It has been superficially recognized, but not appreciated, that carcinoma of the vulva differs in many respects from carcinoma of the uterine cervix. Past studies have suggested that "leukoplakia and leukoplakia-like lesions" are probably premalignant conditions since they are commonly associated with invasive cancer. Conversely, studies of the dystrophies during the past 15 years have indicated that the latter uncommonly progress to invasive cancer if treated adequately. Twelve patients with lichen sclerosus et atrophicus of the vulva were treated by a nitrous oxide cryoprobe. All had had severe itching which was relieved in the post-operative period in 75% and for 3 years in 50%. Eight had histologically proven leukoplakia which cleared post-operatively in five (62.5%) and remained clear at three years in four (50%). In one patient pruritus, leukoplakia and ultimately squamous carcinoma appeared.

Seventeen patients, after abnormal Papanicolaou smears, were found to have vaginal lesions ranging from moderate dysplasia to carcinoma in situ. They were evaluated by colposcopy and biopsy. They were treated with topical 5-fluorouracil (5-FU) in the vagina, administered by the patient twice a day for 10 to 14 days. A review of 41 evaluable patients was made in order to study vaginal intraepithelial neoplasia (VAIN) and to investigate new methods of treatment. Colposcopic examination of the vagina revealed white epithelium alone in 20 patients and white epithelium associated with vascular punctation in 15. No lesion had a vascular mosaic pattern. Two cases of verruciform xanthoma of the vulva are described. This rare lesion, previously reported to occur only in the oral cavity, is characterized by a verrucous epithelial proliferation accompanied by xanthoma cells distributed exclusively in the papillary dermis. Vulvar intraepithelial neoplasms may be occurring more frequently among younger age groups. Six patients with carcinoma in situ or severe vulvar dysplasia are reported, as well as one patient with microinvasive carcinoma, all under the age of 30 years. The vulvar lesions were noted to be mostly multicentric. They were associated with other intraepithelial neoplasias of the cervix or vagina in four of the six cases. The lesions were most often mistaken for condyloma acuminatum. Histologic changes were primarily Bowen's disease or Bowenoid dysplasia.

Presented is a case of a 6-year-old girl with biopsy-proved lichen sclerosus et atrophicus (LSA) localized to the anogenital region. She was seen approximately 3 months after the development of vulvar pruritus and burning on micturition. Following vulvar biopsies the patient was treated with hydrocortisone cream. Four months postbiopsy the patient is asymptomatic but with the coalesced white plaques remaining.
