OBJECTIVES: To compare the efficacy of fractional carbon dioxide laser (FxCO2) and focused ultrasound (FUS) in treating vulvar lichen sclerosus (VLS).
METHODS: This single-center retrospective study included biopsy-proven VLS patients who did not respond to first-line topical corticosteroids and were treated with FxCO2 or FUS as an alternative between January 2020 and October 2022. The efficacy was evaluated using the Cattaneo score and clinical scoring system (CSS). A subgroup analysis was conducted to compare efficacy based on VLS severity.
RESULTS: At the 3-month follow-up, the total effe

BACKGROUND: A systematic approach to surveillance of vulval dermatoses and pre-cancers is recommended for early detection of vulval cancer and nurses are crucial in care delivery. Typically, patients with gynaecological cancers have significant unmet information needs; however the extent in patients with vulval dermatoses and pre-cancers is unknown.
AIM: To evaluate knowledge gaps and educational requirements of patients attending vulvoscopy clinics at a Western Australian tertiary centre.
METHODS: A retrospective audit examined data of 307 patients attending vulvoscopy clinics. A cross-sect

Vulvar lichen sclerosus is a chronic, progressive inflammatory dermatosis that predominantly affects postmenopausal women but can also present in premenopausal individuals. It can cause significant scarring, leading to anatomical distortion, urinary dysfunction and impaired quality of life. The author describes a case of a woman in her late 30s with extensive vulvar involvement of lichen sclerosus, resulting in introital stenosis, dyspareunia and painful micturition. The patient was managed with a combination of topical steroids and systemic immunosuppressants, followed by surgical interventio

OBJECTIVE: Untreated vulvar lichen sclerosus (VLS) can lead to irreversible anatomical changes and increase malignancy risk. Some patients show poor response to standard treatments, resulting in refractory cases (RVLS).
METHODS: To explore risk factors associated with RVLS and integrate treatment strategies for improved clinical management, we conducted a retrospective analysis, which included patients with VLS who visited our outpatient clinic between March 2017 and March 2025. Additionally, an evidence synthesis of the currently reported treatment regimens for RVLS was conducted.

INTRODUCTION: Paediatric vulval lichen sclerosus (VLS) is a chronic disease with distressing symptoms and severe consequences when left untreated. Majority of existing data on pathophysiology and treatment is based on studies conducted among adult patients. Whereas the course of VLS, its symptomatology and prognosis are distinct to some extent in paediatric and adolescent patients as compared with adults. The purpose of this scoping review is to systematically examine what symptoms of VLS are typical of paediatric and adolescent patients, how often specific signs and symptoms are reported in t

Vulvar lichen sclerosus (VLS) is a chronic, recurrent dermatosis predominantly affecting postmenopausal women. It is characterized by atrophic and sclerotic changes in the vulvar skin, often causing severe discomfort and functional impairment. Although potent topical corticosteroids remain the primary treatment, high-intensity focused ultrasound (HIFU) emerges as a promising alternative in recurrence cases. This study reports the case of a 73-year-old woman with VLS treated using HIFU, aiming to assess the safety and efficacy of this novel therapeutic approach. The patient presented with signi

Vulval squamous cell carcinoma (VSCC) represents approximately 90% of all vulval cancers. Despite its rarity relative to other gynaecological or skin malignancies, the recent growing body of epidemiological and clinical evidence has resulted in important refinements to national guidelines. This review offers a comprehensive overview of the current evidence regarding the epidemiology, clinical features, diagnosis, and management of VSCC. There are two distinct aetiological pathways currently identified in the pathogenesis of VSCC. Human papillomavirus (HPV)-dependent VSCC is commonly found in y

OBJECTIVE: This study evaluated the therapeutic effect of 5-aminolevulinic acid photodynamic therapy (ALA-PDT) in refractory Vulvar lichen sclerosus (VLS) patients.
METHODS: This prospective study enrolled a total of 60 patients with refractory VLS. These patients received 6 sessions of ALA-PDT treatment. Follow-ups were conducted at 3, 6 and 12 months after treatment to evaluate the improvement of patients' subjective symptom and objective clinical signs. In addition, the Dermatology Life Quality Index (DLQI) was used to assess the quality of life (QoL) of patients after treatment.

Vulvar neoplasia is on the rise and patients often present to dermatologists with lesions of varying morphology associated with pruritus, burning sensation, or pain. Early diagnosis results in an early treatment and improved quality of life. Differentiating vulvar neoplasia and intraepithelial malignancies from benign and inflammatory conditions is imperative. Histopathology and immunohistochemistry (IHC) are helpful. Herein, we report eight women who presented with varying features. Five of them had predisposing factors- four had lichen sclerosus, and one was human immunodeficiency virus (HIV

Platelet-rich plasma (PRP) therapy is a non-invasive, autologous treatment with regenerative potential in gynaecology beyond fertility applications. This review evaluates PRP in non-fertility-related gynaecological conditions affecting women's quality of life (QoL). Methods: Following PRISMA-ScR guidelines, we searched Embase, CINAHL, Web of Science, Scopus, CENTRAL, and MEDLINE for studies on PRP in conditions such as vulvar lichen sclerosus (VLS), vulvovaginal atrophy (VVA), sexual dysfunction (SD), stress urinary incontinence (SUI), and interstitial cystitis/bladder pain syndrome (IC/BPS).

Vulvar diseases are a neglected area of women's health, profoundly affecting patients' QOL. Lichen sclerosus is a chronic inflammatory vulvar skin disorder leading to severe itching, pain, scarring, and an increased risk of malignancy. Despite this burden, the molecular pathogenesis of vulvar lichen sclerosus is not well-understood, limiting treatment options. In this study, we analyze lesional, nonlesional, and healthy vulvar skin using technologies including spatial and single-cell transcriptomics. Our findings identify unifying molecular changes across multiple cell types in lesional vulvar

Vulvar lichen sclerosus (LS) causes significant morbidity, and diagnostic delays of 5-15 years are common. This study uses a previously described secret-shopper model to evaluate access to specialized vulvar care in the United States. Of 246 clinics contacted, 165 (67.1%) were accepting new patients across 37 states. Median wait time was 55 days, significantly shorter for cash-pay clinics (15 days) than those accepting commercial insurance (63.5 days, P=.0014). Cash-pay fees averaged $860 for a new-patient appointment. Fifty-one clinics (20.7%) were closed or the clinician had retired, 18 (7.3

Vulvar lichen sclerosus (VLS) involves chronic inflammation, immune dysregulation, and abnormal extracellular matrix remodeling, involving extracellular matrix protein 1 (ECM1) and non-coding RNAs, particularly miR-155. Platelet-rich plasma (PRP) and adipose-derived mesenchymal stem cells (ADSCs) offer regenerative potential through the release of growth factors and cytokines that promote angiogenesis, fibroblast proliferation, collagen synthesis, and tissue repair, which could potentially compensate for the disordered matrix in VLS. This systematic review aimed to evaluate the current evidenc

INTRODUCTION: The vulvar microbiome is adjacent to that of the skin and the vagina and connects microbiomes present on a stratified epithelial barrier to that of a mucosal barrier. Yet, the characterization of the microbiome in the vulvar region of the body is understudied, although dysbiosis in the microbiome of the skin or vagina have been linked to impairments in women's health.
METHODS: To better understand the role of the vulvar microbiome during healthy aging or during presentation of vulvar diseases, we analyzed the vulvar microbiome by shotgun metagenomic sequencing on composition at

Lichen sclerosus (LS) is a chronic inflammatory skin condition that most often affects the perineal region and can greatly impact quality of life. This report presents the case of a 69-year-old woman with a 20-year history of vulvar LS. Despite standard treatment with topical corticosteroids, as well as multiple other therapies, including some nonstandard approaches, her symptoms persisted. Laboratory evaluation revealed elevated IgG antibodies to tomato. After eliminating tomatoes and other nightshade vegetables from her diet, the patient remained symptom-free for over a year. This case highl
