungal tests were ne

Dermatophyte-Selective Imidazole-Thiosemicarbazides: Potent In Vitro Activity Against Trichophyton and Microsporum with No Anti-Candida Effect.

Dermatophytes are highly infectious pathogenic fungi that colonize keratinized tissues like skin, hair, and nails, causing superficial infections such as tinea capitis, onychomycosis, tinea corporis, and tinea pedis in humans and animals. In immunocompromised patients, they may invade deeper tissues and organs, leading to severe or life-threatening conditions if untreated or inadequately managed. While most infections respond to topical antifungals, some require complex treatment and show resistance to standard therapies. Therefore, novel antifungal agents are needed. We investigated the antid

Diagnosis and Management of Tinea Infections.

Tinea infections (often called ringworm) are caused by dermatophyte fungi and classified by the body site involved. Tinea corporis and tinea capitis are most common in prepubertal children, and tinea cruris, tinea pedis, and tinea unguium (most common type of onychomycosis) are more likely in adolescents and adults. Clinical diagnosis without testing may be unreliable because other conditions can resemble tinea infections (eg, tinea corporis can be confused with eczema, and onychomycosis with dystrophic toenails from repeated low-level trauma or psoriasis). Tinea corporis, tinea cruris, and ti

Emerging Antifungal Resistance in Dermatophytosis: A Clinicomycological Study From North India.

Introduction Dermatophytosis is a common superficial fungal infection affecting the skin, hair, and nails and is increasingly being recognized as a public health challenge. In recent years, there has been a noticeable increase in chronic and recurrent cases, possibly due to changing patterns of antifungal resistance, misuse of medications, and host-related factors. This study aimed to assess the demographic and clinical characteristics of patients with dermatophytosis, evaluate treatment history and recurrence, and analyze antifungal resistance patterns. Material and methods A total of 130 pat

Emerging Zoonotic Dermatophytosis: A 10-Year Case Series from a Dermatology Clinic in Japan.

We retrospectively analyzed 99 cases of zoonotic dermatophytosis diagnosed by culture over a 10-year period at a dermatological clinic. Microsporum canis was the most frequently isolated species (89 cases), followed by Trichophyton verrucosum (4 cases), T. mentagrophytes (4 cases), T. benhamiae (1 case), and T. erinacei (1 case). Patients < 15 years of age accounted for 31.3% of the cases. Tinea corporis was the predominant clinical form, with frequent facial and upper-extremity involvement. Topical corticosteroid misuse was noted in 37 patients, and multiple family members were affected in 20

Epidemiology of dermatophytes isolated from superficial dermatological samples taken during 2020-2023 in Zaragoza (Spain).

BACKGROUND: Dermatophytoses are transmissible infections that affect one billion people worldwide and have a significant impact on public health. Dermatophytes distribution evolves geographically and over time. Consequently, local epidemiology should be periodically assessed to control infection. AIMS: To describe the local distribution of dermatophytes and types of dermatophytosis through the analysis of samples from patients with suspected superficial mycoses, and to identify areas of improvement. METHODS: A retrospective epidemiological analysis of mycological culture results from skin, h

Susceptibility to Dermatophytosis in SDR9C7-Nonsyndromic Epidermal Differentiation Disorder: Observation of Cutaneous Inflammation Involving IRF4 and IL-17.

SDR9C7-nonsyndromic epidermal differentiation disorder (nEDD) is a form of ceramide synthesis disorder that is known to confer susceptibility to dermatophytosis. Recurrent widespread tinea corporis developed in a SDR9C7-nEDD patient with homozygosity for the Japanese founder variant (c.826C>T, p.Arg276Cys) during apremilast treatment to improve ectropion. Ceramide analysis of the stratum corneum revealed decreased protein-bound ceramides, indicating skin barrier dysfunction. Based on previous studies of decreased Irf4 mRNA in Sdr9c7-/- mice, IRF4 transcription factor-dependent IL-17 production
