Vulvar lichen sclerosus (LS) and lichen planus (LP) are chronic inflammatory dermatoses that significantly impair quality of life through pruritus, pain, sexual dysfunction, and risk of scarring or malignancy. Although they share lichenoid histopathologic features, LS primarily affects keratinized skin, while LP commonly involves mucosal surfaces and may present with erosive, papulosquamous, or hypertrophic variants. Disease underrecognition often delays diagnosis and treatment. This review summarizes the epidemiology, pathophysiology, clinical presentation, histopathology, and management of v

Lichen sclerosus (LS) is a chronic inflammatory condition of the vulva. Although LS is thought to arise from autoimmune mechanisms, it is unclear whether disease drivers differ by menopausal status-an important distinction given its potential progression to differentiated vulvar intraepithelial neoplasia (dVIN) and vulvar squamous cell carcinoma (VSCC). We performed a retrospective review of 182 biopsy-proven LS cases (2020-2025), stratified by menopause status. In addition, we examined 27 VSCC cases for PD-L1 staining to assess immune modulation and potential therapeutic relevance. Premenopau

Sexual health is central to quality of life; yet, its relationship to dermatologic disease remains insufficiently studied. Skin conditions can directly contribute to sexual dysfunction through pruritus, scarring, pain, and body-image distress, and genital involvement is common but often overlooked. We present two cases to examine the ethical implications for dermatologic practice. In the first, a patient with vulvar lichen sclerosus reports avoiding intimacy, but the dermatologist does not explore the concern further. In the second, a patient with well-controlled plaque psoriasis never discuss

OBJECTIVE: This scoping review aimed to examine the existing literature on the long-term management of stable vulvar lichen sclerosus (VLS), with a focus on care settings, provider roles for long-term management of uncomplicated disease, and to identify gaps within these frameworks.
METHODS: PubMed, Embase, and Web of Science were searched from inception to August 2025. Eligible sources included original research studies, audits, surveys, clinical practice guidelines, and consensus statements published in English with full-text availability. Studies involving adult women with VLS that address

Vitiligo and lichen sclerosus (LS) are autoimmune dermatoses that may share overlapping clinical and histopathologic features, posing diagnostic challenges when they occur concurrently. We report a biopsy-confirmed case of vulvar vitiligo and LS in a 61-year-old woman presenting with pruritic, depigmented patches on the labia majora and minora. Histopathologic examination showed epidermal thinning with papillary dermal hyalinization and a dense, band-like lymphocytic infiltrate consistent with LS. Melan-A immunostaining demonstrated a complete absence of epidermal melanocytes, confirming concu

Discoid lupus erythematosus (DLE) is a chronic cutaneous manifestation of lupus that may occur independently or precede systemic disease. Vulvar involvement is exceptionally rare and can mimic more common vulvar dermatoses. We report a case of vulvar hypertrophic DLE in a 55-year-old African-American woman with systemic lupus erythematosus who was presented with an asymptomatic erythematous, ulcerated vulvar lesion initially suspected to represent herpes simplex virus infection or lichen sclerosus. Histopathologic examination demonstrated vacuolar interface dermatitis, follicular plugging, bas

Vulvar Lichen Sclerosus (VLS) is a chronic inflammatory dermatosis of unknown etiology affecting the external genitalia. In pediatric patients, it can lead to significant discomfort and progressive structural changes in tissues. In recent years, there has been increasing interest in the participation of immune checkpoints and inflammatory mediators in the pathogenesis of chronic diseases, including VLS. Immune checkpoints, such as PD-1, PD-L1, CTLA-4, CD200, and CD200R, play a crucial role in modulating the immune response and may serve as potential diagnostic markers and therapeutic targets.

Lichen sclerosus (LS) is a chronic, relapsing, inflammatory skin condition that primarily affects the anogenital region. Treatment with potent topical corticosteroids (TCS) can help control symptoms for many patients, but complications such as atrophy and scarring often persist. Platelet-rich plasma (PRP) has emerged as a regenerative therapy with potential application in LS. We review the clinical evidence and highlight gaps and future research needs. We searched PubMed and Web of Science databases for studies of PRP in LS. The outcomes evaluated included symptom relief, quality of life (QoL)

OBJECTIVES: Incompletely treated vulvar lichen sclerosus can lead to major complications and reduced quality of life. Limited treatment options beyond super-potent topical steroids are available for these recalcitrant cases.
METHODS: In this descriptive, retrospective case series, 3 female patients ages 36-63 presenting with recalcitrant, biopsy-proven vulvar lichen sclerosus were treated with oral upadacitinib at a dosage of 15 mg daily and monitored at 1-2 month intervals.
RESULTS: All 3 patients experienced clinical improvement in signs of lichen sclerosus and a decrease in symptom severi

BACKGROUND: Vulval Lichen Sclerosus (VLS) is a chronic inflammatory condition requiring lifelong management to prevent severe complications including malignancy. A shared care model between general practitioners (GPs) and public non-GP specialists (dermatologist or gynaecologist) for patients with stable LS would alleviate the demand on public vulval services. This study aims to assess GPs' level of comfort in performing vulval examinations for patients with VLS which would underpin the feasibility of shared care. It also explores GPs' level of confidence in vulval health, preferences for futu

Hypertrophic lichen sclerosus (HLS) is poorly characterized. A recent study described an atypical form of LS (p53 wild type) with a CK17/D2-40 immunohistochemical profile intermediate between classic LS and differentiated vulvar intraepithelial neoplasia (dVIN). Our study aims to characterize HLS through IHC and molecular profiling to determine if it is a classic or atypical LS lesion. We reviewed HLS diagnosed in 12 biopsies (2014 to 2025) and 5 resections for vulvar squamous cell carcinoma (vSCC) (1998 to 2015) from our institution (mean patient age 59; range: 37 to 97). We defined HLS by no

Vulvar lichen sclerosus (VLS) is an often underrecognized idiopathic T-cell-mediated inflammatory dermatosis that affects approximately 0.5-3% of the female population. Symptoms may include itching, burning, irritation, and dyspareunia; however, a significant proportion of patients are asymptomatic. Typical signs include hypopigmentation of the tissues, hyperkeratosis, erosions, fissuring, thinning of the skin, ecchymosis, and erythema. Anatomic changes that may develop include fusion of the clitoral hood, regression of the labia minora, and scarring and stenosis of the vaginal introitus. Thes

Lichen sclerosus is a cutaneous disorder affecting the external genitalia, with diagnosis and treatment generally managed by a dermatovenerologist. However, in clinical practice, gynecologists are frequently the first physicians to encounter affected patients. Lichen sclerosus is characterized as chronic, lymphocyte-mediated inflammatory dermatosis that requires a multidisciplinary approach. The incomplete understanding of its etiopathogenesis is reflected in limited therapeutic options, which are primarily directed toward stabilization of clinical manifestations, symptom control, reduction of

Background: Vulvar lichen sclerosus (VLS) represents a chronic inflammatory dermatosis associated with significant symptoms, architectural changes of the vulva, and impaired quality of life (QoL). Photodynamic therapy (PDT) with 5-aminolevulinic acid (5-ALA) has emerged as a potential therapeutic option for patients with disease refractory to standard topical treatments. The aim was to investigate whether PDT is associated with QoL and sexual functioning outcomes. Methods: Patients with refractory VLS were treated with PDT using a 10% 5-ALA nanoemulsion applied to affected areas and covered wi

Although external genital cutaneous sarcoidosis is rare, it can closely resemble lichen sclerosus (LS) and delay diagnosis. We report a case of a 69-year-old woman with long-standing vulvar symptoms and LS-like pallor of the labia minora and clitoris. Biopsy showed non-caseating granulomas with negative infection stains, consistent with vulvar sarcoidosis. This case supports the need for early biopsy in cases of persistent or atypical vulvar pallor and highlights sarcoidosis as an uncommon but important LS-mimic.

BACKGROUND/OBJECTIVES: Vulval lichen sclerosus (LS) is a chronic inflammatory dermatosis with recognised associations with autoimmune disease, including thyroid disease, vitiligo, and psoriasis. Current expert consensus guidelines recommend investigating for autoimmune disease when clinically indicated by symptoms or signs. More recently, focused screening for thyroid disease and pernicious anaemia in women with LS has been proposed. This study aimed to assess the prevalence of autoimmune disease in a tertiary multidisciplinary vulval clinic and to determine whether findings from this cohort p

The vulvar dermatologic examination is a crucial component of gynecologic and dermatologic practice, requiring a thorough understanding of normal anatomy and physiologic variations, and common pathologic conditions. This review outlines key elements of the history-taking process, including demographic factors, symptoms, product use, and relevant medical history, which help guide diagnostic evaluation. A structured approach to distinguishing normal anatomic variations from disease is emphasized, including the identification of physiological changes across the lifespan and common benign lesions
