Twenty patients with vulvar dystrophy (19 Lichen sclerosus, 1 Lichen ruber planus) were treated for 3 months with etretinate (Tigason) with an initial dose 0.54 mg/kg/day, maintenance dosage 0.26 mg/kg/day. All the patients had been unsuccessfully treated previously with topical oestrogen and corticosteroids. Although lichen sclerosus was first described almost a century ago, a grading system was not devised, and the minimal histologic criteria required for diagnosis were not defined. The present study was based on the sequential sectioning of nine vulvectomy specimens removed for invasive carcinoma. Both epidermal and dermal changes were noted, but only the dermal ones were unique to the disease. Early changes included subtle densification of the basement membrane as well as hyalinization of the papillary dermis and superficial capillaries. Based on these findings, a grading system was developed.

Involucrin, a precursor of the envelope protein present in human stratum corneum, may be used as a histologic marker. Involucrin expression was studied in biopsy specimens from different types of vulvar lesions. In general, the pattern of staining for involucrin has been the one described for similar pathologies of the integument in other bodily areas. However, an unexpected pattern was noted in lichen sclerosus of the vulva.

Sixteen patients with bowenoid papulosis (eleven male patients with bowenoid papulosis of the penis and five female patients with bowenoid papulosis of the vulva) were studied clinically, histologically, and virologically and followed up from 12 months to 5 years. The authors describe a family in which the mother and one son are affected by oesophageal leiomyomatosis and nephritis with haematuria. The mother also presents hypertrophy of vulva and clitoris, and her son has perceptive deafness and congenital cataract. In the medical literature only 15 cases of oesophageal leiomyomatosis in children and adolescents could be found. The association with Alport's syndrome was first described by Torres and Guarner in 1983.

Lichen sclerosus et atrophicus was an associated lesion in 16 of 30 (53%) patients with invasive vulvar carcinoma. In one case invasive carcinoma developed in the background of lichen sclerosus et atrophicus. Additionally in one case lichen sclerosus was found at the margin of the carcinoma. Despite the low malignant potential, patients with vulvar lichen sclerosus et atrophicus need long-standing observation and recurrent histologic examinations.

Serum vitamin A and E concentrations were determined in six patients with vulvar epithelial carcinoma and in 25 patients with lichen sclerosus and atrophicus (LSA) of the vulva. No difference could be found in these levels between the two groups. LSA patients received oral etretinate for two months at least; improvement was observed in 93% of the patients. Etretinate therapy had no effect on the serum levels of vitamins A or E.

A demonstration multiphasic cancer screening program was conducted in a rural village in Thailand under the coordination of Bangkok's National Cancer Institute. The village's physician, mayor, and other members of the community assumed a variety of volunteer roles in order to provide logistical support. The article reports about the relatively rare diseases kraurosis vulvae (lichen sclerosus et atrophicus) and leucoplakia vulvae and the symptoms, aetiology, histology and therapy of these diseases. Both diseases are considered to be a "dystrophy" since 1976, by international convention. A skinning vulvectomy and split-thickness skin graft were used in the management of four women with benign vulvar disease after the failure of standard medical and surgical therapy. Three women remain free of the primary disease process, with a follow-up of 12-36 months. One woman with lichen sclerosus and another with "diffuse atrophy" developed recurrent disease in their skin grafts 45 and 96 months, respectively, after skinning vulvectomy. Symptomatic improvement was achieved subsequently in both women with the topical application of testosterone.

The nevus spongiosus albus mucosae (resp. white sponge nevus in the angloamerican literature) is a hereditary leukokeratosis localised preferably in the oral mucosa, but may simultaneously manifest itself in other regions, e.g. perianally. We report the case of a 18-year old patient with a primarily and exclusively extraorally located nevus spongiosus albus mucosae of the vulva. The differential diagnosis and clinical as well as therapeutical problems of this extremely rare disease are discussed.
