Direct visualization of the entire length of the fallopian tube lumen, from the uterotubal ostium to the fimbria, using a transvaginal approach, has been achieved. Small, flexible hysteroscopes with outside diameters (ODs) ranging from 3.3 to 4.5 mm and operating channel diameters of 1.5 to 1.8 mm were used to pass guide wires, over-the-wire catheters and a falloposcope with an OD of 0.5 mm safely along the fallopian tube lumen. Endometrial sampling is the mainstay of management of the postmenopausal patient with uterine bleeding. Thirty women with postmenopausal bleeding were studied prospectively. Before endometrial sampling, a vaginal probe ultrasonographic examination was performed. Eleven patients demonstrated a thin "pencil line" endometrial echo in which the maximum anteroposterior thickness on the long axis view was less than or equal to 5 mm. Groups of 10 F344/N rats and B6C3F1 mice of each sex were exposed to cobalt sulfate heptahydrate aerosols of 0, 0.3, 1.0, 3.0, 10, or 30 mg/m3, 6 hr per day, 5 days per week, for 13 weeks. Ultrasonograms of 69 patients suspected for endometrial hyperplasia and cancer have been surveyed in retrospect. Transvaginal ultrasonographic findings were compared with clinical data and hysteroscopic findings and verified by morphologic examination of endometrial biopsy samples. The study demonstrated that transvaginal ultrasonography identified 88.4% of these conditions and the clinical examination and hysteroscopy 46.4% and 89.4%, respectively. Pelvic examinations in 1,906 women at the Gynaecological Room, Out-Patient Department, Siriraj Hospital, in June 1990 were studied. The range of ages was between 14 and 85 years, the mean age being 34.5 +/- 11.5 years. One hundred cases of mullerian adenosarcoma of the uterus were encountered in patients 14 to 89 years of age (median, 58 years), who usually had the symptom of abnormal vaginal bleeding. Clinical and pathologic findings in a pregnant 15-year-old with vaginal polyposis disclosed numerous polyps throughout the vagina and portio of the cervix that disappeared spontaneously 6 weeks post partum. These polyps can be confused microscopically with sarcoma botryoides, although they are benign and may regress completely.

A 25-yr-old primigravid patient with large atypical fibroepithelial polyps of the vagina is presented. The tumors necessitated interruption of the pregnancy by cesarean section at 34 wks. After delivery the tumors were excised, and the patient has been symptom-free for 12 months. There was an intercurrent human papilloma virus (HPV) infection in the vaginal epithelium. The nature of rare atypical vaginal fibroepithelial polyps is discussed on the basis of the literature. These lesions are clinically benign and are curable by simple excision, although histologically they resemble sarcomas.

Thirteen cases of female genital tract fibroepithelial polyps containing atypical stromal cells are described, eight arising from the vulva, a site of origin not previously reported. These tumours must be differentiated from sarcoma botryoides, which they resemble both grossly and microscopically. Although fibroepithelial polyps are benign, two of our cases recurred after incomplete excision.

A combination of estrogen and levonorgestrel was continuously delivered to 23 adult rabbits for up to 2 years via a Silastic ring device sutured into the vagina. A case of pseudo-sarcomatous polyps of the vagina in a pregnant woman caused anxiety because of the possible differential diagnoses of sarcoma botyroides of the vagina or mixed mesenchymal tumours of the genital tract.
