Nail-matrix kinetics were studied in 21 patients (19 with onychomycosis, two with tinea corporis) as soon as taking itraconazole (Sporanox) 100 mg daily for up to 7 months. Itraconazole was detected in the distal nail as soon as 1 month after the start of therapy (42 ng/g in fingernails and 16 ng/g in toenails). Human infections due to microsporum gypseum are rare. We report on a female patient who had tinea corporis caused by M. gypseum after a cutaneous wound. The numerous lesions located on the borders of the wound had an eczematous appearance.

An atypical strain of Microsporum canis isolated from a two-year-old boy with tinea corporis is described. When cultured on Lactritmel agar the strain presented the typical pigment of M. canis without developing characteristic macroconidia. After 6 weeks, scarce, rudimentary, fusiform macroconidia 120-150 microns long developed on Lactritmel and Sablac microcultures. In vitro, the strain developed hair perforating organs. Experimental inoculation of a healthy volunteer produced a tinea with typical fluorescence and endoectothrix attack. After isolation the strain remained stable. These results

Oxiconazole nitrate (1%) cream became available in the United States in 1989 for the once-daily treatment of tinea pedis, tinea cruris, and tinea corporis. It has also proved valuable in the once-daily treatment of tinea (pityriasis) versicolor. In vitro oxiconazole is highly effective against many dermatophytes, including Trichophyton rubrum, Trichophyton mentagrophytes, Trichophyton tonsurans, and Epidermophyton floccosum. In the period between April 1984 and April 1988 a total of 4,294 clinically suspected cases of dermatomycoses were examined for causative fungi. Of these cases 768 (17.9%) were suspected as tinea corporis (427 cases) and tinea cruris (341 cases). Twenty two different nationalities were represented. Both sexes were involved and the age range was 19 days to 70 years. Of the 768 patients examined, 738 (96.1%) had their diagnosis confirmed by direct microscopy and in 631 of these positive cases the etiology was determined. During a two year period all patients referred to the Dermatology Department with the diagnosis of dermatophytosis were studied. We obtained 158 dermatophyte isolated belonging to 12 species of dermatophytes. The most frequent causal agents were M. canis in 58 (36.7%), T. mentagrophytes var mentagrophytes in 36 (22.7%), E. floccosum in 29 (18.3%) and T. rubrum in 17 (10.7%). M. canis was the most prevalent causal agent of tinea capitis and tinea corporis. The most frequent species in the tinea cruris was E. floccosum. A total of 4,294 clinically suspected cases of dermatomycoses belonging to 26 different nationalities were examined between April 1984 and April 1988. Fungi were demonstrated in routine potassium hydroxide/dimethyl sulfoxide mount in 3,814 cases (88.8%) and the etiology was determined by culture in 2,458 cases (57.2%). We have reviewed all the dermatophytoses diagnosed in Galicia during four consecutive 9-year periods 1951-86 and 1987. Fifty-five patients with griseofulvin-unresponsive dermatophytosis caused by Trichophyton rubrum were treated with itraconazole. They had either tinea corporis or "dry type" infections of the palms, soles, or nails. In a double-blind randomized study, 92 patients with culturally proven tinea corporis and/or tinea cruris were treated orally with either terbinafine (Lamisil) (125 mg b.i.d.) or griseofulvin (500 mg b.i.d.) for up to 6 weeks. The two groups of patients and distribution of the target lesions were similar, but the analysis of the clinical scores showed that the terbinafine group had slightly higher mean scores at baseline (P = 0.186). At the end of therapy the proportion of patients with negative microscopy and culture was 78% in the terbinafine group and 83% in the griseofulvin-treated group.

The authors studied 31 patients with Tinea cruris, corporis or pedis interdigital. The patients were treated with 100 mg of itraconazole, once a day, during the lunch, for 15 days. Mycological and clinical evaluations were performed at the pre-treatment, by the end of treatment and 2 weeks after finishing the treatment. The results showed that itraconazole is effective for the treatment of dermatophytosis, at the studied posology, with 100% of mycological cure. Clinical evaluation showed a significant reduction (Friedman test--p less than 0.001) of signs and symptoms.

Seventy patients with tinea cruris or tinea corporis were treated with naftifine cream 1% or vehicle once daily for 4 weeks in this double-blind, randomized study. After two weeks, the patients using naftifine had a significantly higher mycologic cure rate than the vehicle-treated patients (79% vs. 31%, p less than 0.001), and they showed significantly better resolution of signs and symptoms. Statistically significantly differences favoring naftifine over its vehicle were found throughout the treatment period and 2 weeks posttreatment.

Initial dose-finding and placebo-controlled studies in skin dermatophytoses demonstrated that itraconazole was more effective than placebo and that short-term treatment with itraconazole 100 mg daily gave better results than 50 mg daily. Thirty-eight cases of human infection with the recently described dermatophyte species Trichophyton raubitschekii were studied and a description is presented of the clinical and epidemiological features of infection with this organism. Microsporum gypseum infections are 0.4% of 4900 suspected dermatomycoses examined with direct microscopic exam and culture on Lactrimel medium or D.T.M. This geophilic dermatophyte was isolated in 20 cases (1.5% of all dermatophytes). More frequently M. gypseum caused tinea corporis (13 cases: 65%) and rarely tinea capitis (2 cases: 10%), tinea faciei (2 cases), tinea manus (2 cases) and tinea cruris (1 case).
