Itraconazole is an effective medication against the most common dermatophytoses. It has been shown to be more active than griseofulvin and ketoconazole. Ease of use, affinity for keratinized tissues, lack of toxicity, continued activity after discontinuation, and the possibility of using fixed schedules are advantages of itraconazole. The fixed schedules indicated by pharmacokinetics and clinical studies are one 100 mg capsule daily for 15 days in cases of tinea corporis and tinea cruris and the same dosage for 30 days in cases of tinea pedis and tinea manuum. These fixed treatments have some

Cryostat sections of skin and nerve lesions of leprosy were stained with monoclonal antibodies recognising Mycobacterium leprae antigens and indirect immunofluorescence. In both the tuberculoid and lepromatous lesions, PGL1, 55-65-kDa, 17-kDa protein antigens and cross-reactive non-protein antigens were present. 65-kDa antigens were seen mainly in the skin lesions of lepromatous leprosy. The infiltrates in both the skin and nerve granulomas of tuberculoid and lepromatous leprosy showed membranous staining with monoclonal antibodies recognising PGL1 and 55-65-kDa antigens. Bacilli in the lesion

The antifungal efficacy and tolerability of naftifine (cream, gel, 1% solution), were tested in an open, mycologically controlled study in 29 patients (mean age 32.7 +/- 2.8 years; 15 males, 14 females) with dermatomycosis caused by dermatophytes and yeasts. Particularly, 16 patients were affected with Tinea corporis; 11 patients by Pityriasis Versicolor and 2 other by cutaneous candidiasis. The mean treatment period was 32.9 days in a range from 1 to 44 days. Only one patient, with cutaneous candidiasis dropped out on the first day, because of a primary irritative dermatitis. Onchocerciasis is a chronic parasitic infection in which infiltration of the skin by microfilariae has been associated with a number of different pathological changes. This survey compared the prevalence of different forms of skin disease in two villages, one of which was located within the endemic zone for onchocerciasis (Zapallo Grande), in a lowland rain forest area of western Ecuador. The commonest skin diseases in both villages were scabies, pyoderma and pityriasis versicolor. In addition changes closely correlated with the presence of microfilariae in skin snips were found in Zapallo Gra

Fifty patients with tinea corporis were treated with either 500 mg of griseofulvin (microsize) or 500 mg of terbinfine daily for 6 weeks. Thirty-three patients completed the study. Thirteen (87%) of 15 patients who took terbinafine were considered successfully treated, while 13 (73%) of 18 patients in the griseofulvin group noted resolution of their disease. Five patients taking griseofulvin were forced to discontinue the therapy because of side effects, compared with two patients taking terbinafine. This preliminary study indicates that a new oral antifungal agent of the allylamine class, ter

Terbinafine (Lamisil) is the newest compound within a class of antimycotic drugs called allylamines. It is active against a broad range of dermatophytes and yeasts and exerts its fungicidal action by inhibiting squalene epoxidation during sterol synthesis in fungal membranes. Effective therapy (complete cure or mycological cure with minimal signs and symptoms) has been observed in 70-90% of 1200 patients treated topically with 1% cream for tinea corporis/cruris, tinea pedis, cutaneous candidiasis and pityriasis versicolor. Cure in patients treated systemically (125 mg b.i.d. orally) has been d

Studies were conducted to assess the bioequivalence of a new antimycotic formulation, ciclopirox olamine lotion 1%, to an established compound, ciclopirox olamine cream 1%. Results of in vitro studies, using skin samples from human cadavers and domestic pigs, demonstrated that the two formulations equally penetrate all layers of the stratum corneum and inhibit the growth of Trichophyton mentagrophytes and Candida albicans. In vivo studies in guinea pigs and in human volunteers demonstrated the comparable therapeutic efficacy of the lotion and the cream in experimental trichophytosis. In additi

Seventy-eight patients with tinea corporis or tinea cruris participated in a double-blind study with either 100 mg itraconazole or 500 mg ultramicronized griseofulvin for 15 consecutive days. Clinical outcome was significantly in favor of itraconazole at completion of treatment (72% response rate vs. 51%) and at the follow-up visit (91% response vs. 64%). The most important difference between both treatments was the mycologic outcome, for which itraconazole showed a cure rate of 87% compared to 57% for griseofulvin 2 weeks after completion of therapy. It is suggested that 100 mg of itraconazol

A new antimycotic imicazole drug, oxiconazole, used both in vitro and in vivo in the treatment of 212 cases of tinea corporis, tinea cruris and tinea pedis is reported. In vitro, it shows marked antimycotic effect against 24 strains of pathogenic fungi except those of Wangiella dermatitides and 3 strains of Aspergilli. The minimal inhibition concentration (MIC) to Epidermophyton floccosum and Candida guilliermondi are 1 microgram/ml and 0.5 microgram/ml. The cure rate of 2% cream of oxiconazole in 124 cases of tinea corporis and tinea cruris is 90.3%, and in 88 cases of tinea pedis, 89.78%.

In an unusual nosocomial outbreak, 13 staff and 11 patients in an acute and chronic health care facility were infected with the zoophilic dermatophyte, Microsporum canis. The dermatophyte was apparently introduced into the facility by a single infected patient. Likely modes of subsequent disease transmission include person-to-person contact, handling of contaminated laundry, and use of a shared razor. Infection control measures for managing such outbreaks are discussed.

An immunocompromised patient, 6 weeks after bone marrow transplantation, developed extensive skin infection due to Trichophyton rubrum. Because she could not tolerate ketoconazole and was hypersensitive to griseofulvin, she received itraconazole with complete recovery within 30 days. This case demonstrates that itraconazole may be effective treatment for dermatophytosis, even in immunocompromised hosts, and that there is no cross-reactivity between ketoconazole and itraconazole.

Langerhans cells (LC) were defined by indirect immunofluoresence using OKT6 monoclonal antibody in the skin lesions of 11 untreated patients with tinea corporis. The number and distribution of OKr6 + epidermal LC in the lesions was very low in comparison to that found in the normal skin. In most of the lesions, T6 + LC could not be seen in the epidermis. A like antigens were not detectable on the keratinocytes. These observations suggest deceased CMI response to the fungal infection and that the reduction in LC may be a contributing factor in the pathogenesis of tinea corporis.

A patient treated with immunosuppressant drug therapy for a non-Hodgkin's lymphoma and who presented with a tinea corporis developed a sudden outbreak of numerous subcutaneous nodules. Deep-seated hyphae and spores were found in skin biopsy specimens. There was no evidence of keratin debris or histologic features of granulomata and giant cells. Cultures from tinea and nodules grew Trichophyton rubrum and T. verrucosum. There are no more than 60 cases of deep dermatophytosis reported in the literature.

Data from 104 subjects with tinea cruris or tinea corporis were evaluated in this double-blind, randomized study. The subjects applied naftifine cream 1% or econazole nitrate cream 1% to affected areas twice daily for 4 weeks. After 1 week of treatment naftifine had an overall cure rate of 19% compared with 4% for econazole (p = 0.03). A difference in favor of naftifine, although not statistically significant after the first week, persisted throughout treatment. Two weeks after the end of treatment both medications had overall cure rates of approximately 80%. Three percent of the naftifine-tre

A total of 719 cases of dermatophytoses was studied in the Division of Dermatology, Department of Medicine, Siriraj Hospital, Mahidol University, Bangkok, Thailand, during January to December 1986. Infection caused by fungus comprises 12%, while the incidence of dermatophytoses was 5.5% of skin disorders. The age group commonly infected was 12-21 years old (mean age of 18 years); 40% was female and 47.5% was male. In the female patients the common prevalent infections were tinea corporis 29%, tinea cruris 23%, tinea pedis 16%, while in male patients it was 28%, 39% and 14% respectively. Only 4

Of the 1124 human immunodeficiency virus (HIV)-infected patients studied, one or more cutaneous lesions were found in 113 (98.3%) of the 115 patients with acquired immunodeficiency syndrome (AIDS) and in 541 (53.6%) of 1009 patients with AIDS-related complex (ARC). Kaposi's sarcoma, multidermatomal, necrotic herpes zoster, and pruritic maculopapular rashes are common cutaneous manifestations of AIDS and its related complex (ARC) in Zambia. The maculopapular rash results from a lymphoplasmacytic angiitis in the dermis, possibly in response to the presence of HIV in the dermis. Candidiasis, seve

In this pilot study, itraconazole was administered to 18 patients with tinea corporis. The dosage was 100 mg daily for four to six weeks. Follow-up visits took place two, four, and six weeks after the first dose. Skin samples were cultured, and clinical symptoms were assessed at each visit. At two weeks clinical symptoms were significantly reduced in all patients, and cultures were negative in approximately 50%. After four and six weeks, respectively, 83% and 89% of patients had negative cultures and 89% and 94% were considered markedly improved or cured in an overall assessment. None of the p
