Trichophyton mentagrophytes Complex in Onychomycosis: A Retrospective Study of Internal Transcribed Spacer Genotypes and Squalene Epoxidase Gene Mutations.

Increasing attention has been given to the Trichophyton mentagrophytes complex as an agent of dermatophytosis. Despite technical challenges in differentiating lineages within this complex-which necessitates genotyping of the internal transcribed spacer (ITS) region-surveillance efforts are warranted due to the emergence of novel pathogens and rise in antifungal resistance. In North America, although studies have examined the molecular epidemiology of the T. mentagrophytes complex isolated from skin, there is scant information regarding this complex for the onychomycosis population. In this stu

Treating Onychomycosis with Efinaconazole: Considerations for Diverse Patient Groups.

Onychomycosis is a common nail disease that manifests with varying severity and frequency in specific patient populations, warranting a personalized treatment approach. Novel topical antifungals, such as efinaconazole 10% approved for use in North America and Japan, offer a safe treatment option for many of these patients, though real-world use requires special considerations. In this scoping review, a literature search was conducted in October 2025 using PubMed, Embase (Ovid), the Cochrane Central Register of Controlled Trials (CENTRAL), and Web of Science (Core Collection). Terbinafine-Resistant Dermatophytes Isolated in Japan.

An epidemiological study of terbinafine (TBF)-resistant dermatophytes was conducted in 2023 as a follow-up to our 2020 and 2022 surveys. Dermatophytes were isolated in 2023 from the same dermatology clinics in Tokyo, Saitama, Shizuoka, and Kumamoto, Japan, as in the previous studies. In total, 311 isolates (122 Trichophyton interdigitale isolates, 1 Trichophyton indotineae isolate, and 188 Trichophyton rubrum isolates) were obtained from 311 human cases of dermatophytosis (195 tinea pedis cases, 54 tinea unguium cases, 28 tinea corporis cases, 27 tinea cruris cases, and 7 tinea manuum cases).

Diagnosis and Management of Tinea Infections.

Tinea infections (often called ringworm) are caused by dermatophyte fungi and classified by the body site involved. Tinea corporis and tinea capitis are most common in prepubertal children, and tinea cruris, tinea pedis, and tinea unguium (most common type of onychomycosis) are more likely in adolescents and adults. Clinical diagnosis without testing may be unreliable because other conditions can resemble tinea infections (eg, tinea corporis can be confused with eczema, and onychomycosis with dystrophic toenails from repeated low-level trauma or psoriasis). Tinea corporis, tinea cruris, and ti

Epidemiology of dermatophytes isolated from superficial dermatological samples taken during 2020-2023 in Zaragoza (Spain).

BACKGROUND: Dermatophytoses are transmissible infections that affect one billion people worldwide and have a significant impact on public health. Dermatophytes distribution evolves geographically and over time. Consequently, local epidemiology should be periodically assessed to control infection. AIMS: To describe the local distribution of dermatophytes and types of dermatophytosis through the analysis of samples from patients with suspected superficial mycoses, and to identify areas of improvement. METHODS: A retrospective epidemiological analysis of mycological culture results from skin, h

Turkish Bath-Associated Infectious Dermatoses: A Comprehensive Review of Risk Factors, Clinical Manifestations, and Preventive Measures.

People have used the traditional Turkish bath for centuries to cleanse themselves, maintain their health, and treat various diseases. The communal use of these areas may contribute to the development and spread of certain dermatoses due to contact with water and environmental factors. Tinea pedis, verruca, molluscum contagiosum, and Pseudomonas aeruginosa folliculitis are among these infections. Undoubtedly, pseudomonal infections are the most well-known waterborne infectious diseases. Pseudomonas aeruginosa folliculitis (hot tub folliculitis), Pseudomonas hot-foot syndrome, toe web infections

Epidermophyton floccosum, an etiological agent of tinea pedis and tinea unguium: about two cases.

Dermatophytia is an infection caused by keratinophilic filamentous fungi. The distribution of dermatophytes varies by country and geographic area. Epidermophyton (E) floccosum has experienced a downward trend in recent years. It is an anthropophilic dermatophyte that causes mainly skin infections in humans. We report two observations of a tinea pedis and a tinea unguium due to E. floccosum. A 70-year-old diabetic patient was admitted to dermatology to manage leg erysipelas. The patient had scaly plantar keratoderma and intertrigo of the toe-web spaces. Mycological sampling showed at direct exa

Treatment Options for Onychomycosis: Efficacy, Side Effects, Adherence, Financial Considerations, and Ethics.

BACKGROUND: Onychomycosis is a fungal infection of the nail unit that affects a large patient population globally. Onychomycosis, or tinea unguium, has a benign chronic clinical course; however, it can cause complications in certain patient populations suffering from diabetes and peripheral vascular disease. As nails grow slowly, onychomycosis requires a lengthy treatment plan, and choosing appropriate treatments can be challenging. There are a variety of treatment modalities available for patients including topical, oral, laser, light therapy, procedures such as avulsion and matrixectomy, sup

Pediatric dermatophyte onychomycosis: a review.

Recent studies have reported an increase in pediatric onychomycosis prevalence worldwide, suggesting that this population may be increasingly affected by the infection. A summary of the epidemiological impact, antifungal treatment options, special considerations for at-risk subpopulations, and methods to prevent infection and recurrence are discussed. A systematic review of available epidemiological studies found the worldwide prevalence of culture-confirmed pediatric toenail onychomycosis to be 0.33%, with no significant increases in prevalence over time. A systematic review of studies invest

Extended Use of Topical Efinaconazole Remains Safe and Can Provide Continuing Benefits for Dermatophyte Toenail Onychomycosis.

INTRODUCTION: Efinaconazole 10% topical solution labeling for onychomycosis describes phase III trials of 12 months of treatment; the slow growth of onychomycotic nails suggests a longer treatment period may increase efficacy. We present here the first evaluation of extended use of efinaconazole 10% topical solution for up to 24 months. MATERIALS AND METHODS: Enrolled patients (n = 101) had one target great toenail with mild to moderate distal lateral subungual onychomycosis and applied efinaconazole 10% topical solution to all affected toenails once daily for 18 months (EFN18) or 24 months (

Epidemiology of Onychomycosis in the United States Characterized Using Molecular Methods, 2015-2024.

Onychomycosis is a recalcitrant fungal infection of the nail unit that can lead to secondary infections and foot complications. Accurate pathogen identification by confirmatory testing is recommended to improve treatment outcomes. In this study, we reviewed the records of 710,541 patients whose nail specimens were sent to a single molecular diagnostic laboratory between 2015 and 2024. PCR testing revealed a more comprehensive spectrum of pathogens than previously reported, which was corroborated by the demonstration of fungal invasion on histopathology. Consistent with our current understandin
