To investigate clinical condyloma, abnormal cervical cytologic findings, and evidence of human papillomavirus infections, 89 adolescent girls were examined. Cellular DNAs extracted from exfoliated cervical cells were examined for human papillomavirus genomic sequences by Southern transfer hybridization using 32P-labeled human papillomavirus DNA probes. Repeated topical application of 13-cis-retinoic acid resulted in complete disappearance of leukoplakia of the vulva in 8 out of 16 patients, among them 2 out of 3 with recurrence after vulvectomy. A considerable regression of leukoplakia was seen in 7 other patients, usually after 1 to 2 months of daily retinoid treatment. After 2-4 months of maintenance therapy and during 3-7 months of the follow up period no recurrences were seen. Side effects of treatment could be managed. Fifteen women with lichen sclerosus (LS) of the vulva were treated for 3 months with 0.6 mg/kg of etretinate. The clinical response was good in 11/15 of the patients and, in agreement with this, light microscopy showed decreasing atrophy and hyperkeratosis together with subsiding inflammatory changes. There is no known effective treatment for lichen sclerosus et atrophicus. It is therefore necessary to check new methods of treatment. We treated 14 patients with testosterone propionate ointment. In 10 of 14 cases the desired result was achieved. This therapeutic effect was more effective than the results in the control group.

Twelve types of human papillomaviruses (HPV) have been isolated thus far from papillomatous and Bowenoid lesions of the human genital tract. Four type, HPV 6, 11, 16 and 18 are most frequently found. HPV 6 and 11 cause the typical genital warts (condylomata acuminata) and mild dysplastic lesions of the cervix characterized by a high degree of koilocytotic atypia. HPV 16 and 18 are preferentially found in Bowenoid papulosis and Bowen's disease at external genital sites. Moderate and severe cervical dysplasias with little or no koilocytosis appear to be common manifestations of these infections

In an open uncontrolled study eight patients with vulval lichen sclerosus et atrophicus were given etretinate (Tigason) 1 mg/kg/day. The dose was then gradually reduced according to effect and side effects. Six patients showed improvement in symptoms and in clinical morphology based on the physician's evaluation and photographic documentation. The development of vulval lichen sclerosus et atrophicus in monozygotic twins is described. This is the first report of the occurrence of lichen sclerosus et atrophicus in two genetically identical individuals, and is considered to provide further evidence that inherited factors are of relevance in the aetiology of this disorder.
