Two feet-one hand syndrome is also defined as bilateral plantar tinea pedis with coexistent unilateral tinea manuum. Toenails and fingernails may also be affected and the dermatophyte Trichophyton rubrum is the usual cause. When there is nail involvement, especially of the toenails, treatment with an oral antifungal agent should be considered because topical therapy alone is usually not effective. However, relapses are common. With the advent of new, more effective antifungal drugs such as itraconazole, terbinafine, and fluconazole, it is hoped that this troublesome and recalcitrant disorder m

BACKGROUND: Onychocola canadensis is a nondermatophyte mold associated with onychomycosis particularly in temperate climates (eg, Canada, New Zealand, and France). The slow growth rate of O canadensis and lack of resemblance to any other known nail-infecting fungus may have delayed its discovery. We are aware of 23 mycologically confirmed cases of O canadensis in the literature.
OBJECTIVE: We describe 10 previously unreported Canadian patients, specimens from whom grew O canadensis. We also review the literature on infections associated with this organism.
METHODS: Cases of O canadensis onyc

A multicentre, randomized, double-blind, parallel-group trial was conducted to compare the efficacy and safety of two dosages of itraconazole in the treatment of tinea pedis or manuum. Of the 135 patients recruited, 66 received itraconazole 400 mg daily for 1 week and 69 received itraconazole 100 mg daily for 4 weeks. In the evaluable patients observed-case analysis, mycological cure was seen in 63% of patients in the 400 mg/1 week group and in 75% in the 100 mg/4 weeks group at the end of a 6-week follow-up period (not significantly equivalent). In the evaluable patients worst-case analysis a

The estimated lifetime risk of acquiring a dermatophyte infection is between 10 and 20 percent. Recognition and appropriate treatment of these infections reduces both morbidity and discomfort and lessens the possibility of transmission. Dermatophyte infections are classified according to the affected body site, such as tinea capitis (scalp), tinea barbae (beard area), tinea corporis (skin other than bearded area, scalp, groin, hands or feet), tinea cruris (groin, perineum and perineal areas), tinea pedis (feet), tinea manuum (hands) and tinea unguium (nails). To determine the best treatment ap

BACKGROUND: The two feet-one hand syndrome is not uncommon; however, there have only been a few reports on this condition. This study was undertaken to obtain a better understanding of the epidemiology of the two feet-one hand syndrome.
METHODS: A retrospective chart review was conducted of all the patients seen in our practices over the past 15 years with the diagnosis of two feet-one hand syndrome.
RESULTS: A total of 80 patients with mycologically confirmed disease were identified (men, 72 (90%); women, 8 (10%); 77 (96%) Caucasian; 3 (4%) African-American; age (mean +/- standard error (SE

BACKGROUND: This review summarizes the different species of dermatophytes isolates in our laboratory between 1991 and 1995. We describe the clinical forms and establish the distribution over this period of time.
METHODS: Retrospective survey of samples from outpatients of the Dermatology Service in Miguel Servet Hospital where mycologic cultures are required. The extraction of samples is made by scrapes with a carpet or scalpel and they are cultured on Saboureaud agar with chloramphenicol and dermatophytes agar for 3 weeks. All plates were incubated at 28 degrees C. The identification of isol

OBJECTIVE: To describe fungi and yeasts isolated from skin and nail infections in the Netherlands.
DESIGN: Retrospective.
SETTING: Centraalbureau voor Schimmelcultures (CBS), Baarn, the Netherlands.
METHOD: Results of mycological investigation of skin and nail samples in the period 1992-1993 were analysed. After a clinical diagnosis of mycosis, performed by dermatologists and general practitioners, material was sent to the CBS for mycological research.
RESULTS: The clinical diagnosis of onychomycosis was rather accurate, especially if made performed by dermatologists. Mycoses of the skin w

A total of 12,150 cases of suspected dermatophytoses in different areas of Iran were studied between 1986 and 1991. The age groups most commonly infected were 1-9 and 20-29 years. Clinical diagnosis was confirmed in only 9345 cases by laboratory examination. Of these positive samples, 1633 cases were found to be positive by direct microscopic observation only, 429 by culture only and 7283 by both techniques. Scalp ringworm of children was the most common type of tinea. But according to the rate of infection, tinea corporis, tinea pedis, tinea cruris, tinea manuum, tinea unguium and tinea barba

Superficial fungal infections are common. Most diagnoses of fungal infections of the skin can be made by physical examination, assisted by the use of a Wood's lamp, skin scrapings for microscopic examination, and fungal cultures. Dermatophyte infections are common at all ages, in both sexes, and they have a worldwide distribution. These infections include tinea capitis, tinea cruris, tinea pedis, tinea corporis, tinea manuum and tinea barbae. Tinea versicolor, caused by Malassezia furfur, and candidal infections are also common. Treatment modalities include oral and topical agents. Good person

In the period between April 1984 and April 1988 a total of 4294 clinically suspected cases of dermatomycoses were examined for causative fungi. Of these cases 680 were suspected as tinea pedis and tinea manuum. These cases belonged to 21 different nationalities. Both sexes were represented and 649 cases (95.4%) were adults while 31 (4.6%) were prepubertals. Clinical diagnosis was confirmed by direct microscopy in 505 cases or 74.4% (485 adults and 20 prepubertal children). Out of these, 504 cases were positive on culture and yielded 516 isolates. Candida species and other yeasts were responsib

Epidemiological an mycological data of a 30 years long study on 366 cases of tinea manuum. A sudden raise on the last five years (249 cases). The mycosis is caused by T. rubrum (86%), mostly affects men (70%) among 11 and 40 years of age, on one hand (61%) and seldom both (21%). The course last 2 years in average. The KOH examination confirms the diagnosis (93.5%).

A new orally active antifungal agent, terbinafine, was used in the treatment of tinea pedis ('dry type' or moccasin type) and tinea manuum. Fifty-three adults over the age of 16 years with fungal infections of the feet and/or hands were treated with either oral terbinafine, 250 mg, or placebo, once daily for 2 weeks. The diagnosis of fungal infection was confirmed by examination of skin scrapings by microscopy and culture. Of these, 28 patients were evaluable for efficacy. At 8 weeks, 12 out of 14 (86%) patients who received terbinafine were mycologically negative (microscopy and culture) comp

A total of 4,294 clinically suspected cases of dermatomycoses belonging to 26 different nationalities were examined between April 1984 and April 1988. Fungi were demonstrated in routine potassium hydroxide/dimethyl sulfoxide mount in 3,814 cases (88.8%) and the etiology was determined by culture in 2,458 cases (57.2%). Tinea versicolor was the predominant fungal infection (30.9% of all infections). Onychomycosis and paronychia ranked second in prevalence (16.8%). Candidal onychomycosis was the most common type of infection. Scalp ringworm among children ranked third (15.3%), Microsporum canis

Hendersonula toruloidea and Scytalidium hyalinum are opportunistic organisms that can produce tinea pedis, tinea manuum, and tinea unguium. These infections, which clinically mimic those caused by dermatophytes but are caused by nondermatophyte agents, are correctly called dermatomycoses. When examined microscopically with potassium hydroxide, the agents of these disorders resemble those of dermatophytosis, showing narrow, septate, branching hyphae. It, therefore, becomes important to culture samples taken from sites with these tineas on cycloheximide-free agars even if they are potassium hydr

Approximately 200 patients with clinically and mycologically diagnosed dermatophytoses were treated with itraconazole. The drug was administered at a dose of 100 mg with the main meal. Therapy duration was as follows: tinea corporis and tinea cruris, 14 days; tinea pedis and tinea manuum, 28 days; tinea unguium of hands, three months, and feet, six months; tinea capitis, 30 days. Clinical and mycological control tests (eg, direct microscopy and culture) were performed prior to treatment, at the end of treatment and two and four weeks after the end of treatment. Blood chemistry parameters were

Itraconazole is an effective medication against the most common dermatophytoses. It has been shown to be more active than griseofulvin and ketoconazole. Ease of use, affinity for keratinized tissues, lack of toxicity, continued activity after discontinuation, and the possibility of using fixed schedules are advantages of itraconazole. The fixed schedules indicated by pharmacokinetics and clinical studies are one 100 mg capsule daily for 15 days in cases of tinea corporis and tinea cruris and the same dosage for 30 days in cases of tinea pedis and tinea manuum. These fixed treatments have some

Over 4000 cases of fungal nail, sole and palm infection recorded during a three-year study period were investigated to determine the prevalence and significance of etiologic agents other than dermatophytes and yeasts. Littman's oxgall agar, which permitted isolation of cycloheximide-sensitive species but restricted overgrowth by fast growing contaminants, was used to promote isolation of these disease agents. Non-dermatophytic filamentous fungi made up 2.3% of the confirmed etiologic agents obtained overall, and constituted 3.3% of the agents obtained from nail infections. The most common spec
