Two patients with lichen sclerosus (et atrophicus) are described in whom there was conspicuous localization of lesions to sites subjected to friction from tight clothing. This was highly suggestive of the Köbner phenomenon. Both patients experienced symptomatic improvement following avoidance of pressure on the skin from tight clothing.

Lichen sclerosus et atrophicus (LSA) is uncommonly reported in family members and is seen occasionally in young children. We report genital LSA in two young sisters who were diagnosed a year apart. The English language literature on familial LSA in childhood is reviewed and discussed.

Mostly, older women suffer from dystrophic disease of the vulva, which develops in a chronically progressive way. The main symptoms are substantial pruritus and secondary alteration of the skin. Good results could be obtained by a combination of different conservative treatments. Relapses occurred in about 10% of the cases. In this study, immunohistochemistry was used to investigate p53 and proliferating cell nuclear antigen (PCNA) expression in 40 cases of lichen sclerosus et atrophicus (LSA). Nuclear immunoreactivity for both p53 and PCNA was found in the basal and parabasal keratinocytes in the majority (91%) of the lesions. The slides of 64 vulvectomy specimens from vulvar squamous carcinoma were reviewed in order to study the histopathologic changes adjacent to the neoplasia. Normal epithelium was found in 7 cases (11%) and epithelial alterations adjacent to carcinoma in 59 (89%). Lichen sclerosus and lichen planus affecting cutaneous sites are easily distinguishable clinical. Clinical signs on mucosal sites, however, may not allow differentiation between these diseases, and reliance is frequently placed on histopathological findings. Acetowhite lesions in the vulva disclosing koilocytosis have been related to infection by human papilloma virus (HPV). Because of the clinical resemblance of these lesions to oral hairy leukoplakia (OHL), and EBV-manifestation, 23 women with acetowhite koilocytotic lesions in the vulva were examined. Super potent topical steroids are an effective treatment for many patients with vulval lichen sclerosus, but concerns exist about their long-term safety. Review of 12 women up to 39 months following a course of super potent topical steroids shows that subsequent control of disease activity and symptoms is much easier than before therapy, with most patients requiring only small amounts of moderate potency topical steroid. A randomized study was conducted on 79 patients with vulval lichen sclerosus who were treated for three months with four topical drugs including testosterone (2%), progesterone (2%), clobetasol propionate (.05%) and a cream based preparation. To evaluate the efficacy of the treatments, patients were examined before and after therapy for symptoms, gross appearance of the lesions and histopathologic features.
