Overcoming oral medication allergy and cervical stenosis in an anovulatory in vitro fertilization patient: a case report.

BACKGROUND: We present a rare and challenging case showing how an allergy to oral medications and cervical stenosis can complicate fertility treatment. The novelty lies in the exclusive use of injectable medications for both in vitro fertilization and frozen embryo transfer, the use of non-oral medications for luteal support, the off-label gonadotropin regimen for frozen embryo transfer stimulation, and the use of cervical dilatation to allow an atraumatic embryo transfer. CASE PRESENTATION: A 32-year-old white woman with long-term anovulatory infertility had a rare allergy to oral agents com

First documented case of cervical stenosis resulting in pyometra and fallopian tube torsion.

INTRODUCTION: Cervical stenosis (CS) is a condition characterized by the narrowing of the cervical canal, which can lead to complications such as pyometra and fallopian tube torsion (FTT). The interplay of these conditions is rarely documented in the literature. CASE PRESENTATION: A 60-year-old Ethiopian woman, with a history of recurrent genitourinary infections and 16 years post-menopausal, presented with sudden onset right-sided lower abdominal pain, nausea, and foul-smelling vaginal discharge. Physical examination revealed a mass consistent with a 14-week size uterus and tachycardia. Ultr

Risk of cervical stenosis after cervical excision in postmenopausal patients.

OBJECTIVE: Cervical excision may cause cervical stenosis, leading to suboptimal follow-up of dysplasia and delayed diagnosis of cervical and endometrial pathology. This study aimed to quantify the risk of stenosis development after electrosurgical cervical excision in postmenopausal patients. METHODS: Retrospective cohort study based on data collection from electronic medical records and the Danish National Pathology Data Bank. Patients aged ≥45 years who underwent electrosurgical cervical excision in the Gynecological Department, at Randers Regional Hospital from1st of January 2012 to 31st D

Laparoscopic transfundal approach to cervical dilation: a novel treatment for cervical stenosis and cervical factor infertility.

OBJECTIVE: To demonstrate a novel technique used to restore cervical patency in a patient with severe iatrogenic cervical stenosis. DESIGN: Surgical video case report. SETTING: A single academic institution. PATIENT(S): We highlight the case of a 35-year-old nulliparous woman with a history of primary infertility. Her past medical history was significant for focal, invasive, well-differentiated squamous cell carcinoma of the cervix, for which she underwent a loop electrosurgical excision procedure. During her infertility assessment, she was found to have an extremely stenotic cervix that wa

Diagnostic cervical excision in patients with HPV positivity, cytological abnormalities- and preoperative cervical stenosis.

AIM: There is currently no protocol for classifying patients with HPV persistence and preoperative stenosis of the cervical canal. This has a significant impact on cytology results, colposcopy results and the possibility of obtaining reliable cervical histology outcomes. Our analysis clearly shows that colposcopy and cytology underestimate the histological results in patients with limited visibility due to the presence of a type 3 transformation zone (TZ). Our analysis revealed a significant discrepancy between the colposcopy and cytology results and the histological outcomes. Insufficient col

Uterine cervical stenosis: from classification to advances in management. Overcoming the obstacles to access the uterine cavity.

BACKGROUND: To date hysteroscopy is the gold standard technique for the evaluation and management of intrauterine pathologies. The cervical canal represents the access route to the uterine cavity. The presence of cervical stenosis often makes entry into the uterine cavity difficult and occasionally impossible. Cervical stenosis has a multifactorial etiology. It is the result of adhesion processes that can lead to the narrowing or total obliteration of the cervical canal. PURPOSE: In this review, we summarize the scientific evidence about cervical stenosis, aiming to identify the best strategy

A randomized clinical trial of a new anti-cervical stenosis device after conization by loop electrosurgical excision.

BACKGROUND: The complications inherent to conization include vaginal bleeding, cervical stenosis, amenorrhea, dysmenorrhea, and deep dyspareunia. Cervical stenosis is the most important complication due to the clinical repercussions. Studies show rates of cervical stenosis ranging from 1.3 to 19% after the Loop Electrosurgical Excision Procedure (LEEP). OBJECTIVE: Our primary outcome was to compare the role of a new endocervical device to prevent cervical stenosis after LEEP in patients with high-grade squamous intraepithelial lesions (HSILs). METHODS: A randomized clinical trial was perform

Collagen Vitrigel Membrane-Coated Nylon Line Prevents Stenosis After Conization of the Cervix Uteri.

Cervical stenosis is a postoperative complication of conization for uterine cervical malignancy, but a standard method of preventing this complication has yet to be established. Collagen vitrigel is a collagen-based biomaterial that has antifibrotic and epithelization promoting actions. We evaluated the antistenotic effect of an indwelling collagen vitrigel membrane-coated nylon line (CVNL) after cervical conization in rabbits. In one group of rabbits, a CVNL was placed in the cervical canal after conization. In another group, a nylon line without a collagen coating was placed in the cervical

Successful trachelectomy and re-vaginoplasty for cervico-vaginal stenosis following unsuccessful uterovaginal anastomosis and vaginoplasty in congenital cervical and vaginal aplasia: description of technique and a case report.

We describe a successful surgical technique of abdominal trachelectomy and re-vaginoplasty for cervico-vaginal stenosis following unsuccessful uterovaginal anastomosis and vaginoplasty in a patient with congenital cervical and vaginal aplasia. After the surgical procedure, cervico-vaginal stenosis was resolved and periodic menstruation without dysmenorrhoea resumed. While long-term follow-up is essential to ensure successful pregnancy and delivery, we conclude that this novel surgical procedure is a promising alternative for improvement of the quality of life and normal sexual function, and fo

The Invisible External Cervical Os. Tips and Tricks to Overcome this Challenge during In-Office Hysteroscopy.

OBJECTIVE: Hysteroscopy is considered the gold standard technique for the diagnosis and management of intrauterine pathology allowing to "see and treat" patients in 1 session if desired [1-3]. Pain and the inability to enter the uterine cavity are the most common limitations of hysteroscopy, especially when performed in an office setting [4-7]. Cervical stenosis is a common hysteroscopic finding frequently encountered in postmenopausal women, especially in patients with a history of cervical procedures such as cone biopsy [8]. It represents a challenge even for the most expert hysteroscopist.
