Studies of 82 skin specimens from 81 cases of lichen sclerosus et atrophicus (LSA) are reported. Typical LSA pathological changes showed atrophy of the epidermis, hydropic degeneration at the basal layer, and homogenization of the collagen fibers in the superficial dermis with inflammatory cell infiltration underneath. Ultrastructural changes showed intercellular spaces enlarged, particularly near basal cells. Some mitochondria were swollen and rough endoplasmic reticula were enlarged. Seven patients with a histologic diagnosis of lichen sclerosus et atrophicus of the vulva were treated by laser ablation of the affected area to a depth of 1.0 to 2.0 mm. The inpatient procedure was performed under general anesthesia with a carbon dioxide laser unit. Healing was complete 6 weeks postoperatively. All but one patient was free of recurrent symptoms at follow-up, which ranged from 12 to 37 months. Laser ablation is an acceptable treatment for patients who have symptoms due to lichen sclerosus of the vulva that are refractory to other measures.

Lichen sclerosus typically affects the vulva of postmenopausal women. The paper is a histo-clinical analysis of 75 cases of vulvar dystrophy defined so far as leukoplakia or kraurosis vulvae. The series included 31 cases of hypertrophic dystrophy, 34 cases of lichen sclerosus and 10 cases of mixed dystrophy. In 52 cases changes developed after menopause. In two cases cellular atypia was found. Itching was the most frequent physical symptom. In the paper current views on the connection vulvar dystrophy with the development of invasive vulvar carcinoma have also been discussed.

Subclinical vulvar lesions have been studied using the magnifying colposcope after 5% acetic acid application in 65 women with H.P.V. related cervical lesions and/or cervical intraepithelial neoplasms. Levels of tropic hormones of the pituitary, glucocorticoids and male and female sex hormones were assayed in 17 cases of kraurosis and leukoplakia of the vulva prior to treatment. Most patients revealed an abnormally high androgen/estrogen ratio, elevated level of glucocorticoids and low concentrations of thyrotropic hormone, prolactin and T3. The above disturbances are closely related and interdependent.

Light and electron microscopic studies were carried out on 17 cases of lichen sclerosis et atrophicus of the vulva before treatment and on 8 after treatment. For comparison, normal vulval skin was obtained for examination from 3 cases undergoing plastic surgery. The affected skin, prior to therapy, showed various degrees of atrophic and degenerative changes with widening of intercellular spaces and a diminished number of desmosomes. 129 patients with carcinoma of the vulva were treated at the Dept. of Obstetrics and Gynaecology from 1966-1985. FIGO stage I was observed in 21% of the cases, FIGO stage II in 50%, FIGO stage III in 10% and FIGO stage IV in 8% of the patients. In 10% of the patients, definite classification was not possible. The clinical features and differential diagnosis of chronic erosive vulval disease are described. We present a case in which self-treatment of lichen sclerosus with large doses of topical testosterone resulted in marked masculinization and may have caused secondary amenorrhea. These changes have not previously been reported as side effects of topical testosterone use.
