Risk factors for preterm birth and prolonged pregnancy following cervical cerclage in women with cervical insufficiency: a retrospective analysis and predictive model.

BACKGROUND: Cervical insufficiency is one of the major risk factors for preterm birth. The efficacy of cervical cerclage (CC) in improving pregnancy outcomes remains controversial. This study aimed to compare obstetric outcomes between patients with cervical insufficiency who underwent CC and those who did not to determine the optimal cutoff point for preterm birth on the basis of fetal survival outcomes and to identify risk factors influencing preterm birth. Furthermore, we sought to analyze the determinants of pregnancy prolongation after CC and to develop predictive models using pregnancy p

Women's motivations for and experiences with transabdominal cerclage: A qualitative study.

INTRODUCTION: Transabdominal cerclage (TAC) is used to prevent pregnancy loss and preterm birth in women with previous failed transvaginal cerclage or insufficient cervical tissue. TAC requires laparoscopic or open abdominal surgery, cesarean birth, and the potential long-term effect are currently unknown. While the procedural aspects, indications, and pregnancy outcomes associated with TAC are subject to ongoing research, little is known about how women experience the decision to undergo a TAC and the pregnancy that follows. MATERIAL AND METHODS: We conducted a qualitative interview study wi

Offspring long-term infectious morbidity following pregnancies with cervical cerclage.

INTRODUCTION: Cervical cerclage is an acceptable procedure in women with cervical insufficiency and is known to be effective in the prevention of preterm delivery. However, limited data exist regarding long-term health outcomes among offspring exposed to cerclage during pregnancy. Since the presence of a foreign body during pregnancy may change the vaginal microbiome, we aimed to study whether a cervical cerclage is associated with long-term infectious morbidity of the offspring. STUDY DESIGN: A retrospective population-based cohort study was performed at a tertiary medical center, including

Expert Consensus on the Diagnosis and Management of Cervical Insufficiency in China (2026 Revision).

UNLABELLED: Cervical incompetence occurs when the cervix fails to maintain its normal morphology and function during second-trimester, leading to recurrent second-trimester miscarriages or preterm births. To standardize and guide the clinical diagnosis and treatment of cervical insufficiency in China, the Perinatal Medicine Branch of the Chinese Medical Association and the Obstetrics Group of the Chinese Society of Obstetrics and Gynecology developed this consensus by referencing related research evidence and so on. This consensus provides evidence-based analysis and recommendations on key cli

Does suture number matter in transvaginal cervical cerclage? A propensity score-weighted cohort study.

PURPOSE: To evaluate whether the number of sutures used during transvaginal cervical cerclage influences obstetric and neonatal outcomes when surgical technique, suture material, and perioperative management are standardized. METHODS: This retrospective cohort study included 125 women who underwent transvaginal cervical cerclage using identical monofilament suture material. Cerclage was performed with either a single suture (n = 23) or a double suture (n = 102) in this non-randomized observational cohort. The primary outcome was term delivery (≥ 37 weeks' gestation). Secondary outcomes includ

Optimal management of pregnancies presenting with membrane prolapse.

INTRODUCTION: Membrane prolapse represents a severe manifestation of cervical insufficiency, occurring in 0.3-1.0 % of pregnancies and posing significant risks for extreme prematurity and perinatal mortality. Despite decades of clinical experience, optimal management remains controversial. CONTENT: This narrative review synthesizes current evidence on pathophysiology, diagnosis, and management of membrane prolapse. We examine biomechanical and inflammatory mechanisms, diagnostic criteria, including cervical length assessment and amniocentesis, as well as therapeutic strategies, such as emerge

Dynamic Systemic Inflammatory Markers for Predicting Perinatal Outcomes After Emergency Cerclage in Women With Membrane Prolapse.

AIM: Although preoperative systemic inflammatory markers have been linked to cerclage outcomes, the prognostic value of postoperative measurements and their dynamic changes remains unclear. We aimed to evaluate postoperative inflammatory markers and their dynamic changes as predictors of outcomes in women with prolapsed membranes undergoing emergency cerclage. METHODS: This single-center retrospective cohort study included 34 singleton pregnancies with prolapsed membranes treated with emergency McDonald cerclage. Neutrophil-to-lymphocyte ratio (NLR), systemic immune-inflammation index (SII),

Cervical insufficiency revisited: a proposal for a new treatment-aligned definition.

Cervical insufficiency [CI] has traditionally been defined as painless cervical dilation resulting in recurrent second-trimester pregnancy loss in the absence of labor or placental pathology. Definitions from the American College of Obstetricians and Gynecologists and the Royal College of Obstetricians and Gynecologists emphasize historical clinical presentation. However, contemporary obstetrics relies increasingly on ultrasound-based cervical length surveillance and evidence-based cerclage indications rather than retrospective diagnosis. The objective of the review was to propose a definition

Dynamics of immune-checkpoint regulators and inflammatory cytokines and chemokines associated with preterm birth before and after cervical cerclage.

BACKGROUND: Cervical insufficiency (CI), characterized by painless cervical dilation during the second trimester, is a major cause of preterm birth. However, the local microenvironment associated with CI, particularly the roles of immune-checkpoint regulators (ICRs) and inflammatory cytokines and chemokines (ICKs), remains poorly understood. This study aimed to investigate the changes in ICRs and ICKs before and after cervical cerclage, as well as to identify predictors of preterm birth in patients with CI. METHODS: A total of 64 women with CI comprised the study group, and 128 endocervical s

The value of cervical length changes for the prediction of preterm birth with normal mid-trimester cervical length; a prospective longitudinal study.

INTRODUCTION: Spontaneous preterm birth (sPTB) remains a leading cause of neonatal morbidity and mortality. Mid-trimester transvaginal cervical length (CL) measurement is an established screening tool, but it is unclear whether longitudinal change in CL between the 1st and 2nd trimesters (ΔCx) adds predictive value in women with a normal mid-trimester cervix. METHODS: This prospective longitudinal cohort study included singleton pregnancies with 1st and 2nd trimester transvaginal CL measurements. Women with a normal mid-trimester cervix and no antenatal progesterone exposure comprised the pri

Impact of real-world confounders on the accuracy of an AI model to support read out of skin prick automated test results.

γ-Herpesviruses (γ-HVs) are notable for their ability to establish latent infections of lymphoid cells(1). The narrow host range of human γ-HVs, such as EBV and KSHV, has severely hindered detailed pathogenic studies. Murine γ-herpesvirus 68 (γHV68) shares extensive genetic and biological similarities with human γ-HVs and is a natural pathogen of murid rodents(2). As such, evaluation of γHV68 infection of mice inbred strains at different stages of viral infection provides an important model for understanding viral lifecycle and pathogenesis during γ-HVs infection. Upon intranasal inoculation,

Uterine surgery before pregnancy is associated with later preterm birth and stillbirth: a population-based record linkage study.

BACKGROUND: In high-income countries, uterine surgery is performed for the management of failed or unplanned pregnancies, for excision of polyps and fibroids, for correction of congenital anomalies, for investigation of subfertility, and to facilitate the use of an intrauterine contraceptive device. Many women undergo at least 1 procedure before their first birth. OBJECTIVE: The study hypothesis was that any uterine surgery requiring dilation of the cervix could weaken the cervix and lead to a higher risk of preterm birth in a later pregnancy, that uterine surgery could leave endometrial scar

Clinical value of magnetic resonance imaging in the early diagnosis of cervical insufficiency in pregnancy.

BACKGROUND: Cervical insufficiency (CI) is a leading cause of mid-to-late trimester pregnancy loss, making early and accurate diagnosis critical. While clinical diagnosis primarily relies on transvaginal ultrasound (TVUS), its limitations in visualizing cervical microstructure are recognized. High-resolution magnetic resonance imaging (MRI) offers exceptional soft-tissue contrast, potentially enabling a detailed assessment. This study aimed to explore the descriptive imaging features of MRI in suspected CI. MATERIALS AND METHODS: This retrospective study enrolled 77 pregnant women with clinic

Relative position of cerclage - an ultrasound marker for prediction of preterm birth.

BACKGROUND: A transvaginal cerclage (TVC) identified high in the cervix is associated with prolongation of pregnancy. However, the relative position of the TVC (length above the cerclage/total cervical length) may be a better predictor of cerclage success as it is proportional of total cervical length. This study evaluates the relative position of TVC and its association with spontaneous preterm birth, comparing its predictive value against other post-cerclage ultrasound parameters. METHODS: A retrospective cohort study was conducted of singleton pregnancies with TVC. Demographic and ultrasoun
