Involucrin, a protein subunit of keratinocyte cross-linked envelopes, is a distinctive marker for suprabasal differentiation in stratified squamous epithelium. Fifteen cases of primary carcinoma in situ or severe or moderate dysplasia of the vagina have been diagnosed and treated during the period 1966-75 at the Department of Obstetrics and Gynecology, Turku University Central Hospital. Cell-free extracts were prepared from tissue specimens of intraepithelial carcinoma of the cervix uteri of human patients and subsequently injected into newborn and juvenile mice of NMRI strain via several routes. Cytological controls of vaginal epithelium were carried out in periods of 8--12 wk. Groups of mice were killed at varying intervals for histological examination. A case of verrucous carcinoma of the cervix uteri is reported. The cytologic findings in a Papanicolaou smear of the cervical area that permitted a diagnosis of verrucous carcinoma as opposed to epidermoid carcinoma or hyperkeratotic severe dysplasia are described. The importance of a proper diagnosis of verrucous carcinoma in order to avoid its treatment by radiation is stressed since verrucous carcinoma can turn into a more aggressive malignancy after such treatment.

Thirty-three women were examined on three or more occasions, each at 2-month intervals. The overall recovery rate of cytomegalovirus (CMV) was 28% (60/215). CMV was recovered from 53% of the cases where the local antibody was 1 : 64 or greater but the greatest difference occurred below titers of 1 : 32. Below that level virus was recovered in less than 5%, compared to a prevalence of 35% for cases with a titer of 1 : 32 (P less than 0.001). This report presents the cytologic findings and the rates of dysplasia for 4,589 young women enrolled in the National Cooperative Diethylstilbestrol-Adenosis (DESAD) Project. Proper technique and competent cytopathology are important in obtaining and evaluating the Pap smear. Colposcopy and directed biopsies, combined with endocervical curettage, will permit outpatient diagnosis in about 85 percent of patients with cervical intraepithelial neoplasia. Other patients will require knife conization for diagnosis. A wide range of treatment options exists. Selection of therapy is based on both the patient's reproductive plans and the severity of the disease. Regular follow-up with Pap smears is mandatory after any treatment.

During the years 1952--1977, 646 patients with abnormal cervical cytology underwent one biopsy (595) or ring biopsy of the cervix. The histological diagnoses were invasive carcinoma 20 (3%) microinvasive carcinoma 16 (2%), carcinoma in situ and severe dysplasia 418 (65%), other dysplasias 149 (23%). The cervical scrape technique used in diagnostic cytology may strip off epithelial sheets larger than 0.5 mm. We call them smear biopsies. We followed with cytological, histopathological and clinical examinations 28 nonpregnant women whose smear biopsies showed severe dysplasia or carcinoma in situ.
