Safety and oncologic outcomes of total laparoscopic versus abdominal hysterectomy following diagnostic conization for adenocarcinoma in situ and stage IA1 cervical cancer: a multicenter retrospective study.

BACKGROUND: While simple hysterectomy is the standard treatment for adenocarcinoma in situ (AIS) and stage IA1 cervical cancer, minimally invasive surgery has been increasingly adopted. However, evidence on the safety and efficacy of total laparoscopic hysterectomy (TLH) for these conditions remains limited. We compared the safety and efficacy of TLH and total abdominal hysterectomy (TAH) in patients undergoing simple hysterectomy following diagnostic conization. METHODS: We conducted a multicenter retrospective study of 196 patients with cervical intraepithelial neoplasia grade 3 (CIN3), AIS

Cervicovaginal Microbiome Signatures Across Cervical Disease States: A Prospective Cross-Sectional Analysis.

Background/Objectives: The cervicovaginal microbiome has emerged as a critical determinant of cervical health. In this study, we aimed to characterize the cervicovaginal microbiome across a spectrum of cervical health states and to identify community-level features that distinguish invasive disease from precursor states. Methods: We analyzed cervicovaginal samples of 86 patients with normal epithelium, low-grade (LSIL) and high-grade (HSIL) intraepithelial lesions, and cervical carcinoma (CCU) and available HPV genotyping. Vaginal samples were subjected to full-length 16S rRNA gene sequencing

Hematocervix and Hematometra: A Rare Case Report of HPV-16 Induced Cervical Intraepithelial Neoplasia (CIN).

INTRODUCTION: Hematocervix and hematometra are uncommon manifestations of cervical intraepithelial neoplasia (CIN). While cervical stenosis is more commonly associated with senile at-rophy or post-procedural scarring, its occurrence in the context of CIN remains exceptionally rare. CASE PRESENTATION: A 49-year-old female presented to the gynaecology outpatient department with complaints of diffuse pain in the lower abdomen for the last 3 months. On examination, the vagina appeared atrophic, and a reddish-blue swelling was noted at the location of the cervix. Cough impulse was positive. On bim

Obstructed hemivagina and ipsilateral renal anomaly syndrome: a case series of 4 patients with insights into diagnosis and treatment strategies.

Obstructed hemivagina and ipsilateral renal anomaly syndrome was first identified by Purslow in 1922 as a result of abnormal embryologic development of the Müllerian (paramesonephric) and Wolffian (mesonephric) ducts. The syndrome typically presents with cyclical lower abdominal pain and abnormal vaginal discharge in postpubertal females. Although the optimal timing and method of surgical treatment remain unclear, resection of the vaginal septum is commonly performed. This article presents 4 previously asymptomatic patients who developed an acute abdomen after menarche and were treated with hy

Impact of repeatedly screening negative on cervical cancer risk.

BACKGROUND: We demonstrated that cervical cancer risk following any screening result is lower if there is a known prior negative screening history vs an unknown screening history. We extended these findings to look at how screening performs following repeatedly negative screening results. METHODS: Approximately 1.7 million women aged 30-64 years underwent triennial human papillomavirus (HPV) and cytology co-testing from 2003 to 2021. We modeled 5-year risks of cervical intraepithelial neoplasia grade 3 or more severe diagnoses (CIN3+) and invasive cervical cancer for the initial co-test, and

Trends in Cervical Cytology Test Reports and Cervical Dysplasia Severity according to Social Deprivation in Mexico (2013-2019): An Analysis of a National Database.

BACKGROUND: Cervical cancer is a leading cause of cancer-related mortality in women living in low- and middle-income countries such as Mexico. Preventive and screening programs are often inaccessible to socially deprived populations, whose limited access to timely diagnosis and treatment reduces the chance of detecting premalignant lesions early. METHODS: Cervical malignant and premalignant lesion positivity rates were analyzed from 2013 to 2016 using the Bethesda system, whereas human papillomavirus (HPV) positivity rates were analyzed from 2017 to 2019. Both were stratified according to sta

Clinical Significance of Atypical Squamous Cells of Undetermined Significance (ASC-US) and Atypical Squamous Cells-Cannot Exclude High-Grade Squamous Intraepithelial Lesion (ASC-H) in a Low-Income Clinical Setting: A Retrospective Analysis.

BACKGROUND: Atypical squamous cells represent a significant category in cervical cytology screening, with a varying degree of malignant potential between ASC-US and ASC-H. In low-resource settings, challenges in follow-up and testing necessitate a clear understanding of the risk associated with these diagnoses. OBJECTIVE: To determine the Positive Predictive Value (PPV) for premalignant and malignant cervical lesions (CIN2+) in patients with ASC cytology results, and to evaluate management challenges, particularly the high loss to follow-up, in a low-resource clinical setting. METHODS: This

Cervicovaginal cytokine profiles and their associations with colposcopic findings in adolescent girls and adult women in South Africa: an exploratory cross-sectional study.

BACKGROUND: Colposcopy is used to evaluate cervical inflammation and detect precancerous and other cervical lesions; however, access remains limited in many low-resource settings. Identification of cervicovaginal immune biomarkers associated with specific colposcopic findings may provide insight into mucosal immune alterations linked to increased susceptibility to sexually transmitted infections (STIs) and human immunodeficiency virus (HIV), although these associations do not imply diagnostic or predictive utility in a cross-sectional context. METHODS: In this exploratory cross-sectional stud

Lactobacillus iners and vaginal microbiota diversity as risk factors of uterine cervix dysplasia: a prospective study.

PURPOSE: The aim of this study was to observe the vaginal microbiota composition in patients with cervical dysplasia. METHODS: In this prospective study, 91 samples for vaginal microbial diversity examination were taken from the cervix and posterior vaginal fornix. Eighteen bacterial species, including Lactobacillus species, were identified by real-time PCR. Relative bacterial quantities (RQs) were calculated, and log2-transformed RQs were visualised with boxplots. The relationships between cytological abnormalities and bacterial species were calculated by nonparametric one-way ANOVA (Kruskal

Balancing diagnostic accuracy and cost in the follow-up of histological low-grade squamous intraepithelial lesions.

INTRODUCTION: Primary cervical screening with testing for human papillomavirus (HPV) brings increased detection of women with low-grade squamous intraepithelial lesion recommended for follow-up with colposcopy. This study evaluates whether co-testing (HPV + cytology) or testing for HPV alone can safely replace colposcopy as follow-up for histologically confirmed low-grade squamous intraepithelial lesions. It estimates these strategies' diagnostic performance in detecting histological high-grade squamous intraepithelial or more severe lesions at follow-up and analyzes their relative cost-effect

Hidden endometrial cancer due to vaginal obliteration associated with lichen sclerosus/lichen planus.

An elderly postmenopausal woman with untreated vulvar lichen sclerosus and lichen planus (LP) overlap presented with abdominal pain, constipation and urinary symptoms. Severe vulvovaginal scarring caused complete vaginal obliteration, preventing conventional speculum examination and transvaginal ultrasonography. Endorectal and abdominal ultrasonography revealed a large cystic pelvic mass. Urgent laparotomy identified extensive haematocolpos and uterine distension due to complete outflow obstruction. The patient underwent a hysterectomy, bilateral salpingo-oophorectomy and subtotal colpectomy.

Agreement and Diagnostic Performance of Urinary HPV Testing Versus Clinician-Collected Cervico-Vaginal Sampling: A Prospective Cross-Sectional Study in a Romanian Cohort.

Background: Cervical cancer screening uptake in Romania remains below 20%, with invasive sampling cited as a major participation barrier. Urinary HPV testing is a non-invasive alternative, but data from Eastern European populations are scarce and reporting stratified by histological grade is inconsistent. Objectives: To evaluate the agreement between paired urinary and clinician-collected cervico-vaginal HPV testing, and to assess the diagnostic performance of urinary HPV testing for biopsy-confirmed cervical intraepithelial neoplasia grade 2 or worse (CIN2+). Methods: Prospective cross-sectio

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l
