Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Hidden endometrial cancer due to vaginal obliteration associated with lichen sclerosus/lichen planus.

An elderly postmenopausal woman with untreated vulvar lichen sclerosus and lichen planus (LP) overlap presented with abdominal pain, constipation and urinary symptoms. Severe vulvovaginal scarring caused complete vaginal obliteration, preventing conventional speculum examination and transvaginal ultrasonography. Endorectal and abdominal ultrasonography revealed a large cystic pelvic mass. Urgent laparotomy identified extensive haematocolpos and uterine distension due to complete outflow obstruction. The patient underwent a hysterectomy, bilateral salpingo-oophorectomy and subtotal colpectomy.

Expert opinion on characteristics of vulval lichen sclerosus: initial identification of important clinical features through an international electronic Delphi consensus study.

BACKGROUND: Lichen sclerosus (LS) is a chronic inflammatory condition that mainly affects genital skin. It causes distressing symptoms that impact daily quality of life, as well as progressive anatomical changes and a potential risk of cancer. Vulval LS is often misdiagnosed and treatment delayed. If nonexperts can be supported to identify clinical features of LS, they may diagnose the condition sooner and commence timely treatment or refer a patient for assessment and confirmatory biopsy. OBJECTIVES: To reach international expert consensus on clinical diagnostic features for vulval LS. METH

Barriers to diagnosing and treating vulval lichen sclerosus: a survey study.

BACKGROUND: Vulval lichen sclerosus (VLS) is a chronic inflammatory condition that is frequently misdiagnosed and under-recognised. To date, qualitative research has focused on lived experience of VLS, with women attributing diagnostic delay to poor interactions with healthcare professionals (HCPs), often due to lack of knowledge. In the UK, women with VLS are most likely to present to primary care. AIM: To establish HCPs' perspectives on the identification, management, and education of vulval skin disease, with a focus on VLS. DESIGN AND SETTING: A mixed-methods study survey undertaken acro

Amitriptyline for the treatment of vulvodynia in patients with vulvar lichen sclerosus: a case series of 20 patients.

Vulvar Lichen Sclerosus (VLS) is a chronic autoimmune disease that often leads to vulvodynia, a debilitating chronic pain condition in the vulvar region. Treating vulvodynia in the context of VLS presents a significant therapeutic challenge due to the lack of standardized protocols. This article presents a case series of 20 women treated with amitriptyline for vulvodynia secondary to VLS, accompanied by an extended review of the literature. The results suggest that amitriptyline may offer significant pain relief and improvements in the patients' quality of life, although adverse effects need t

Platelet-rich fibrin attenuates inflammation and fibrosis in vulvar lichen sclerosus via the TGF-β/SMAD3 pathway.

Vulvar lichen sclerosus (VLS) is a chronic, inflammatory, and progressive skin disease mainly involved in the anogenital area. Platelet-rich fibrin (PRF) is a fibrin adhesive-concentrated platelet-rich plasma (PRP) used for tissue repair and angiogenesis. In this study, we explored the effects of PRF on VLS patients, further to utilize the established VLS animal model, to confirm the therapeutic effect of PRF and regulation on the TGF-β/SMAD3 pathway. Among the 46 VLS patients included in the analysis, injectable RPF (i-PRF) treatment improved the symptoms of VLS. The immunohistochemical analy

A randomized controlled trial to evaluate a novel dual laser therapy for vulvar lichen sclerosus: exploratory study assessing the impact of menopausal status.

OBJECTIVE: A randomized controlled trial showed that Neodymium:YAG/Erbium:YAG laser therapy was safe and significantly improved clinical outcomes and subjective symptoms of vulvar lichen sclerosus (LS). Most improvements were similar to those after the recommended first-line therapy with topical steroid. In this exploratory study, we wanted to analyze the impact of menopausal status on perception and treatment outcome. METHODS: Sixty-six women with clinical lichen sclerosus were assigned to the laser or the steroid arm (2:1). Participants of the laser arm received four laser treatments with a

Vulvodynia and Chronic Vulvar Pain: Influencing Factors and Long-Term Success After Therapeutic Local Anesthesia (TLA).

INTRODUCTION: Vulvodynia is a debilitating sexual disorder with a high prevalence of 7-11%. In the study reported here, we analyzed long-term results from a prospective, non-controlled observational study to enhance our understanding of the success of therapeutic local anesthesia (TLA) and to investigate factors that predict a response or failure of therapy, with the overall aim to gain new insights into the complex medical condition of vulvodynia. METHODS: A total of 45 patients diagnosed with severe chronic vulvodynia or chronic vulvar pain (Numeric Analog Scale [NAS] ≥ 6, median 7.9, durat
