Pregnancy outcomes among women with and without HIV infections who underwent excisional treatment for high-grade cervical intraepithelial neoplasia: a retrospective cohort study in low-resource settings.

OBJECTIVE: The standard treatment for high-grade squamous intraepithelial lesions is excisional involving the uterine cervix, while surveillance is an acceptable approach for low-grade squamous intraepithelial lesions. There is controversy about excisional treatment on pregnancy outcomes. The objective of this study was to determine pregnancy outcomes in women living with and without HIV who underwent excisional treatment for high-grade cervical intraepithelial lesions. DESIGN: This retrospective cohort study compared the pregnancy outcomes of women with and without HIV who were or were not t

Loop electrosurgical excision procedure and perinatal outcomes post-human papillomavirus cervical screening: A longitudinal cohort study.

OBJECTIVE: This study examines the associations between the loop electrosurgical excision procedure (LEEP) and the risk of adverse perinatal outcomes among women who have undergone primary human papillomavirus (HPV) screening. METHODS: In this longitudinal cohort study, women who received HPV screening and were followed up in a randomized trial in British Columbia (BC), Canada, were linked to a province-wide perinatal registry; those with a singleton live birth after screening (index birth) were included (N = 1765). Adjusted odds ratios (aORs) were calculated to compare the risk of preterm bi

HPV testing with 16/18 genotyping for risk stratification among women with normal cytology: a multicenter prospective cohort study from China.

UNLABELLED: To evaluate the clinical performance of Hybribio's 14-type HPV real-time PCR with 16/18 genotyping (HBRT-H14) and its risk stratification utility among women with normal cytology (NILM). From 2017 to 2020, a multicenter cohort enrolled 8,401 women aged 30-64 years with NILM cytology. Baseline HPV testing used HBRT-H14. Women positive for HPV 16/18 were referred for colposcopy; follow-up was annual for 3 years or until the detection of cervical intraepithelial neoplasia grade 2 or worse (CIN2+). Analyses included 6,679 women who completed follow-up. Overall HPV positivity was 11.4%,

Head-to-head comparison of the MyGene human papillomavirus-sexually transmitted infection assay versus the Cobas 4800 human papillomavirus test for detecting high-grade cervical intraepithelial neoplasia.

OBJECTIVE: Novel human papillomavirus (HPV) tests must undergo thorough validation before implementation in the cervical cancer screening population. The MyGene HPV sexually transmitted infection (STI) assay (MHSA), an emerging test based on emerging next-generation sequencing, presents as a promising tool for enhancing detection capabilities in screening settings. This study aims to conduct a comparative analysis of the clinical performance of two assays (Cobas 4800 vs. MHSA) in detecting 14 high-risk (GR)-HPV genotypes for the identification of high-grade cervical intraepithelial neoplasia (

Cell cycle gene expression and microbial profiles associated with HPV related cervical neoplasia and cervical microbiome shifts.

Background and objectives The improper regulation of the cell cycle is a significant risk factor for cervical cancer (CC), which is a serious health concern for women worldwide. The interaction between viral pathogens and microbiota with host cells could be vital in this dysregulation. The research aimed to explore changes in the transcription levels of CDK2 and CCNE1, and how these changes relate to the HPV genotypes in women with cervical intraepithelial neoplasia (CIN) compared with non-infected healthy women. Additionally, microbiome and gene expression analyses were conducted in a subgrou

Worldwide Evaluation of Cervical Cancer Self-Sampling Devices.

Nearly 90% of cervical cancer and related fatalities occur in low-income and middle-income countries and low-resourced areas within developed countries. In the 1980s, self-sampling was introduced to reach this vulnerable population and those with behavioral aversion to clinician-based screening. Currently, dozens of self-sampling devices have been studied for cervical cancer screening. Here, we consolidated data from an extensive peer-reviewed literature search to summarize the prevalence, accuracy, and acceptance rates of cervical cancer screening self-sampling devices worldwide. We focused,

Genotype-Specific HPV E6/E7 mRNA Triage Improves Risk Stratification and Reduces Referrals in DNA-Positive ASC-US/LSIL: A Real-World Cohort from Nordland, Norway.

HPV DNA-positive women with ASC-US/LSIL cytology represent a heterogeneous risk group in cervical screening and require efficient triage. We evaluated a genotype-specific 7-type HPV E6/E7 mRNA assay (PreTect HPV-Proofer 7; types 16/18/31/33/45/52/58) in a real-world quality-assurance cohort at Nordland Hospital (Bodø, Norway). Among HPV-positive women with ASC-US/LSIL reflex cytology, 225 had sufficient residual liquid-based cytology material and a valid mRNA result; 175 had complete follow-up (2022-2025) and were included. Overall, 44.6% (78/175) were mRNA-positive (ASC-US 45.2%; LSIL 43.3%).

Prevalence of high-risk human papillomavirus among women with different HPV-vaccination status in Sichuan, China.

While HPV vaccination has significantly reduced HPV 16/18 prevalence in other countries, HPV vaccination was not approved in China until 2016, most of those vaccinated are over the age of 20, the efficacy remains unknown. In this cross-sectional study, we analyzed cervical specimens from 6971 women (20-40 years) undergoing routine gynecological check-ups (2022-2024). The Overall HR-HPV prevalence was 11.12% (775/6971). Vaccination reduced HPV 16/18 prevalence by 60.5% (RR = 0.395, 95% CI: 0.280-0.558) with the 2/4-valent vaccine and 59.8% (RR = 0.402, 95% CI: 0.277-0.583) with the 9-valent vac

Risk Stratification for Underlying Cervical Intraepithelial Neoplasia Grade (CIN)3+ in Reproductive-Age Women With Biopsy-Confirmed CIN2: Implications for Fertility-Preserving Management.

IntroductionManaging biopsy-confirmed cervical intraepithelial neoplasia grade 2 (CIN2) in women of reproductive age poses clinical challenges. Immediate treatment with large-loop excision of the transformation zone (LLETZ) is associated with a substantial risk of adverse obstetric outcomes. This study aimed to identify the risk factors for predicting CIN3+ lesions in reproductive-aged women with biopsy-diagnosed CIN2, to inform personalized management strategies that are particularly relevant to China's evolving fertility policies.MethodsThis retrospective cohort study analyzed the data from

The Burden of HPV35 in African Cervical Pathologies: Prevalence, Contributing Factors, and Vaccine Prospects.

Human papillomavirus (HPV35) is globally associated with only 2% of invasive cervical cancers (ICC) but demonstrates a disproportionately higher prevalence in sub-Saharan Africa, reaching up to 10% in previous reports. In this review, we provide updated data from multiple African countries, revealing high HPV35 prevalence rates in women with precancerous and ICC lesions. Among women with ICC, the highest prevalence of HPV35 was observed in Mozambique (30% and 19%), Kenya (26% and 22%), South Africa (17%), Burkina Faso (13.7%), Zimbabwe (11%), and Tanzania (11.2%). Similarly, in women with prec

Randomized clinical trial protocol: Acceptability and feasibility of combination treatment for cervical precancer among South African women living with HIV (ACT 2).

BACKGROUND: Global efforts to eliminate cervical cancer currently focus on expanding access to human papillomavirus (HPV) vaccination and cervical screening. However, these efforts lack equal investment in the treatment of precancerous cervical intraepithelial neoplasia grade 2/3 (CIN2/3), particularly among women living with human immunodeficiency virus (HIV; WLWH). This gap leaves a generation of WLWH at high risk of persistent/recurrent CIN2/3 and progression to cervical cancer, as standard surgical treatments are less effective in this population. METHODS AND DESIGN: We conducted a random

HPV genotype prevalence in low- and high-vaccine coverage, USA populations: Data from real-world cervical cancer screening using the Onclarity HPV assay.

OBJECTIVE: HPV vaccination is an effective strategy for reducing both HPV infection and cervical disease. In the USA, vaccination with the 4-valent HPV vaccine (Merck) has led to a significant reduction in target genotypes HPV16 and 18. However, recent work suggests that vaccination can result in prevalence changes of HPV genotypes not targeted by vaccination (non-vaccine target types). Here we compared genotype prevalence results and HPV-associated cervical intraepithelial neoplasia grade 2 or worse (≥CIN2) results from low- and high-vaccine coverage USA populations. METHODS: Low-coverage va

Recurrent cytologic inflammation on cervical smear and risk of low-grade cervical abnormalities in cytology-based screening: A 25-year multicenter cohort study from HPV-resource-limited settings.

BACKGROUND: Cytologic inflammation is a common finding in cervical screening, yet its clinical significance remains uncertain, particularly in settings where human papillomavirus (HPV) testing is not routinely available. We examined whether recurrent inflammatory findings on Papanicolaou (Pap) smears predict subsequent cervical abnormalities in a cytology-based screening context. METHODS: We conducted a 25-year multicenter hospital-based cohort study including 21,574 women who underwent at least three consecutive Pap smears between 1997 and 2021 at three teaching hospitals in Taiwan. Clinical evidence on a Coriolus versicolor-based vaginal gel for HPV-related cervical disease: a narrative review.

BACKGROUND AND OBJECTIVE: Human papillomavirus (HPV) infection is the most common sexually transmitted disease worldwide. Its persistence may lead to cervical dysplasia and, eventually, cervical cancer. Currently, there are no approved treatments specifically targeting HPV infection or low-grade squamous intraepithelial lesions (LSILs). A multicomponent vaginal gel containing Trametes versicolor extract, prebiotics, moisturizing agents, and anti-inflammatory coadjuvants has been developed to support natural HPV clearance and promote lesion healing or regression. The aim of this review is to su

Accuracy of human papillomavirus testing in detecting persistent or recurrent disease at follow-up after conization.

INTRODUCTION: Currently, the Test of Cure after conization consists of two elements, namely human papillomavirus (HPV) testing and cytology. Given the high sensitivity of HPV testing and its modest resource requirements, we will explore its potential as a stand-alone test for identifying persistent/recurrent cervical intraepithelial neoplasia (CIN) during follow-up after conization, including women at increased risk, such as those with involved resection margins. MATERIAL AND METHODS: This retrospective study examined 473 women who underwent conization in Funen, Denmark, between January 1, 20
